Murmurs of the Heart A heart murmur is a sound produced as blood flows through the chambers and large blood vessels of the heart during the cardiac cycle of contraction and relaxation. Some murmurs are benign or harmless and are more of a finding than a condition. A benign murmur is not associated with any significant underlying abnormality of the heart or its vessels. Many young people can have benign flow murmurs and still have normal cardiac structure and function. A pathologic heart murmur is one associated with a structural or functional abnormality of the heart. Most murmurs are produced as blood flows past the cardiac valves, which separate the chambers of the heart, or through the valves that lead to the great vessels of the lungs and the systemic circulation. If the flow is excessive or turbulent, a murmur may be manifest. Blood flowing through a tight valve will produce a murmur. Blood that is leaking back across an improperly sealing valve also can cause a murmur. Occasionally, abnormal communications (holes) between chambers of the heart can result in the presence of a murmur. Diagnosing a heart murmur begins with auscultation of (listening to) the heart-usually with a stethoscope. The location, quality, pitch and variation in the sound are all important clues to whether the murmur is benign or pathologic. One of the most useful tests in evaluating a murmur is an echocardiogram. This is a sound wave test that produces a real-time image of the beating heart and is very good in assessing the status and function of the cardiac valves and chambers. Sometimes further studies, such as cardiac catheterization, will be required to further track down the specific abnormality and its significance, and to determine which cases will require treatment.

What is a heart murmur?
A heart murmur is an abnormal sound caused by turbulent blood flow. They can occur as a result of any various heart valve defects or type of congenital heart disease. The first normal sound of the heart results from the closure of the tricuspid and mitral valves at the exits from the upper chambers of the heart. The second normal heart sound is caused by the closure of the pulmonary and aortic valves at the exits from the ventricles when they have finished contracting. With a stethoscope the second heart sound is heard to consist of a double sound, especially in children and young adults. This is caused by a slight delay between the closure of the aortic and pulmonary valves. This is a normal sound. In children and young adults that is often a normal low pitched third heart sound after the second sound. But in people over the age 40, this sound is abnormal and a sign of heart failure. bodyOffer(5688) If there is any abnormality of the heart muscle a fourth sound that is low pitched will often precede the first sound. An opening snap is also abnormal and may occur during the opening of the mitral valve. This is a high pitched sound that is heard shortly after the second heart sound and may be accompanied by a presystolic murmur. This is associated with mitral stenosis. An ejection sound or click is a high pitched sound caused by the abrupt halting of the valve opening. These are known to occur in hypertension or high blood pressure and in the heart valve defects such a mitral valve prolapse, aortic stenosis or pulmonary stenosis. Any abnormalities of the heart valves or heart structures can create turbulent blood flow that causes characteristics sounds that are called murmurs. Turbulent blood flows usually occur as blood moves through narrowed or leaking valves. Research has shown that not all heart disease will cause murmurs and not all murmurs indicate heart disease.

The heart normally makes a recognisable sequence of whooshing and clicking sounds as blood is pumped through its chambers and valves open and close. Heart murmurs are abnormal sounds coming from the heart that can be heard using a stethoscope (or sometimes even just by listening with an ear at the chest.) What causes it? Heart murmurs are very common in babies and young children. The majority are not a sign of a problem with the heart but simply result from the fast rate at which children’s hearts beat, or normal variations in the structure of the heart. Some factors, such a fever or excitement, make an innocent murmur more likely because they increase the speed at which the heart pumps. But a heart murmur is also the commonest sign of congenital heart disease, due to abnormal development of the heart tissues. This includes atrial or ventricular septal defects (“hole in the heart”, or abnormal hole between two heart chambers), patent ductus arteriosus (a connection between vessels from the heart which should close after birth but doesn’t), abnormalities of the heart valves and a variety of other rarer problems. Heart murmurs may also be caused by heart failure and infection involving the heart.

What is a heart murmur?
A heart murmur is simply an extra sound heard when listening to the heart with a stethoscope. The normal sounds a heart makes are from the valves closing. If there is any turbulence in the blood flow through the heart or major blood vessels around the heart, a murmur is heard. Return to Top
