Gynecomastia in Haitians Puerto Rico, Florida, Texas, New York
Since the fall of 1981, gynecomastia has been occurring in Haitian men located in the Immigration and Naturalization Service's (INS) Service Processing Centers. Although no cause has been found, the condition appears to be spontaneously resolving. In November 1981, several Haitian men at the INS Fort Allen Service Processing Center, Puerto Rico, presented at the Public Health Service Clinic complaining of breast enlargement. Because of this unusual occurrence of gynecomastia, CDC was asked on November 30, to assist in an epidemiologic investigation. Of 540 male Haitians 18-50 years old examined, 77 (14.3%) had gynecomastiadefined as a palpable, firm, discoid, subareolar, unilateral or bilateral breast mass. Of these 77 persons, 11 had noted the problem in Haiti, and 5 in Miami. Of 187 non-Haitian, male employees of the Center examined, 6 (3.2%) had gynecomastia, and of these, 2 had the problem before the facility opened. The difference in prevalence between Haitians and Center employees was statistically significant (p 0.0005). After the disorder was recognized in Fort Allen, cases of gynecomastia were found in Haitians in other INS facilities, including the INS Service Processing Center, El Paso, Texas, and the Krome North Service Processing Center, Miami, Florida. Because all Haitians entering the United States and processed by INS are first processed at Krome, further investigation was continued there. On January 4, 1982, examination of the entire Haitian male population at Krome for gynecomastia was completed. Of 522 male Haitians, 52 (10.0%) had gynecomastia. Of these 52, 2 reported to have had the disorder before leaving Haiti. One of 10 non-Haitian aliens processed at Krome also reported the recent development of gynecomastia.

Follow-Up of Gynecomastia among Haitian Males
The following account updates the recent report of gynecomastia among Haitian male entrants located at the Immigration and Naturalization Service's (INS) Service Processing Centers (1). Current findings continue to indicate a correlation between the date of entrant arrival and spontaneous resolution of gynecomastia. The prevalence of gynecomastia at the INS Fort Allen Service Processing Center, Puerto Rico, has decreased from 77 (14.3%) of 540 examined in December, 1981, to 47/508 (9.3%) in May 1982. On May 11, 1982, 66 of the 77 Haitian males with gynecomastia detected at Fort Allen in early December 198l were re-examined; 46 patients (69.7%) no longer had evidence of gynecomastia, and six had decreased breast size. Therefore, of the originally detected cases, 52/66 (78.8%) have either totally or partially remitted. Most of these cases occurred among Haitians who had arrived in the United States by late August 1981. In addition, 10 of 12 Haitian males with gynecomastia detected at Fort Allen after their transfer from the El Paso, Texas, and Otisville, New York, Service Processing Centers between late December and early March were examined on May 11; nine (90%) no longer had evidence of gynecomastia. Twenty-one new cases were detected at Fort Allen among Haitian males who had had no evidence of gynecomastia when examined in December. Of the 52 Haitian males transferred to Ft. Allen from El Paso and Otisville, six (11.5%) now have gynecomastia.

Loin Mass, Back Pain and Gynecomastia

(Special Contributor) Case History: On examination, he displayed predominantly left-sided facial acne with patchy hair loss and generalized edema, particularly evident in the ankles. The abdomen was distended with a palpable left-loin mass. Examination of the endocrine system showed gynecomastia with testicular atrophy. Orthopedic examination revealed a tender thoracolumbar junction with no neurological deficit in the perineum or lower limbs. Investigations: Plain X-Rays were obtained of the thoracolumbar spine, and showed collapse of the vertebral bodies of T9 - L1 consistent with metastatic disease. CBC, Ca, BUN, creatinine, K, TFTs, and serum electrophoresis were normal. Alkaline phosphatase and ESR were slightly elevated. A 24-hour urinary cortisol was markedly elevated on two occasions at 2010 and 1265 compared to the normal range of 140 - 305 nmol/24h. In order to show the extent of bony metastases, in particular to assess the possibility of spinal stabilization, a bone-scan was performed. This showed abnormal activity at T6 - L2, as well as multiple "hot spots" in the rib cage (Figure1). Subsequent abdominal CT scanning revealed a well encapsulated mass of mixed density containing calcification, and blending intimately with the upper pole of the left kidney. It was adherent to the spleen, and displaced the pancreas anteriorly. The lesion was consistent with an adrenal neoplasm (Figure 2). A biopsy was performed which showed a vascular neoplasm containing pleomorphic cells with granular pink and occasionally clear cytoplasm and marked nuclear pleomorphism. Special staining characteristics favored a diagnosis of adrenocortical carcinoma.

