Plantar fasciitis (pronounced PLAN-tar fashee-EYE-tiss) is an inflammation of the plantar fascia ("plantar" means the bottom of the foot, "fascia" is a type of connective tissue, and "itis" means "inflammation").
The plantar fascia encapsulates muscles in the bottom of the foot, but it also supports the arch of the foot by acting as a bowstring that connects the ball of the foot to the heel. It endures tension that is approximately 2 times body weight during walking at the moment when the heel of the trailing leg begins to lift off the ground. This moment of maximum tension is increased and "sharpened" (it increases suddenly) if there is lack of flexibility in the calf muscles. A percentage increase in body weight causes the same percentage increase in tension in the fascia. Due to the repetitive nature of walking, plantar fasciitis may be a repetitive stress disorder (RSD) not unlike carpal tunnel syndrome and tennis elbow. All three conditions benefit greatly from

Several layers of fatty tissue surround the heel bone, softening the impact or walking and running and protecting the bones and muscles of the foot. Beneath this padding, a fibrous band of connective tissue (the fascia) extends from the heel bone, supports the arch and reaches across to the toes. Pain can result when these tissues become irritated or inflamed, or when small spurs grow on the heel bone. Inflammation Most cases of heel pain are characterized by inflammation. First, the fascia begins to pull on the bone and the tissues become irritated, then inflamed. Inflammation of the fascia is called fasciitis.

A Monthly Foot Fact from Foot.com Teaneck, NJ (August 26, 2001) - Severe rear foot pain that accompanies the first step out of bed in the morning is a sign of plantar fasciitis, a foot condition often accompanied by a heel spur. A heel spur occurs when a calcium deposit forms on the heel bone, causing a bony protrusion. The heel bone is the largest bone in the foot, and absorbs the greatest amount of shock and pressure from walking, running and other activities. The bone is connected to the forefoot by the plantar fascia, a broad band of fibrous tissue located along the bottom surface of the foot. When the plantar fascia is stretched and pulls away from the heel, usually the result of over-pronation (flat feet), calcium deposits can form to fill in the gap, causing heel spurs. People with unusually high arches can develop the problem, and women have a significantly higher incidence of heel spurs than men because of the types of footwear they often wear, including high heels. Dancers or athletes who stretch the plantar fascia on a regular basis are also at risk. According to Dr. Suzanne Belyea, D.P.M., Medical Director of Foot.com, heel spurs themselves do not cause the severe pain. "When people have an over-stretched plantar fascia, it tightens up at night. The first step out of bed pulls it away from the insertion site, and the resulting inflammation of the plantar fascia is what causes the pain. Heel Pain A Greek myth tells the story of Achilles, who as a baby was dipped into the magical waters of the river Styx to make him strong. But the one part of Achilles that did not touch the water, the part by which he was held, was his heel, which would be his one weak spot and would later prove to be his downfall. Myth proves to be a reality to the many people who experience heel pain and problems. According to Running Research News, a million runners a year have moderate to serious heel pain, and the American College of Foot and Ankle Surgeons (ACFAS) reports that heel pain is a complaint voiced by 14 percent of the adult population and suspects that millions more silently cope with it. Adults, regardless of occupation or activity level, develop heel pain most frequently, although children can also be affected if the growing bone becomes irritated.

ANATOMY
The plantar fascia is a thick, broad, inelastic band of fibrous tissue that courses along the bottom (plantar surface) of the foot. It is attached to the heel bone (calcaneus) and fans out to attach to the bottom of the metatarsal bones in the region of the ball of the foot. Because the normal foot has an arch, this tight band of tissue (plantar fascia) is at the base of the arch. In this position, the plantar fascia acts like a bowstring to maintain the arch of the foot.

Typically, plantar fasciitis results from repeated trauma to the tissue where it attaches to the calcaneus. The result of the damage and inflammation is pain.
If there is significant injury to the plantar fascia, the inflammatory reaction of the heel bone may produce spike-like projections of new bone called heel spurs. The spurs are not the cause of the initial pain of plantar fasciitis, they are the result of the problem. Most heel spurs are painless. Occasionally, they are associated with pain and discomfort and require medical treatment or even surgical removal

I have been suffering w/ heel pain for 5 months now. It started in Feb 98' when me and a friend started walking 4 miles a day 3 days a week. I noticed the foot pain right away. (I just assumed my body had to adjust to the walk) but the pain got worse and I quit walking 3 weeks later. The pain is VERY bad in the morning. Both heels hurt but the left heel more than the right heel. I went to my family doctor and they gave me a pair of inserts for my shoes but they don't work. So I went to a foot doctor and he said it was heel spurs (He didn't do x-rays or nothing) he said, "lose some weight and do exercises that don't require me to stand up on my feet". I can't afford expensive shoes and gaggets. What can I do? Should I have the surgery? I'm in serious pain. My husband is in the military and now in Saudi for 4 months and I don't have no one to turn to for support! Thank you! Sharon Godwin :)
