What is gonorrhea? Gonorrhea is a disease caused by bacteria called Neiserria gonorrhoeae , the gonococcus. The bacteria that cause gonorrhea are found in the mucous areas of the body (the vagina, penis, throat and rectum) and in semen or vaginal fluids. It is one of the most commonly reported sexually transmitted diseases (STD) in the United States.
Who gets gonorrhea? Any person who has sex can be infected with gonorrhea. Most often, gonorrhea is found in younger people (ages 15-30) who have multiple sex partners. Gonorrhea is reported more frequently from urban areas than from rural areas.
What are the symptoms of gonorrhea? Most men infected with gonorrhea will have burning while urinating and a yellowish white discharge from the penis. Most women do not have symptoms. Those few women with symptoms will have a discharge from the vagina and possibly some burning while urinating. Women may also have abdominal pain or abnormal bleeding. Commercial Printing CDC to announce new gonorrhea guidelines Published Wednesday, April 28, 2004 ATLANTA (AP) - Because of growing cases of drug-resistant strains of gonorrhea, the government is expected to announce this week that common treatments against the sexually transmitted disease no longer work and that other drugs should be used, top experts said Monday. The Centers for Disease Control and Prevention plans to announce tomorrow new recommendations for treating gonorrhea, spokeswoman Jessica Frickey said, but she declined to give details. Two health officials who worked with the CDC on its new gonorrhea guidelines said a different antibiotic will be recommended.

Gonorrhea - Neisseria gonorrhoeae
Epidemiology: Gonorrhea is the leading reportable STD in adults in the US. In 1994, 407,600 cases were reported, more from urban than rural areas.
Incubation period: 1-14 days, the average 2-5 days.
Symptoms: Infections affect primarily the skin and mucous membranes - proctitis, pharyngitis, cervicitis, urethritis (anterior urethritis with purulent discharge in males).
Clinical manifestations: Local complications include epididymitis (males) and pelvic inflammatory disease (females). Gonococcal conjunctivitis may occur in adults via direct inoculation by contaminated hands (contact wearers must take precautions), while newborns may develop ophthalmia neonatorum after inoculation in the birth canal of their infected mothers. Prophylactic therapy consists of erythromycin or silver nitrate given shortly after birth. Blood borne infection (DGI) is another complication which is characteristically manifested in the skin as dirty gray pustules with an erythematous base and purulent center. These lesions are accompanied by fever, tenosynovitis and migratory arthralgias or arthritis of small joints. If undiagnosed, the skin lesions will eventually clear, and the disease will localize to a large joint. Perihepatitis (Fitz-Hugh Curtis syndrome), endocarditis, or meningitis are rare complications.
Treatment: Usually ceftriaxone, to which most organisms are sensitive. The patient may be started on treatment before diagnosis is certain and before results of antibiotic sensitivities are known. Treatment is necessary because failure of only 1% poses substantial selective pressure to select resistant strains. Treatment should include a seven day course of tetracycline because there is a risk of simultaneous infection with Chlamydia.
