    Definition
    Gout is a form of acute arthritis that causes severe pain and swelling in the joints. It most commonly affects the big toe, but may also affect the heel, ankle, hand, wrist, or elbow. It affects the spine often enough to be a factor in back pain. Gout usually comes on suddenly, goes away after 5-10 days, and can keep recurring. Gout is different from other forms of arthritis because it occurs when there are high levels of uric acid circulating in the blood, which can cause urate crystals to settle in the tissues of the joints. Description Hyperuricemia doesn't always cause gout. Over the course of years, however, sharp urate crystals build up in the synovial fluid of the joints. Often, some precipitating event, such as an infection, surgery, the stress of hospitalization, a stubbed toe, or even a heavy drinking binge can cause inflammation. White blood cells, mistaking the urate crystals for a foreign invader, flood into the joint and surround the crystals, causing inflammation-in other words, the redness, swelling, and pain that are the hallmarks of a gout attack. Causes and symptoms Gout affects an estimated one million Americans; according to the National Institutes of Health, it accounts for about 5% of all cases of arthritis. It occurs more often in men than in women; the sex ratio is about 4:1. Uric-acid levels tend to increase in men at puberty, and, because it takes 20 years of hyperuricemia to cause gout symptoms, men commonly develop gout in their late 30s or early 40s. Women more typically develop gout later in life, starting in their 60s. According to some medical experts, estrogen protects against hyperuricemia, and when estrogen levels fall during menopause, urate crystals can begin to build up in the joints. Excess body weight, regular excessive alcohol intake, the use of blood pressure medications called diuretics, and high levels of certain fatty substances in the blood (serum triglycerides) associated with an increased risk of heart disease can all increase a person's risk of developing gout.

Simply remembering this diagnosis is enough to solve the puzzle, since the usual laboratory and pathological fingings are present and make confirming the diagnosis a simple matter.Gout is an inflammatory arthopathy that occurs as a result of hyperuricemia. It is the most common inflammatory arthritis in men over thirty years old. Hyperuricemia is generally secondary to under-excretion of uric acid. Only 10% of gout patients over produce uric acid and this may be a result of
    myeloproliferative, lymphoproliferative or hemolytic disease. Uric acid is filtered at the glomerulus and is subject to tubular reabsorption. Drugs that interfere with tubular handling such as thiazides, cyclosporine, and salicylates can cause a gout crisis. Hyperuricemia is also correlated with obesity, alcohol use, and high protein intake. Gout primarily affects adult men and the peak incidence occurs in the fifth decade. The diagnosis of gout is dependent on the presence of urate cystals in joint fluid and the development of tophi,
    not the presence of hyperuricemia.

      Causes
      Excessive uric acid (hyperuricemia) in the blood and deposits of urates of sodium in and around the joints. Several different metabolic abnormalities this condition. Approximately 90% of gout patients are male. The peak age for onset of symptoms in men is between 40 and 50; women rarely have gout before menopause. Gout is closely related to the diet, but may be brought on by stress. Obesity and an improper diet increase the tendency for gout. Also called the "disease of kings" and "rheumatism of the rich" because of the rich diets people consume.

      Symptoms
      Most cases are without symptoms. When an acute attack occurs it usually begins at night with moderate pain that increases in intensity to the point where no body position provides relief. The big toe is a frequent site. If you think you might have gout, have your doctor do a test to diagnose and treat the condition.

    To some, gout sounds like a disease out of a novel about the 16th century. That's probably because this joint disease famously afflicted Henry VIII, Leonardo da Vinci, Voltaire and other luminaries from the past. It's been theorized these historical figures had gout because they had the money to enjoy red meat and seafood, foods rich in a group of compounds called purines, which have been linked with an increased risk of gout. Purine leads to high levels of uric acid, which can build up in the joints and crystallize, causing attacks of gout. Today, more people of all income brackets have access to red meat and seafood and tend to eat fewer protective fruits, vegetables and low-fat diary products. As a result, gout is more of an equal-opportunity disease, affecting more than five million American adults. Some speculate that the followers of the popular high-protein, low-carb diets may be at particular risk. And a new study confirms what has long been suspected: that there is a link between red meat and seafood and increased gout risk. It also found that some foods protect against gout.

    Joint inflammation which is called arthritis and pain that is caused by crystal deposits in the joints is known as gout. This metabolic disorder will usually affect a single joint and in some ways is associated with kidney stones since about 20 percent of the people who have this disorder develop kidney stones. Gout, which is also known as acute gouty arthritis, occurs when deposits of monosodium urate crystals accumulate in the joints due to an abnormally high level of uric acid in the blood. In some cases the joint inflammation becomes chronic and even deforming with time. bodyOffer(17947) The normal human body will produce some uric acid that is a by product of the cells breaking down. Since the body is constantly breaking down cells and forming new ones there is always some uric acid in the blood. This, coupled with the fact that some foods contain precursors of uric acid creates a low level of uric acid that is eventually eliminated by the kidneys and will show up in most blood test. But, when the kidneys can not eliminate enough of this uric acid from the blood stream high levels cause monosidium urate crystals to accumulate in the joints causing gout. In some cases the body may produce large levels of uric acid when there is a hereditary enzyme abnormality or disease which causes the cells to multiply and be rapidly destroyed. There are also some types of kidney disease that make it functionally impossible for the kidneys to eliminate uric acid.

The uric acid crystallizes and settles in the joints in the body, most often in the big-toe joint. When it does so, it produces an excruciating pain.
