It is estimated that 75% to 90% of all persons who complain of chronic or frequent headaches suffer from tension headaches. They are the most common type of primary headache, and while they share some characteristics with the more serious migraine, they also display certain distinct differences that set them apart. Some experts believe that tension headaches and migraines represent two ends of a common spectrum, and that migraines, which are severe but irregular in occurrence, sometimes progress or transform to the less severe, but more frequent, tension-variety headache. Physical Findings When a patient is examined by the physician, there are often findings of muscular tenderness. This is often present in the areas of the neck, at the base of the skull, shoulders, upper arms, and the jaw and face. Some people may show signs of clenching the teeth. The scalp and forhead may also be painful when palpated. The neurological examination is usually normal.

Also known as muscle contraction or chronic daily headache, this is the most common variety of headache. Most people will have suffered from tension headaches at some point in their lives and never seek medical assistance but for a small minority of patients the condition is severe, long-standing and may be difficult to treat. What causes tension headaches and who is at risk? It was long believed that tension headaches were due to overcontraction of the muscles covering the scalp and back of the neck. However, all types of chronic (prolonged) headache may give rise to increased muscle tension and individuals with tension headaches often describe the continuous squeezing discomfort even when completely relaxed. Despite this condition being one of the most common to affect humans, surprisingly little is understood of the exact mechanisms giving rise to the symptoms.

There are recurring discussions of the use of psychedelics, most often psilocybin and LSD, in the underground and personal treatment of migraines and cluster headaches. Cluster headaches (also called Suicide Headaches) are a specific variety of extreme, recurring headaches and are classified with migraines. A good overview of Cluster headache symptomology can be found at clusterheadache.com . Very briefly, Cluster headaches are cripplingly painful headaches which occur in clusters or periods where the sufferer experiences them at a high frequency, sometimes several per day, over the course of weeks or months, then a period of no headaches, then another cycle of pain.
Erowid has received several reports of migraine sufferers who have used low doses of LSD to treat migraines and chemicals structurally similar to LSD are commonly prescribed for migraine treatment, but more recently data has begun to accumulate that psilocybin may successfully reduce the incidence of cluster headaches for weeks or months and may also reduce the pain during an attack.

Spinal Manipulation vs. Amitriptyline in Chronic Tension Headaches
This randomized controlled trial compared the effectiveness of spinal manipulation and amitriptyline for the treatment of chronic tension-type headache. This study consisted of a 2 week baseline period, a 6 week treatment period and a 4 week post-treatment follow-up period. Of the 150 patients who were enrolled in the study, 24 (16%) dropped out, 5 (6.6%) from the spinal manipulative therapy and 19 (27%) from the amitriptyline group. During the treatment period both groups improved at very similar rates in all primary outcomes.
In relationship to baseline values at four weeks after cessation of treatment, the spinal manipulation group showed
  • a reduction in headache intensity,
  • a reduction in headache frequency,
  • a reduction in over-the-counter medication usage, and
  • a improvement in functional health status.

The amitriptyline group showed improvement from baseline values in the same four major outcome measures of 6% or less
Controlling for baseline differences, all group differences at four weeks after cessation of therapy were considered to be clinically important and were statistically significant. This sustained therapeutic benefit may reduce the need for self-administered analgesic medication. There is a need to assess the effectiveness of spinal manipulative therapy beyond four weeks and to compare spinal manipulative therapy to an appropriate placebo such as sham manipulation in future clinical trials. What You Should Know About Tension-Type Headache
The most common type of headache is tension, or muscle contraction, headache. While it is estimated that 90 percent of all adults have experienced tension-type headache, women are more frequently diagnosed with chronic tension headache than men. It is not clear whether this is due to a difference in prevalence or because of the fact that women in general are more likely to seek medical treatment. Tension-type headache is characterized by a constant, dull pain or feeling of uncomfortable pressure in the neck, temples, forehead, or around the head in "hatband" fashion. Sometimes the neck feels very tight. Tension-type headache usually occurs bilaterally (i.e., on both sides of the head at the same time). Loss of appetite may occur with tension-type headache, but nausea and vomiting usually do not. However, some people who experience migraine and tension-type headache may have nausea and vomiting with both. Sensitivity to light or noise may occur with tension-type headache, but generally they do not both occur together.
