Why We Must Get It Right

CHF is being described as the next epidemic. Five million Americans have heart failure and up to 700,000 new cases are diagnosed every year. This illness causes one million hospitalizations and has a part in 250,000 deaths every year in the USA alone. Heart failure is the only heart condition actually on the increase.
Medicare spends more on CHF than it does on all forms of cancer combined. The yearly direct cost of CHF care is anywhere from 10 to 38 billion dollars US. Most of this cost comes from hospital stays for acute, or

Congestive heart failure is the term used to describe the inability of the heart to pump as much blood as the body needs. Its symptoms are widespread weakness, shortness of breath, swelling in the legs, abdominal discomfort or pain, and a loss of appetite. Despite its pronouncement-of-doom sound and the wide range of symptoms, congestive heart failure is generally a treatable condition although calling it a "condition" is not exactly accurate. Congestive heart failure itself is a symptom of some underlying condition which is depriving the heart of the strength to meet the body's metabolic demands. The most frequent cause of the condition in older patients is atherosclerosis the progressive narrowing of the heart's own arteries by cholesterol plaque buildups, which starves the heart itself for oxygen and nutrients. In younger patients, it is more likely to be from a faulty heart valve or from cardiomyopathy damage to the heart muscle from an infection or other cause. The task of the cardiologist is to find and treat that underlying cause. In the case of atherosclerosis, balloon angioplasty to reopen the blocked arteries or bypass surgery to route blood around the constriction will often relieve the symptoms of congestive failure. In valve defect cases, valvuloplasty replacement of a defective valve may be needed. Cardiomyopathy may be addressed with medication, though, in extreme cases, the diseased heart may be incapable of recovering, and heart transplantation may be indicated.

New hope for heart failure patients
Arthritis drug may slow disease progression
HOUSTON, Texas (CNN) A genetically engineered protein called Enbrel, approved last year for the treatment of rheumatoid arthritis, is continuing to show promise against another threat congestive heart failure. The leading cause of hospitalizations for older Americans, congestive heart failure is a progressive weakening of the heart muscle that robs the organ of its ability to pump. It affects 4.5 million people in the United States and some 400,000 new cases are diagnosed each year.

The term heart failure is frequently misused, especially when given as cause of death: it is not synonymous with " cessation of heartbeat There are many different ways to categorize heart failure, including the side of the heart involved (left heart failure vs. right heart failure) or whether the abnormality is due to contraction or relaxation of the heart (systolic heart failure vs. diastolic heart failure). Individuals with heart failure are sensitive to small shifts in their intravascular volume status (the amount of fluid in their circulatory system ). Increasing the volume in their circulatory system can cause symptoms and signs of decompensated heart failure, while decreasing the volume in the circulatory system can cause hypotension Signs of decompensated heart failure include pulmonary edema (fluid building up in the lungs)

Answer Heart failure is a clinical syndrome (a collection of symptoms) which has a number of different causes. It is an abnormality in the mechanical function of the heart muscle which results in inadequate cardiac output. Cardiac output can be thought of as the amount of blood which is pumped by the heart to supply oxygen and other nutrients to the muscles and vital organs. It is caused by diseases of the heart valves, coronary arteries or the heart muscle (myocardium) itself. So, it will be found in ischaemic heart disease(see Ischaemic heart disease ), long term high blood pressure causing the left ventricle (chamber) of the heart to enlarge, and in diseases in which the heart valves are damaged. Disturbances in heart rhythm can also cause heart failure. There are a number of other conditions which can result in heart failure such as anaemia, vitamin B1 defficiency (Beriberi), hyperthyroidisim, pregnancy, and some types of liver disease.

CHAPEL HILL, North Carolina (CNN) Women with congestive heart failure can live twice as long as men with the same disease, according to a study released Monday. Researchers said the cause is most likely a biological difference between the sexes: Women's heart muscles respond better to some types of heart disease, such as high blood pressure. The study, reported in the American Heart Association journal Circulation, focused on patients with advanced heart failure. "What we think may be going on is there are biological differences between men and women that effect the way they respond to the heart failure state," said lead researcher Dr. Kirkwood F. Adams Jr., a professor of medicine and radiology at the University of North Carolina.

The tool kit may be available for download after the first phase of HF GAP is completed in December 2003. The project interventions and Tool Kit were presented to the first round of cardiology participants and nursing staff in a kick-off event held May 31-June 1, 2002 in Sunriver, Ore. The Heart Failure GAP registry was also presented at the meeting. The registry is a database for tracking individual patient care and population management. Project participants are encouraged to use the registry or a similar system for data collection and reporting. The kick-off event was the first ACCF-CME accredited GAP event.

Web posted at: 4:51 p.m. EDT (2051 GMT)
NASHVILLE, Tennessee (CNN) Almost five weeks after the body of country legend Tammy Wynette was exhumed at the insistence of her daughters, medical examiners say the results of an autopsy show that Wynette died of heart failure. "There were no blood clots in her lungs in the autopsy, or under microscopic examination," says Davidson County medical examiner Dr. Bruce Levy. "But there was significant evidence of previous blood clots which compromised her lungs and put pressure on her heart." Wynette, 55, died in April 1998 of what her personal physician, Dr. Wallis Marsh, diagnosed as a blood clot in her lungs. Such clots, along with intestinal ailments, were among several health problems suffered by the "First Lady of Country Music" in her last years. Levy says two drugs were found in Wynette's blood, both of them medications Wynette was known to be taking at the time of her death. But he added that there was no evidence of a third drug, Dilaudid, a painkiller known to dissipate in embalming fluid.
