Gynecomastia
Dr. Michael Bermant is Board Certified by the American Board of Plastic Surgery Male breast growth can have a major emotional impact on a young man. There is just something about protruding breasts in a male that can result in teasing from peers. Typical behavior is to hide the chest when in public or shun others. Those bothered by the problem avoid swimming, taking their shirts off, and other activities where others can see their breasts. Many do not discuss their concerns with parents, peers, and do not understand the problem. Starting that dialog is important. Gynecomastia is a common condition affecting between 40 and 60% of all males. Breast development easily happens during adolescence and may have many possible causes. It can appear as a small lump just under the areola (colored skin around the nipple). During growth, this mass can be tender.

Gynecomastia
Gynecomastia is defined as the visible or palpable development of breast tissue in boys or men. It has been divided into four types: Type I: (pubertal or benign adolescent breast hypertrophy) refers to the common entity seen in pubertal males. Incidence may be as high as 60-70%. It is typically a firm, tender, subareolar mass anywhere from 1-5 cm in diameter. The pubertal adolescent frequently complains of pain in the breasts, particularly when wearing binding clothing. It usually spontaneously resolves within 2 years. Type II (physiological gynecomastia without evidence of underlying disease, or with evidence of organic disease including the effects of specific drugs) refers to a generalized, nonpainful breast enlargement. It is essential to differentiate between physiologic gynecomastia and breast enlargement due either to a pathologic process or to the use of a specific drug. Careful history taking regarding the time of onset, family history, duration of enlargement, history of systemic illness, weight change, and drug or medication use, is important. Physical examination should include height, weight, blood pressure, breast size, and Tanner staging of both breasts and genitals, in addition to a neurologic assessment. Type III gynecomastia is general obesity simulating gynecomastia, and Type IV is pectoral muscle hypertrophy.

A word about breast reduction in men... Gynecomastia is a medical term that comes from the Greek words for "women-like breasts." Though this oddly named condition is rarely talked about, it's actually quite common. Gynecomastia affects an estimated 40 to 60 percent of men. It may affect only one breast or both. Though certain drugs and medical problems have been linked with male breast overdevelopment, there is no known cause in the vast majority of cases. For men who feel self-conscious about their appearance, breast-reduction surgery can help. The procedure removes fat and or glandular tissue from the breasts, and in extreme cases removes excess skin, resulting in a chest that is flatter, firmer, and better contoured. Before surgery 16 months after surgery This is a 17 year old male with a history of gradual growth of his breasts over the past four years. He was bothered by the breasts to the point of not wearing T-shirts and keeping himself covered at the beach. He states he walked with his hands covering his chest so that they were not seen. Gym activities embarrassed him. He did not have a hormonal workup before coming to my office. His breast examination revealed mild to moderate breast enlargement with primarily glandular tissue. His testicles were small. His pediatrician assisted with a work up that revealed Klinefelter's syndrome. This is a common cause of gynecomastia and genetic problem seen in males (1:500). Hypogonadism is a problem with small testicles and low hormone output. Some patients do not develop gynecomastia until after puberty.

Considerations Return to top The most common cause of gynecomastia in the male is puberty. The condition may occur in one or both breasts and begins as a small lump beneath the nipple, which may be tender. The breasts often enlarge unevenly. Gynecomastia during puberty is not uncommon, is self-limiting, and usually goes away over a period of months.
In newborns, breast development may be associated with milk flow ( galactorrhea ). This condition usually lasts for a couple of weeks and in rare cases may persist until the child is two years old. It is caused by exposure to maternal hormones.
Other causes of gynecomastia include chronic liver disease , kidney failure, and exposure to estrogens, androgens (often taken secretly for body building), marijuana , and some medications. Rare causes include tumors, genetic defects, or an overactive thyroid.

What is gynecomastia?
Gynecomastia is a condition in which firm breast tissue forms in males. The breast tissue is usually less than 1-1/2 inches across and is located directly under the nipple. Gynecomastia may be present on one side or on both sides. This condition may make the breast tender. Some men and boys have fat on their chests that makes it look like they have breasts. This condition is called pseudogynecomastia (false gynecomastia). It is not the same thing as gynecomastia. Return to top
What causes gynecomastia?
Gynecomastia is usually caused by changes in hormones at puberty or as part of aging. Hormones are chemicals produced by the glands of the body, such as the thyroid gland, the testes and the ovaries. Gynecomastia may be caused by changes in the balance of 2 hormones, estrogen and testosterone. In rare cases, gynecomastia is caused by prescription drugs, over-the-counter medicines, illegal drugs, tumors or disease. Return to top

You are visitor number to this page. Gynecomastia While most gynecomastia is seen in adolescence and resolves without treatment some cases are the result of endocrine gland malfunction or tumors. Some types of liver disease and cancers of several types can produce gynecomastia. Some cases are due to congenital syndromes and others may be related to drug use such as steroids or marijuana. There is a second peak in incidence late in life and here the risk of ductal carcinoma of the breast must be a stronger consideration so that biopsy of the breast may be necessary. When gynecomastia is severe, when it persists, and in those cases where there is supicion of malignancy surgical treatment may be indicated. In most cases the incisions for this operation can be placed just inside the areolar border where they are less likely to be conspicuous. The glandular tissue must be removed. This firm fibrous tissue extends from the nipple areolar complex down to the surface of the pectoral muscle beneath. In some cases it is helpful to use liposuction to remove some of the fatty tissue which always surrounds this ductal tissue. Without this liposuction some patients would have a "donut" shaped defect after removal of the ductal tissue. Pictures above represent a mild and a moderate case of gynecomastia treated surgically.The preoperative views are on your left, the postoperative on your right.
