It also explains how gout is diagnosed and treated. If you have further questions after reading this booklet, you may wish to discuss them with your doctor. What Is Gout? Gout is one of the most painful rheumatic diseases. It results from deposits of needle-like crystals of uric acid in connective tissue, in the joint space between two bones, or in both. These deposits lead to inflammatory arthritis, which causes swelling, redness, heat, pain, and stiffness in the joints. The term arthritis refers to more than 100 different rheumatic diseases that affect the joints, muscles, and bones, as well as other tissues and structures. Gout accounts for approximately 5 percent of all cases of arthritis.

Gout is a syndrome caused by an inflammatory response to the formation of monosodium urate monohydrate crystals which develop secondary to hyperuricemia. Acute and chronic forms are recognized. Hyperuricemia may be due to environmental and/or genetic factors.
Clinical features

  • Gout affects 2.1 million Americans. Men aged 40 to 50 are most commonly affected.
What is Gout? Known as "the disease of kings and the king of diseases," gout has been studied by physicians and has caused suffering in many people for centuries. Once horribly painful and debilitating, gout is now easily controlled with proper treatment. What causes it? Gout is one of the few types of arthritis where the cause is known. It results from deposits of needle-like crystals of uric acid in the connective tissue, joint spaces, or both. Uric acid is a byproduct of the breakdown of purines or waste products in the body. Normally uric acid breaks down in the blood and is eliminated in urine. When the body increases its production of uric acid or if the kidneys do not eliminate enough uric acid from the body, levels build up. This is called hyperuricemia. Hyperuricemia is not a disease and is not dangerous. However, if excess uric acid crystals form as a result of hyperuricemia, gout can develop.

Written by: Gloria Tsang, RD High levels of uric acid can cause gout. High purine foods in a diet can increase uric acid levels in the body. Therefore, the gout diet usually limits food with high purine. Gout Diet / Low Purine Diet
  • Alcohol, especially beer, should be avoided. Limit alcohol consumption to 1 drink 3 times a week.
  • Drink 2 to 3 L of fluid daily. Adequate fluid intake helps dilute urinary uric acid.
  • Avoid High-Purine foods (see below)
  • Consume a moderate amount of protein. Limit meat, fish and poultry to 4 - 6 oz per day. Try other good protein food such as low fat dairy products, tofu and eggs
  • Limit fat intake by choosing leaner meats, foods prepared with less oils and lower fat dairy products.

Anti-inflammatory drugs are often used There is little scientific evidence to support the use of some current treatments for gout, researchers say.
The condition is usually treated with an anti-inflammatory drug, or with alternatives such as steroids. But the journal Drug and Therapeutics Bulletin says much of the evidence for their effectiveness is anecdotal. Other experts have disputed that claim. Gout affects around 600,000 people in the UK and is becoming increasingly common in men. Gout generally involves very severe attacks of joint pain followed by long periods of remission. The condition is caused by the formation of urate crystals within joints and other tissues. Acute attacks are usually treated with an anti-inflammatory drug - in many cases indometacin. However, the DTB says there are no published studies showing whether this is the most effective treatment. In fact, it says there is evidence to suggest that indometacin is no better, or more easily tolerated than other anti-inflammatory drugs.

Beer is the biggest risk Scientists have at last proved that drinking increases the odds of getting gout - but the level of risk depends on your tipple of choice.
Although it has long been suspected that gout was linked to alcohol, the theory had never been proved. Now a study of nearly 50,000 men has found those who over-indulge in beer, in particular, are at heightened risk. The research, published in The Lancet, was carried out by a team at Massachusetts General Hospital. They found that the condition was less likely to be linked to drinking spirits. People who drank a moderate amount of wine had no increased risk at all. These findings suggest that certain non-alcoholic components that vary across these alcoholic beverages play an important role

Mike Snaith examines one of the more easily treatable rheumatic diseases. Gout is a 20th century success story. Attacks can be treated and prevented and damage to joints and kidneys can be prevented. However, we can never be complacent, as the following case histories illustrate. Case No 1. John McG. was a heavily built Scotsman, with gout for 15 years. Beer with whisky chasers was his tipple. I told him about his weight, his gout and the poor state of his liver and he promised to stop drinking. We started him on allopurinol (Xyloric) with the usual precautions. Over the next few months his attacks diminished, but not much; his uric acid reduced only a little and his weight remained unchanged. When I asked him about his alcohol, he said: "Of course doc, I've done exactly what you said, I've given up the drink". Me: "Your gout should have improved more than this. So you've given up all your drinking, your beer and your whisky?" To which he replied "Oh! doctor, I didn't think you meant the beer as well, I don't really count that as drinking!" Case No 2. David R. was a bachelor, who limped into the consulting room on a stick in slippers. He could not wear his normal size 91/2 shoes because of the painful swelling due to his gout. He was taking 900mg of allopurinol per day. The usual dose is 300mg per day, rarely 600! And his gout was still not controlled. Like Mr McG, he drank too much, but he also ate liver, bacon, sausage or kidneys at least four days a week, or steak when out in a restaurant. Similarly on his trips abroad his diet would have made a vegetarian weep.
