Example of a calcaneal spur.
(provided by Dr. Terry R. Yochum)
What are heel spurs?
Heel spurs are small deposits of bone-like calcium on the heel bone (calcaneus), usually on or near the bottom of the foot where the long tendons of the arches (plantar fascia) insert into the bone.
What causes heel spurs?
Some experts believe that a spur is the body's attempt to strengthen its foundation. Being overweight, having abnormal foot biomechanics, or running regularly on hard surfaces can cause excessive stress on the foot, which may lead to the production of a spur. Other authorities contend that spurs are a normal part of the aging process: some studies have shown that calcaneal spurs are more frequently found in people after age 50. The best data currently available projects that between 11% and 16% of the population have heel spurs. It is also interesting to note that spurs occur almost twice as often in adult females as they do in adult males.

WHAT IS A HEEL SPUR? The heel bone is the largest bone in the foot and absorbs the most stress. A heel spur is an abnormal growth of bone either at the back of the heel, or most commonly on the bottom of the heel. The most common symptom of a heel spur is to have pain when first getting out of bed in the morning, or after resting for 20 minutes then getting up.
WHY DO WE GET HEEL SPURS OR HEEL SPUR SYNDROME? The most common reasons for heel spurs and heel pain is due to excessive stress and stains applied to the heel bone and the soft tissues structures that attach to the heel bone when we walk. This abnormal stress and strain is caused by Abnormal Pronation This is a direct by-product of our modern-day society, where many jobs demand long hours of standing on hard concrete, that are unforgiving on our feet. Over a period of years one of the major structures attached to the heel called the plantar fascia (this is the large rubber band structures that hold up your arch, and that goes from the heel to the ball of the foot) pulls away from the calcaneus (heel bone) and causes microscopic bleeding to occur.

Q: My problem is a painful heel and I've been diagnosed as having a "heel spur." Not much was told to me about how to treat it or get rid of it. I would appreciate your help. H.R.
Bottineau, ND
A: Heel spur syndrome is a very painful and sometimes debilitating inflammation of the plantar fascia, dense connective tissue that attaches at the heel and spreads through the arch of the foot, reattaching to the forefoot. There are two parts to the problem. One is the inflammation, called plantar fasciitis, and the other is the heel spur itself, a small piece of bone that grows out from the heel bone where the plantar fascia attaches. To complicate things further, it is possible to have heel spurs without symptoms, and to have plantar fasciitis without a heel spur. The goal of treatment is to reduce the inflammation with rest, anti-inflammatory medications such as aspirin or ibuprofen, and to protect the area with a heel pad. Cut a hole in the center of the heel in a sorbothane or felt pad under the sore area and wear it in your shoe to provide a cushion for your heel. If you have a high-arched foot, an arch support will reduce tension on the fascia. You should stop weight-bearing activity until you are pain free and can rub the heel area without pain. This may take four weeks to three months. Keep in shape by biking or swimming. Icing for 15 minutes at the end of the day will reduce swelling and inflammation. Massaging and stretching the sole of the foot and toes can also be very helpful.

As we age, our tendons and ligaments become less elastic and problems like plantar fasciitis (PF), an inflammation of the fascia running along the bottom of the foot, and heel spurs crop up more often. PF can take a very long time to heal and for a recovery you need to really take care of yourself. That means stretching your calves and feet several times a day. It means icing the painful area (try rolling your heel over a bottle filled with ice) taking anti-inflammatory medication and perhaps getting some physical therapy from your physician or a sports medicine specialist or podiatrist.
In addition, it is very important that you step into a supportive pair of shoes as soon as you get out of bed. Don't let those bare feet touch the ground in the a.m. as they tend to overstretch and re-tear tendons that were repairing (but shortening) overnight.

Last Updated: Niagara Chiropractic Society On Line Heel Spurs and Plantar Fasciitis "I thought chiropractors just treated backs and necks?" Just like your medical doctor has many tools such as a myriad of different drugs and surgery options, so does a chiropractor have many methods of treating conditions. Manipulation is one tool that chiropractors may use; but, not in this particular case. Many of the below therapies, such as custom-fitted orthotics, are provided by doctors of chiropractic. Plantar fasciitis is an inflammation of the plantar fascia. Some recent reports stress that degeneration rather than inflammation of the plantar fascia occurs. "Plantar" refers to the bottom of the foot. "Fascia" is tissue that surrounds muscle and often separates the skin from muscle or other deep tissue. It functions as a bowstring to maintain the arch of the foot by connecting the heel bone to the ball and toes. Plantar fasciitis is closely related to heel spurs, although the two are not exactly the same. Heel spurs are bony growths that result from too much tension in the plantar fascia. Heel spurs are the common result of repetitive trauma or tension in the plantar fascia for an extended period of time. A heel spur and/or the plantar fascia may "trap" or irritate nerves in the heel area and cause more pain. It is often difficult and usually unnecessary to distinguish between plantar fasciitis and heel spur syndrome. Causes and non-surgical treatments for the two conditions are the same. Many health-care providers use the terms "heel spurs" and "plantar fasciitis" interchangeably.