Gynecomastia (male breast enlargement)

Male breast enlargement, usually diagnosed as gynaecomastia, has been reported since 1998 in men receiving highly active antiretroviral therapy (HAART). Although case reports are rare, its prevalence has not been established, and its causes are unclear. Whilst some researchers believe it is a side-effect of HAART, others believe it has more complex causes.

The term breast can refer to the upper ventral region of the human torso. Alternatively the term is used for each of two parts of that, especially for women: the breasts are parts of the female human body that contain the organs that secrete milk used to feed infants. Males also have breasts and are born with the main milk ducts intact, but while the gland that produces milk is present in the male, it normally remains undeveloped. In some situations male breast development does occur, a condition called gynecomastia. Milk production can also occur in both men and women as a rare side-effect of some medicinal drugs (such as some antipsychotic medication). Both sexes have a large concentration of blood vessels and nerves in their nipples.

The prognosis of HIV infection has improved dramatically since the introduction of HAART. However, numerous adverse effects and limitations regarding tolerability remain a concern. Lipomastia (pseudo-gynecomastia), a breast enlargement due to central adiposity, may occur as part of a fat redistribution syndrome that has been associated with HAART regimens. Several pathogenic mechanisms have been advocated in its development. In the current observational longitudinal study, investigators report on five patients diagnosed with gynecomastia associated with efavirenz ( Sustiva )-based HAART regimens.
  • All cases reached successful immunologic and virologic responses to HAART. The delay of appearance of gynecomastia from the beginning of HAART ranged between 4 to 15 months. In all five cases, gynecomastia regressed after efavirenz withdrawal (mean period of 5 months).

I recently developed a small bump under my left breast. I have been on sustiva, abacavir and d4T. I have recently switched to Sustiva, AZT and Abacavir. I went for an ultrasound and a specialist who was going to to a biospy and decided that I had gynecomastia. The bump is noticeable on the touch and has not gone away and appears to have gotten a bit bigger. What do you think has caused this/or which of the drugs can cause this and will it resolve on it's own as the specialist has advised me it would? Or do you think the drug switch from Zerit to AZT would help this condition? Should I request more tests or have it drained? Your help as always appreciated!!
    It is interesting that you have not indicated if you are male or female, young or old, or many of the other characteristics that might help me make a more "educated" guess about what's up. It is also interesting that when you have a concern about something unusual going on with your body you seem to have decided it is the HIV meds that must have caused it. The bump was evaluated by a specialist who said that you do not need a biopsy, so it is most probably not cancerous. But, since it has grown, I would suggest that you go back to the specialist (what kind of specialist?) and ask them to re-check.

Hi Nelson - can you help me with this? I switched d4t to AZT to complement Sustiva and Abacavir. I have this lump on my left breast which is called gynecomastia and my doc says it's from fat redistribution It really bothers me and I wonder what treatment there is for this - my doctor assures me it will resolve but it is getting bigger. Your thoughts and suggestions are much appreciated. Thanks
    It is difficult to tell sometimes if you have an enlargement of the breast gland tissue (gynecomastia) or just fat deposition in that area (have your doctor retrieve one of the papers listed at the bottom) Sometimes gynecomastia goes away by itself. Sometimes taking estrogen blockers like Arimidex or Nolvadex (Tamoxifen)helps. Some doctors have succesfully used DHT (dihydrotestosterone) gels to be rubbed on the area (you can get it at gulfsouthrx.com). And if nothing else works, surgery is needed. There are three reports of Sustiva causing this problem (we do not know for sure since gyno has also been observed with protease inhibitors). I am enclosing the studies for you to read, print, and bring to your doctor for more discussion. Good luck! Tamoxifen in antiretroviral-associated gynaecomastia.

