Halitosis can be very misunderstood. Is it a result of oral conditions such as periodontal (gum) disease, lack of tongue cleaning or is it more than these things? The answer can be complicated for some people. The following table is an attempt to describe the major types of halitosis most often found - Types of halitosis (bad breath) A. Genuine Halitosis 1. Physiologic halitosis
  • Obvious bad odor No specific disease cause nor pathologic condition that could cause halitosis is present. The main cause is bacteria on the mid to back of tongue.
2. Pathologic halitosis
  • Bad odor caused by disease or pathologic condition.
Originates from nasal or throat area. Originates from lungs Originates from digestive tract. Originates from disorders anywhere in body - odor is blood borne and emitted via the lungs. B. False-halitosis

Halitosis Treatment in the Dental Office
Halitosis, and the treatment of this problem, is quickly becoming the growth area of the 90's in dental practices throughout North America. Oral malodor is a very pervasive problem, and can be simply an embarassment to some, or the sign of potentially serious systemic problems for others. Most of us as dental professionals have received little, if any, formal training in the management of this problem. Professionals are being drawn to establish this treatment service within their offices for several reasons. Perhaps the primary reason is the power of this concept as a marketing tool. It is an excellent way of attracting new patients, particularly those who are not regular users of dental services. The treatment of halitosis is often viewed as a "non-dental" procedure by the public, and so does not carry the psychological impact of other dental treatment. This allows us to get these people comfortable with the dental environment in a non-threatening fashion. Amongst our existing patients it is a strong motivator for the acceptance of dental treatment, particularly in the area of periodontal disease, which is known to be a major source of halitosis. Finally, we now have available the means to quantify this problem, and some definitive means to treat the problem, once properly diagnosed. It is generally known and agreed that VSC's(Volatile Sulfur Compounds) are responsible for the odor, and that 80-90% of these come from oral sources. VSC's are produced by bacterial and cellular degradation, and include hydrogen sulphide, methyl mercaptan, and dimethyl sulphide. The fact that oral odors can originate from sources other than the oral cavity, demands that a proper diagnosis be done, requiring a thorough exam and history. It is known that systemic disorders, medications, and ENT problems can all contribute to halitosis.

  • Bad breath that is of oral cavity origin can be traced to a sulfur compound produced by bacteria. Dead and dying bacterial cells release this sulfur compound which gives the breath an unpleasant odor.
  • Bacterial plaque and food debris accumulate on the back of the tongue. The tongue's surface is extremely rough and bacteria can accumulate easily in the cracks and crevices. Large amounts of sulfur compounds can be produced in this area, making it a frequent site of origin for bad breath.
  • The tooth attracts bacteria containing plaque and if not cleaned regularly and thoroughly, this can result in large accumulations of bacteria which result in bad breath.
  • People who have periodontitis often experience bad breath because of bacteria accumulating in areas that are not cleaned easily, such as deep pockets around teeth.

The complaint of bad breath is unusual in children and the cause should be uncovered and dealt with directly. When your child's breath has an unpleasant odor it could be caused by food, dental problems, or by another underlying problem. The problem may be recent or a long-standing one.

Today's Answer (Published 05/29/1997) (Published 11/11/96) The usual cause of bad breath is bacteria growing on the tongue, and sometimes around the gum lines, too. There are a couple of simple ways to take care of the problem. First, try a tongue scraper. This is a metal instrument that you use to scrape your tongue once or twice a day, cleaning off bacteria. Second, you can brush your tongue with a germicidal toothpaste when you're brushing your teeth. Just take an extra 30 seconds to brush your tongue after you're done with your teeth. One such product that works well is chlorine dioxide, which is in some regular toothpastes (for instance, Oxyfresh). Or, use a toothpaste containing tea-tree oil. The oil is nontoxic and extracted from the leaves of Melaleuca alternifolia

What is halitosis?
Halitosis is an oral health condition characterized by consistently odorous breath. Often identifying the cause of bad breath is the first step toward treating this preventable condition. What causes bad breath?
There are just as many causes of bad breath as there are sources of bacteria in the mouth. Halitosis may be caused by the following:
  • certain foods
    The things you eat contribute largely to your oral health care, including your breath. Items such as garlic and onions, or any food, are absorbed into the bloodstream, become transferred into the lungs, and become expelled in the air you breathe. Until that food becomes eliminated by the body, the potential for it affecting a person's breath is present. poor oral health care
    Without proper and consistent brushing and flossing, and routine examinations by your dentist, food remains in the mouth, which is a breeding ground for bacteria, which, in turn, causes odorous breath. Food that is allowed to collect on the teeth, gums, and tongue may cause the gums to rot, which causes an unpleasant odor and taste in the mouth.

    Halitosis? Halitosis, more commonly known as bad breath, is a condition that affects all people at some time. However, halitosis affects some people all of the time.
    What causes halitosis?
    The primary cause of bad breath is the Volatile Sulfur Compounds (VSCs) that are produced by the bacteria in your mouth. VSCs are a waste product of anaerobic bacteria and account for 95% of halitosis. There are two types of bacteria in the mouth:
    Aerobic (good)
    Anaerobic (bad)
    If you do not brush and floss daily, particles of food remain in the mouth and lodged in between the teeth. These food particles on the tongue, between the teeth, and near the gum line promote bacteria growth and emit an unpleasant odor. Dental appliances, such as: dentures, retainers, mouth guards, etc. can also harbor odor causing bacteria and food particles.
    Persistent bad breath can also be a warning sign of periodontal or gum disease. Periodontal disease is caused by plaque; the soft, sticky, colorless bacterial film that grows on the hard surfaces of the teeth. Left to accumulate, this acid destroys the outer enamel of the tooth, irritates the gums to the point of bleeding, and produces foul breath. With regular dental visits, your dentist can detect and treat periodontal disease early.