Randy, heel spurs effect over two and a half million people. And the pain can be excruciating. Now a new FDA approved treatment is bringing relief to many of those patients in pain. Whether you run marathons, play pro-basketball, or take an occasional walk through the park, you can develop painful heel spurs. Marjorie Greshy, Patient
"If you have this pain, it does limit everything you do. And if you're an active person or even if you like to walk, you can't do it." Majorie says the inflamed ligaments make it difficult to walk without pain. They often sideline her from activities she loves like golf and tennis. Treatment options usually include: physical therapy, heel cushions, steroid injections, stretching and ice. These options work for some patients, but not all. That's why Marjorie decided to try a new FDA approved shock-wave therapy, called the epos-ultra. Dr. George Theodore, Mass. General Hospital
"Shock wave therapy represents an exciting new alternative. Shock waves are generated from the device and targeted to the maximum point of inflammation in the foot. The ultrasound allows us to localize that area specifically and achieve maximum efficiency with the shock wave.

Although some experts distinguish between the terms plantar fasciitis and heel spurs, others do not, and the distinction is difficult to make, especially for a non-specialist.  We will use the terms interchangeably.  The heel bone, the largest bone in the foot, is known anatomically as the calcaneus. It is submitted to a great deal of shock and pressure, as it transmits weight from the body to the ground. Many people experience pain when they put weight on the heels of their feet, especially as they stand, run, or walk.  While there are a number of conditions that may result in heel pain, the most common one is known as plantar fasciitis. Sometimes bony growths, known as heel spurs, develop on the heel bone but having a bony spur may not be painful at all. The word plantar refers to the sole of the foot, and the plantar fascia is a broad band of fibrous tissue connecting the heel bone to the ball of the foot and the toes. The plantar fascia supports the medial longitudinal arch of the foot. Every time your foot hits the ground, the plantar fascia is stretched. Like the string of a bow, the tension on the fascia can therefore be very great. Because the fascia has limited elastic properties, repetitive stretching often results in microtears at its point of origin at the heel.

    How to Identify Heel Spurs
You don't have to be an athlete to develop heel spurs. Caused by an excessive bony growth on the heel, spurs can result in the plantar fascia a ligament that's attached to the foot to pull away from the heel bone. Knowing the symptoms of a heel spur will help prevent further injury to the heel. Steps: Suspect you may have a heel spur if you experience chronic pain in your heel. Especially if the pain occurs in the morning, when your feet first the floor. If heel pain eases after you've walked a bit, this can be a symptom of heel spur. Consider that you've developed a heel spur if you find you're walking on your toes or altering your stride to reduce heel pain. By changing your gait, you are easing the tug of your heel ligament, which, in turn, lessens heel pain. Palpate your heel. A heel spur will feel like a small, bony growth, at least .13 inches or longer, on the underside of your heel. The spur end tends to point towards the toes. If you feel a bony growth, you may have a heel spur. Ask yourself if the pain around your heel is centered in front of the heel, towards the arch. If so, you may have a heel spur.

    How to Treat Heel Spurs
Heel spurs develop when calcium deposits form on the heel. The cause is usually due to excessive stretching of the ligament of the heel, usually from over-pronation. Knowing the cause of the over-pronation makes treatment easier. Steps: Lose weight if your are overweight. Excess poundage puts extra pressure on the feet, and especially the heels. For chronic heel pain associated with spurs, weight reduction often helps reduce heel pain and swelling. Throw away your high heels, or at the least, wear heels sparingly. Heel spurs are much more common in women than in men because of the sytle of ladies' footwear. High heels cause excessive stretching of the heel ligament, and increase stress and trauma to the arch and heel. Wear shoes with good motion control to help stabilize the heel. Stretch before exercising. Gently warm up the ligaments of your feet and legs with slow and easy knee bends, toe curls and rotating the ankles in circles.
