    Washington Parent Magazine The Meaning of Murmurs Gerard Martin, M.D. Your pediatrician tells you that your child has a heart murmur. You react with fear and surprise and are quick to ask, "What does this mean and is my child in danger?" This can be a very frightening situation for new parents because they believe a heart murmur is a sign of a very serious condition. However, about 90 percent of normal children in the first five years of life are noted to have murmurs. A heart murmur is simply an extra sound that the heart makes. A normal beating heart makes two distinct sounds. The first sound, or the "lub", occurs when the valves between the upper (atria) and lower (ventricles) chambers close. These valves close after the ventricles fill with blood and begin to contract. The second sound, or the "dub", occurs when the valves between the ventricles and major arteries close. These valves close after the ventricles have finished pumping. A murmur is a sound made by turbulent blood flow and is heard between the other, normal heart sounds. Murmurs can be innocent, normal sounds or pathologic, abnormal sounds. I usually tell parents to think of murmurs as the sound of water flowing through pipes after the faucet is turned on. The sound may be a normal flow, a leaky faucet or a blocked faucet.

    You're having a routine checkup when the doctor notices something while listening to your chest through the stethoscope. The doctor says you have a heart murmur, and she would like to check it out further.
    What Are Heart Murmurs?
    A heart murmur is an extra sound made by blood flowing through the heart valves in the different chambers of your heart. Normally, the heartbeat makes a "lubb-dupp" sound. The "lubb" occurs during systole with closure of the valves separating the upper and lower parts of your heart (the atria and the ventricles respectively). The "dupp" is heard during diastole when the valves close between the ventricles and the large blood vessels (aorta and pulmonary artery) that receive blood from the heart. often a "swish" sound occurs somewhere during the "lubb-dupp" of the normal heartbeat. With his or her training and experience in evaluating heart murmurs, by listening carefully to the timing and characteristics of the extra sound, your physician often can tell whether it represents a potentially serious problem.

    WHAT IS A HEART MURMUR? A heart murmur occurs when there is turbulence to the normal flow of blood through the chambers of the heart, and is usually due to a problem in a heart valve. When a valve is thickened or damaged and unable to close fully, some of the blood escapes around the valve. A vet can hear a squishing sound which confirms that blood is leaking out of the heart valves during contraction. Murmurs are usually seen in older cats but it is not uncommon to see them in a young kitties. In fact, many kittens are born with murmurs and outgrow them later in life. A lot of cats with murmurs live long healthy lives. There is a small risk that murmurs can lead to congestive heart failure. When older pets have a valve related murmur, the incomplete valve closure is often due to bacterial deposits on the valves. A very common source of this bacteria is dental disease, so it's important to keep your kitty's teeth and gums in good condition.

    The table below describes 10 characteristics that can be described regarding each heart murmur. Most problems arise when trying to describe the shape: judge variation of intensity of the murmur; it may build-up(crescendo), rise and then fall (crescendo-decrescendo), decrease from a loud onset (decrescendo), or remain relatively constant (plateau).

    INNOCENT MURMURS.
    SUPRACLAVICULAR SYSTOLIC MURMUR.
    This murmur begins after the first heart sound and is best heard underneath the right clavicle. The murmur generally is grade II but can be grade III and usually is of short duration, occupying less than one half of systole. Murmur frequency is low and is diamond shaped with a somewhat noisy quality. The murmur radiates towards the neck particularly on the right. Respiration has no effect. The murmur intensity decreases significantly when the child sits with the shoulders hyperextended by bringing the arms behind the patient. This murmur is caused by turbulence in the major brachiocephalic arteries as these vessels arise from the aorta. Differential diagnosis is seldom a problem; the murmur of aortic valve stenosis may come to mind, but can easily be distinguished by its specific characteristics as well as the presence of an aortic ejection sound.

    My daughter, who is 39 years old, was told by her physician at her last yearly exam that she has a heart murmur caused by a faulty valve. The only advice he gave her was to treat a strep throat seriously, if she would have one. I am concerned about this and feel it should be looked at more closely. I, myself, have had an artificial heart valve since 1983, and I would like to take her with me on my next appointment to my cardiologist. What is your opinion? A: When listening to the normal heart with the stethoscope, there should be two sounds"lub dub." These are the sounds made when the 4 heart valves close: the tricuspid and mitral close at the same time making the "lub" sound, and the pulmonary and aortic close during the "dub" sound. Murmurs are swishing sounds created when there is a narrow opening because the valve is scarred or diseased and the blood has to force its way through, or when the valve doesn't close tightly and blood is leaking backwards. There can also be innocent murmurs, especially in children, when an extra sound is heard, but the valve is really fine. People can also have murmurs that remain unchanged for years and never lead to heart problems or require valve surgery.

    The Question: I just took my 10 week old kitten for her first exam and vaccinations and was told that she has a significant heart murmur (rated a 5 on the 1-6 scale). The doctor said she was probably born with it and could live a few months to a few years. I can't afford a lot of expensive tests and treatments but want to keep her as healthy and comfortable as possible. At this time she shows no signs of ill health and has as much energy as her brother. What kinds of symptoms should I watch for and what kind of prognosis are we looking at? The Answer Heart disease carry a certain set of signsThe animal usually is not very activeand breathes hard when excessively stressed or exercised. They may retain fluid and feel puffy. May times, if you watch closely, you you can see the jugualr vein pulsing in their neck (depending on the type of murmur). The cat's membranes may be bluer than normal and the heart rate is faster. Sometimes the murmurs kittens have close up and get better as they get older. They are usually less than grade 5but if is possible that your kitten may get better with time. If not the signs above will get worse until the kitten goes into CHF and you lose it.
