Medical Author: Jay Marks, M.D. What is Helicobacter pylori?
Helicobacter pylori (H. pylori) is a bacterium that causes chronic inflammation (gastritis) of the inner lining of the stomach in humans. This bacteria also is the most common cause of ulcers worldwide. H. pylori infection is most likely acquired by ingesting contaminated food and water and through person to person contact. In the United States, 30% of the adult population is infected. (50% of infected persons are infected by the age of 60.) The infection is more common in crowded living conditions with poor sanitation. In countries with poor sanitation, 90% of the adult population can be infected. Infected individuals usually carry the infection indefinitely unless they are treated with medications to eradicate the bacterium. One out of every six patients with H. pylori infection will develop ulcers of the duodenum or stomach. H. pylori also is associated with stomach cancer and a rare type of lymphocytic tumor of the stomach called MALT lymphoma.

Helicobacter Pylori
Mahahaavalla tarkoitetaan sekä itse mahalaukussa (ulcus ventriculi) että pohjukaissuolessa (ulcus duodeni) sijaitsevaa haavaa. Tauti on suhteellisen yleinen. Tiedetään, että noin 10 % ihmisistä jossakin elämänsä vaiheessa sairastaa mahahaavatautia. Toisilla tauti paranee muutamassa viikossa, eikä koskaan uusiudu, toisilla tauti on uusiutuva ja jopa jatkuva.
Taudin syyt
Mahalaukusta erittyvä suolahappo on se aine, joka syövyttää mahalaukun tai pohjukaissuolen limakalvoa ja aiheuttaa katarrin tai haavan. Kyseessä voi olla liiallinen suolahapon erittyminen tai limakalvon alentunut vastustuskyky suolahappoa vastaan. Suolahappoa lisäävät tekijät ovat mm tupakointi (yksi mahahaavataudin tärkeimmistä syistä), kahvi, stressi ja perinnölliset tekijät. Limakalvon vastustuskykyä alentavat mm ikä, eräät lääkkeet (varsinkin särkylääkkeet) ja alkoholi. Viime vuosina on huomattu, että eräs bakteeri, jonka nimi on Helicobacter pylori , sekä lisää suolahapon eritystä että vähentää limakalvon vastustuskykyä suolahappoa vastaan.

Helicobacter Pylori
Med magsår avses både sår i själva magsäcken (ulcus ventriculi) och i tolvfingertarmen (ulcus duodeni). Sjukdomen är vanlig, ca 10% av befolkningen har någon gång i livet besvär av magsår. Ofta går sjukdomen över på några veckor, för att aldrig sedan återkomma, men för andra kan sjukdomen flamma upp gång efter annan, eller till och med bli kronisk.
Vad beror magsår på?
Magsäcken utsöndrar saltsyra, som sedan irriterar och fräter på slemhinnan i magsäcken och tolvfingertarmen, och orsakar katarr eller sårbildning. Magsår får man antingen om man har alltför stor utsöndring av saltsyra, eller om slemhinnans motståndskraft mot saltsyran inte är tillräcklig. Faktorer, som ökar saltsyreutsöndringen är tobaksrökning (en viktig orsak till magsår!), kaffe och stress. Tendens till stor saltsyreproduktion i magsäcken kan också gå i släkten. Slemhinnans motståndskraft påverkas i negativ riktning av en del mediciner (speciellt värkmediciner, bl.a. acetosalicylsyra) och alkohol, och motståndskraften minskar normalt med åldern. Third of four articles on peptic acid disease Preview : Few areas of practice have been so quickly and thoroughly revolutionized by a single finding as has management of peptic ulcer disease by the 1982 discovery that Helicobacter pylori infection and antral gastritis are strongly related. Empirical therapy for the infection is not recommended; the authors compare several methods of establishing the diagnosis before starting therapy. They also discuss currently used combinations of drugs and who should receive them. Beliefs concerning the cause and treatment of peptic ulcer disease have undergone numerous changes over the past 75 years. In the early part of this century, the disease was thought to be related to stress and dietary factors, so treatment consisted of bed rest and a bland diet. Later came an understanding of the injurious effects of gastric acid, and treatment focused on neutralizing the acidic environment of the stomach with antacids and antisecretory agents. Although the introduction of histamine2 (H2) receptor antagonists revolutionized treatment of peptic ulcer disease, disturbingly high recurrence rates forced many patients to continue long-term acid-suppression therapy. Recent development of proton pump inhibitors added a more effective therapeutic option but did not solve the problem of recurrent disease.     Introduction
    The realization that H. pylori infection is the single most common cause of peptic ulcer has had an enormous impact on our approaches to the management of ulcer disease. The National Institutes of Health (NIH) Consensus Development Conference of 1994 and the American Digestive Health Foundation (ADHF) International Update Conference of 1997 recommended that all ulcer patients with H. pylori infection be treated for the infection. Today, there is no longer any serious debate about the value or appropriateness of such treatment that is based on overwhelming evidence from clinical trials reinforced by several years of clinical practice. Furthermore, there are enormous economic benefits from successfully treating the infection in ulcer patients. Successful eradication of the infection can be equated with cure of the ulcer disease in most individuals who are not exposed to any other known ulcerogen such as aspirin or a nonsteroidal anti-inflammatory drug (NSAID). Patients who are cured of ulcer disease will consume fewer health care resources. H. pylori

    Helicobacter pylori
    Helicobacter pylori is a spiral shaped bacterium. It lives on the gastric epithelium under the mucus layer of the stomach and duodenum. The bacterium is thought to damage this mucus layer, which is the stomach and duodenum's natural protection from gastric acids. Local inflammation caused by the bacterial infection and exposure to these acids can damage the lining of the stomach and duodenum, eventually leading to ulceration and possibly gastric cancer.
