The Cleveland Clinic Foundation Blood in the urine can originate anywhere along the urinary tract and both gross and microscopic hematuria may represent serious underlying disease. Gross hematuria is often alarming and will prompt the patient to seek medical attention. Likewise, the office practitioner may be faced with the incidental finding of asymptomatic microscopic hematuria. DEFINITION Hematuria is defined as the abnormal presence of red blood cells (RBCs) in the urine and is commonly divided into gross and microscopic hematuria. A patient information handout on microscopic hematuria, written by the authors of this article, is provided on page 1154. M icroscopic hematuria is defined as the excretion of more than three red blood cells per high-power field in a centrifuged urine specimen. Because the degree of hematuria bears no relation to the seriousness of the underlying cause, hematuria should be considered a symptom of serious disease until proved otherwise. Microscopic hematuria is defined as the excretion of more than three erythrocytes per high-power field, but the degree of hematuria does not correlate with the seriousness of the underlying cause of the bleeding. The widespread use of dipstick urinalysis in clinical practice and health screening has resulted in increased recognition of microscopic hematuria and has raised concerns about the appropriate diagnostic investigation. The prevalence of asymptomatic microscopic hematuria in adult men and postmenopausal women has been reported to range from 10 percent to as high as 20 percent. Routine screening of all adults for microscopic hematuria with dipstick testing is not currently recommended because of the intermittent occurrence of this finding and the low incidence of significant associated urologic disease.

What is hematuria?
Hematuria means blood in the urine. Microscopic hematuria indicates that the blood is only seen when the urine is examined under a microscope, while gross hematuria means that there is enough blood in the urine so that it can be seen with the naked eye. Despite the quantity of blood in the urine being different, the types of diagnoses that can cause the problem are the same, and the workup or evaluation that is needed is identical. Since blood in the urine must come from one of the organs involved in making or transporting the urine, the evaluation of hematuria requires that we consider the entire urinary tract. This organ system includes the kidneys, ureter (the tube that carries the urine from the kidney to the bladder), bladder, prostate, or urethra (tube leading out of the bladder). It must be emphasized that even a single episode of hematuria requires evaluation, even if it resolves spontaneously. What are the causes of hematuria?

What is Hematuria?
Hematuria is the presence of red blood cells (RBCs) in the urine. In microscopic hematuria, the urine appears normal to the naked eye, but examination under a microscope shows a high number of RBCs. Gross hematuria can be seen with the naked eyethe urine is red or the color of cola. Several conditions can cause hematuria, most of them not serious. For example, exercise may cause hematuria that goes away in 24 hours. Many people have hematuria without any other related problems. Often no specific cause can be found. But because hematuria may be the result of a tumor or other serious problem, a doctor should be consulted. To find the cause of hematuria, or to rule out certain causes, the doctor may order a series of tests, including urinalysis, blood tests, intravenous pyelogram, and cystoscopic examination. Urinalysis is the examination of urine for various cells and chemicals. In addition to finding RBCs, the doctor may find white blood cells that signal a urinary tract infection or casts (groups of cells molded together in the shape of the kidneys' tiny filtering tubes) that signal kidney disease. Excessive protein in the urine also signals kidney disease.

Hematuria
Hematuria simply means blood in the urine. The blood may be visible, so that the urine appears reddish or darker than normal. This is called gross hematuria. If the blood is invisible and is discovered only when a urine sample is examined in a laboratory urine test (urinalysis), the condition is called microscopic hematuria. Actually, hematuria is more a symptom than a condition in itself, because it has many possible causes. A urinary tract infection, kidney or bladder stones, an enlarged prostate in men, cystitis (a bladder infection, usually in women) or bladder, kidney or prostate cancer can all cause hematuria. Other causes include:

DESCRIPCIÓN La presencia de sangre en la orina o hematuria es la aparición de color rojo por la presencia hematíes (glóbulos rojos) en la orina. La presencia de sangre en la orina puede estar causada es ocasionada por problemas en el riñón o por enfermedades de las vías urinarias (uréteres, vejiga urinaria, uretra). A veces la causa es por contaminación de otras vías como puede ser el sangrado menstrual en las mujeres o por problemas de próstata en el hombre. En los niños puede aparecer la sangre por problemas generales como una alteración de la coagulación, hemofilia, trombopenias o malformaciones de la vía urinaria. En estos casos se debe de realizar un amplio estudio de las posibles causas. Una de la causas más frecuentes de la presencia de sangre en la orina es la infección urinaria o cistitis, que en las mujeres es muy frecuente sobre todo en el embarazo.
En estos casos la presencia de sangre en la orina se acompaña de dolor y quemazón o escozor al orinar, además de la sensación de tener ganas de orinar constantemente. En las infecciones urinarias del niño a veces solo aparece la fiebre como único síntoma.

    Peer Review Status: Internally Peer Reviewed Blood in the urine should never be ignored. This could be the first sign of a serious condition. In order to help your doctor with the correct diagnosis, you may try to provide details such as: Was it associated with pain? Did you see blood clots? What shape did the clots have? The color of the blood (brown, cherry, bright red, pink or orange) At what time during urination did you see blood in the urine (beginning or end of stream or during entire stream)? When passing blood in urine is associated with pain, especially abdominal pain, it could be associated with kidney or ureteral stones. The pain could be localized to the back, to the side, to the groin, or the penis in men and the labia in women. If you are passing different-shaped clots in your stream, they could represent bleeding from the urethra or prostate (in men). Clots can be wormlike, and if associated with pain it could represent clots coming from your ureters(tubes from your kidneys to your bladder). The color of the urine can be also affected by certain foods or medications. However, you should always consult your doctor if this develops.
