ABOUT THE DISEASE What GERD is: Gastroesophageal reflux disease is a condition in which acid, bile and partially digested food in the stomach back up into the esophagus. Symptoms: Persistent heartburn and acid regurgitation, although some people with the disease do not have heartburn. Instead, they experience pain in the chest, hoarseness in the morning or trouble swallowing. It's not uncommon to feel like you have food stuck in your throat. GERD can also cause a dry cough and bad breath. Causes: It may result from a malfunctioning or abnormal lower esophageal sphincter muscle, a hiatal hernia, abnormal esophageal contractions or slow emptying of the stomach. Lifestyle factors include overeating, alcohol and tobacco abuse. Treatment: Lifestyle changes and medication.

Researchers at the Karolinska Institute in Sweden report in the New England Journal of Medicine (3/18/99) that if left untreated, chronic heartburn increases the risk of esophageal cancer eightfold; this is an especially disturbing finding given the deadliness of that form of cancer. A number of potent prescription and over-the-counter heartburn medications are available, and they may be effective. Safe natural remedies and simple changes in diet and lifestyle can also provide welcome relief. Helpful supplements
The supplements below can be very effective for relieving heartburn. Some (calcium carbonate, licorice, aloe vera juice, for example) are recommended for immediate relief of symptoms. To improve the entire digestive process, try either gamma-oryzanol, a natural extract from rice bran oil, or a combination of choline, pantothenic acid, and thiamin. Contrary to what its name suggests, the condition called "heartburn" is not a problem with the heart, but a problem with the digestive system of the body. It is just that the pain experienced in heartburn occurs so close to our usual notion of the heart's position, so much so that the source of the pain has often been misconstrued to be the heart itself. Heartburn is that uncomfortable, almost burning sensation often felt behind the breastbone, even climbing up to the level of the throat, after a substantial meal is taken beyond satiety. The condition may also be caused by other factors but overeating remains the most prevalent cause. The pain usually worsens when the sufferer lies down or bends over. This may distinguish it from pain originating from the heart, which, on the other hand, may persist with the same intensity regardless of position. The person may also begin belching, and a sour to bitter taste may accompany each belch. Sometimes, breathing becomes noisy and high-pitched, as the airway of a person suffering heartburn may constrict as a reaction to the noxious acidic juices going up the throat. In worse cases, airway constriction may cause a choking sensation. Heartburn is frequently referred to as acid indigestion. It is generally described as a burning feeling or pain in the chest, usually felt behind the breastbone that sometimes moves upward to the neck and throat. Heartburn may last as long as two to three hours and usually occurs, or is worse, after eating. Heartburn may occur after you eat a large meal or when you bend over or lie down. Smoking, caffeine and alcohol may increase the level of acid in your stomach, causing even more heartburn. While heartburn is typically not serious, this burning feeling which occurs usually behind the breast bone after meals, when accompanied by a sour-tasting fluid in your throat and difficulty swallowing, can be more serious. These are all classic symptoms of a common health problem called gastro-esophageal reflux disease or GERD. (This also is known as reflux disease.)

General Changes You Can Make
Lifestyle changes can make a difference in relieving your symptoms. Doctors usually recommend their patients try raising the head of the bed (about six inches) or sleep on a specially designed wedge. If you cannot raise the head of your bed, you can also slide blocks or books under the legs at the head of the bed. Tilting the bed is better than trying to sleep on several pillows, which can increase pressure or even make GERD worse. Raising the head of the bed and avoiding heavy lifting reduces heartburn by allowing gravity to minimize reflux of the stomach's contents into the esophagus.
Taking off any extra weight and quitting smoking are also recommended because both weaken the LES, (lower esophageal sphincter) and increase stomach acid production and irritation. You also may want to loosen your belt and avoid wearing tight clothing. Exercise is usually recommended to help keep weight off and increase overall fitness. Avoid lifting heavy objects or bending forward if possible. Avoiding foods and beverages that can weaken or take pressure off the LES is also recommended. It is important to avoid food close to bedtime (usually 2-3 hours). Making dietary changes and eliminating the following items from your diet may also help:

Heartburn results from the reflux of acidic stomach contents into the gullet. This happens, for example, when pressure on the stomach is too high or because the muscle at the lower end of the gullet which is designed to prevent reflux is not working properly. Sometimes stress can also lead to heartburn, due to excess acid production. Therefore:
  • Try to avoid putting pressure on the stomach as far as possible. Avoid becoming overweight, or lose weight if you are already overweight. Avoid drinks containing carbon dioxide. Do not wear tight clothes.
Generate as little gastric acid as possible or neutralize gastric acid
Do not eat large large meals, but take 5-6 small meals over the course of the day. Develop a preference for vegetarian foods (wholemeal bread, wholemeal pasta, whole rice, maize, pulses, potatoes, vegetables and sweet fruits). They contain dietary fibre, which helps to neutralize the gastric acid. Be kind to your stomach's mucous membranes
  • Drink coffee and strong black tea in moderation, i.e. no more than 2 cups a day.