Though it doesn't happen to everyone, excessive male breast enlargementmedically known as gynecomastiais a fairly common potential side effect of heavy marijuana use. * That mellowness you feel when you smoke pot is partially caused by a reduction in circulating levels of your male sex hormones, also known as androgens. This can lead to breast enlargement, weight gain, and decreased penis size. These side effects are most common in males younger than age 20, because the body's growth and development is very sensitive to hormonal fluctuations during puberty. ** Heavy regular marijuana use later in life can cause male breast enlargement too, but it is somewhat less likely. Also, for female readers, it's worth mentioning that marijuana use doesn't have the same effect in women. Smoking pot won't increase a woman's breast size. If you're already suffering from male breast enlargement, you should see a doctor. You are not alone many people (not just pot smokers) have this problem, and there are effective hormonal and surgical treatments.

Gynecomastia in men and breast hypertrophy in women appear to be other problems associated with potent antiretroviral therapy. Although reports of HIV-related gynecomastia occurred before the widespread use of HAART,[1] they are appearing more commonly since the introduction of protease inhibitor-based therapy.[2] Two phenomena appear to occur: increased fat accumulation in the breast, or true gynecomastia with increased glandular tissue.
Patients may be asymptomatic or may have discomfort or pain. Although this is most commonly associated with drug therapy, no specific drug or class of drugs has been definitively implicated, and the mechanism of breast enlargement is unclear. It is important to exclude other causes of gynecomastia (eg, renal disease, liver disease, tumors with increased estrogen, inhibitors of testosterone, and other drugs) before attributing the condition to antiretroviral therapy.
Management of this condition is not well defined. As with other metabolic manifestations of antiretroviral therapy, reversal does not always occur when specific drugs are discontinued or switched. Some patients have opted for surgery. An intriguing recent report studied the use of dihydrotestosterone gel applied to the skin.[3]

    • Large breasts: Gynecomastia in men (Estrogen, Drugs (eg Spironolactone, H antagonists, Neuroactive agents), Lung Cancer, Testicular Cancer). Small breasts : Underdevelopment.
    Asymmetry
    • Normal variation Accessory axillary breast tissue Mass lesions : Flattening of skin or mass brought out by raising hands over head.. Dimpling brought out by hands pressed against hips or pushing hands together overhead. Edema from SVC syndrome with prominent collateral veins Surgical excision (Scars, Surgical defects) Unilateral gynecomastia : Lung cancer, Drugs
    Contour Nipple
    • Inversion (Normal, Cancer) Retraction and flattening (Suspect Cancer) Deviation (Suspect Cancer) Nipple discharge
      • Bloody (cancer, Intraductal papilloma) Milk (Post pregnancy, Galactorrhea Endogenous or Exogenous hormone
      Non-Milk: Duct Ectasia Bloody: ( Intraductal Papilloma , CA) Eczema (especially if unilateral)
    Accessory Nipple Accessory nipple can be seen along the line shown in the diagram.

To the Editor: In his review of gynecomastia (Feb. 18 issue), Dr. Braunstein includes a long and detailed list of agents that have the potential to induce the condition. Human growth hormone was not listed. Although not widely known, it has been reported that growth hormone can induce gynecomastia . The underlying mechanisms are not known, but may be related to the lactogenic activity of growth hormone. Recombinant techniques have provided an unlimited supply of growth hormone, and many patients without growth hormone deficiency are being treated with the hormone.
