Plead with gout either produce too much uric acid or more commonly have a problem in removing it. There are a number of possible consequences of this build up of uric acid in the body, including acute and chronic gouty arthritis, kidney stones, and deposits of uric acid in subcutaneous and other tissues (tophi). Gout may occur alone (primary gout) or may be associated with certain medical conditions or medications (secondary gout).
Gouty arthritis should be considered in people who have a sudden onset of a hot, red, swollen joint. It is diagnosed by finding uric acid (monosodium urate) crystals in the joint fluid. These crystals can accumulate over the years in the joint and trigger repeated bouts of inflammation. If left untreated, the accumulated crystals can damage the joint and lead to chronic arthritis. Fortunately, there are effective medications to treat gout.

    What is gout?
    Gout is a disease that causes sudden, severe episodes of pain and tenderness, redness, warmth, and swelling (inflammation) in some joints. Gout is a dramatic example of a type of arthritis called "crystal arthritis," sometimes called "microcrystalline arthritis" because the crystals are very small. The crystals form in the joint space. As the body tries to remove the crystals, a painful inflammation occurs. It usually affects one joint at a timeoften the large joint of the big toe. It also can affect other joints, such as the knee, ankle, foot, hand, wrist, and elbow. In rare cases, it may later affect the shoulders, hips, or spine.
    Incidence and risk factors
    Gout affects more than one million Americans from all walks of life. It can occur at any age, but the first attack often affects men between the ages of 40 and 50. Gout affects women as well. In fact, the number of postmenopausal women who have gout is increasing. However, it sometimes is difficult to diagnose gout in these women because they also may have

The joint is unbearably painful. Even the weight of a sheet is too much. Middle-aged or elderly men are most often affected. What is gout?
Gout is pain and inflammation in a joint caused by accumulation of crystals. In patients with gout, a substance called uric acid builds up, because either the body produces too much or the kidneys do not eliminate it quickly enough. The crystals are made up of this substance. Will my doctor know I have gout just from my symptoms?
A high level of uric acid in the blood along with typical symptoms may provide the answer. To be certain that the problem is gout and not infection or another cause, your doctor may draw a little fluid from the joint and test it for uric acid crystals. What is the treatment for gout?
Everyone who has had an attack of gout agrees that stopping the pain is the first order of business. A nonsteroidal anti-inflammatory drug often helps. Colchicine, a drug especially for gout, usually gives relief, particularly if taken early in the attack, but it may have side effects. Patients who cannot take these drugs may be helped by an injection of corticosteroid into the joint. Treatment after the first attack varies. Some fortunate people never have another attack. In others, attacks recur and may involve additional joints. Sometimes, mild inflammation continues for months and can slowly damage joints without causing painful attacks. In some cases, over time, chalky deposits (called tophi) may cause nodules around the joints. These nodules may have to be treated with drugs or removed surgically.

9.10 Acute Gouty Arthritis
Presentation
Usually a male patient over 50 years old with an established diagnosis of gout or hyperuricemia rapidly develops an intensely painful monarticular arthritis, often in the middle of the night, but sometimes a few hours following a minor trauma. Any joint may be affected, but most common is the metatarsophalangeal joint of the great toe (podagra). The joint is red, hot, swollen, and intensely tender to touch or movement. There is usually no fever, rash, or other sign of systemic illness. The patient may have predisposing factors that increase his risk of developing gout, like obesity, moderate to heavy alcohol intake, high blood pressure, diabetes and abnormal kidney function, or he may be taking certan drugs including thiazide diuretics, low-dose aspirin, and tuberculosis medications (pyrazinamide and ethambutol).
What to do:
  • If the patient has not been previously diagnosed by arthocentesis that showed crystals, then tap the involved joint as described in acute monarticular arthritis . In addition to ruling out infection, look under the microscope for crystals in the joint fluid. Urate crystals look like needles, and may be in white cells. Polarizing filters above and below the sample can help disclose and differentiate crystals.

16 April 2004 - written by BUPA's Health Information Team Gout is a painful condition that affects the joints of men and women. The incidence of gout has been increasing in the developed world for the past 30 years. It has been thought for a long time that drinking alcohol increases your risk of developing gout, but the theory has never been proven. An american study, published in the Lancet, looked at 50,000 men and found that drinking alcohol is linked with an increased risk of developing gout. In addition, it appears that the type of drink also makes a difference: beer appears to be more "risky" than wine.
What did the study look at?
The study was published in the Lancet by researchers at the Massachuetts General Hospital. The 12-year study assessed the lifestyles of 47150 men who did not have gout and followed them to see if they developed gout. Over the course of the study 730 men developed gout. By looking at the amount of alcohol the men drank the study's authors produced valuable information about the association between alcohol and gout. Also, by looking at the types of drink the men drank, the study's researchers have drawn conclusions about the role different drinks play in raising the risk of developing gout.

What is gout?
Gout, otherwise known as podagra or uric acid arthropathy is a rheumatic complaint, that usually attacks a single joint at a time. The disease has a preference for the big toe of middle-aged men - it swells, turns red and becomes sore. The soreness is such that just walking through a room can cause severe pain. It is more common in men than women by a factor of 10 to 1.
What is the cause of gout?
The disease is caused by the deposition of sodium urate (uric acid) crystals in the joints. Uric acid is a by-product of the body's metabolism. Normally the uric acid is removed when urinating, but among patients with a predisposition for gout, the uric acid accumulates in the blood. Among some of these patients, the concentration in the blood is so high that the uric acid 'overflows' and settles in the joints and possibly in the skin.

It is also described as the king of diseases and disease of Kings. Gout is much more common in men then women who as Hippocrates also pointed out, are rarely affected before the menopause. It would seem that gout tends to affect those with a more affluent life style. It has also been said that sufferers from gout are often of more than average intelligence. Gout is caused by a build up of uric acid in the blood. Uric acid is a normal breakdown product of various chemical processes within the body. The build up of uric acid is caused by a defect in the way the body deals with uric acid, this defect is usually inherited. When uric acid builds up it sometimes forms crystals in the joints which then become hot red and painful. It is not true that eating and drinking causes gout however if you do over indulge it may make attacks of gout more likely.

Cayenne (capsicum) powder applied externally can relieve inflammation and pain. Mix with enough wintergreen oil to make a paste and apply to affected areas. DIETARY RECOMMENDATIONS
  • Try to eat 1/2 pound of fresh or canned Royal Ann or black Bing cherries/day. This has been shown to be very effective in lowering uric acid levels and preventing attacks of gout.
