GRACE'S DAUGHTER: Goiter My symptoms began gradually enough not to be noticed. Perhaps this is what growing older feels like? I found myself napping more, going to bed early and feeling less energetic. Climbing the stairs left me breathless. My periods were intense. And there was a gradual but infuriating weight gain, despite all efforts to arrest it.
My legs, then my hands felt bloated. When the bloating reached my face I knew there was something really wrong. In May 1993, a blood test revealed that I had a goiter, a condition which had caused me to develop hypothyroid condition. This could be treated with a daily pill. It took the summer months for the condition to reverse itself. By September I finally felt my strength return.
If this was how Rozetta died, it must have been slow, agonizing, and tragic. Recently I learned that my grandmother Beatrice had a thyroid condition too, but hers was treated and she lived a long life. The discovery of this hereditary inclination has given extra meaning to the passage of the ruby ring.

Goiter
Goiter refers to an enlarged thyroid gland. It can occur in both hypothyroidism, as thyroid cells enlarge in their efforts to trap more iodine, and in hyperthyroid as thyroid cells elongate and swell due to to their vigourous assembly line production of thyroid hormone.
There are approximately 25 different types of goiter, depending on the type of pattern created when cells take up iodine and by characteristic changes in thyroid tissue. Goiters may also be nodular, having hot or cold nodules, or they may be vascular, with changes present in the veins and arteries that supply the thyroid gland. In Graves' disease, iodine distribution is even although the uptake of iodine is increased. For these reasons, goiter in Graves' disease is called "diffuse toxic goiter."
Why do some goiters continue to grow? In hypothyroidism due to iodine deficiency, goiters continue to enlarge as the cells try to pull in more iodine. In autoimmune thyroid disorders, including Graves' disease and Hashimoto's thyroiditis, thyroid growth immunoglobulins (TGI) are the primary cause of goiter enlargement. TGI stimulate thyroid cell growth, whereas thyroid stimulating immunoglobulins (TSI) cause excess thyroid hormone production.

It can range in size from a small lump to a large swelling that can displace the airway and esophagus. Goiters are more likely to develop in women over age 40 and people with a family history of goiters. A variety of conditions can cause a goiter. The most common cause of a goiter used to be a lack of iodine in the diet. It occurred in areas where the soil and water lacked iodine. Goiter became less common after salt containing iodine became widely available. Most simple goiters result from an underactive thyroid gland. Having an underactive thyroid that produces too little thyroid hormone is called hypothyroidism. When the thyroid gland is unable to produce enough thyroid hormone to meet the body's needs, it gets larger in an attempt to increase hormone production. The most common form of hypothyroidism is Hashimoto’s thyroiditis, an autoimmune disorder. Treating an overactive thyroid often results in hypothyroidism, which is treated by replacing the thyroid hormone that is lacking. Consuming large amounts of some foods and drugs interfere with the production of thyroid hormone and may cause goiters to develop. These foods include brussels sprouts, cabbage, cauliflower, turnips and soybeans. Drugs include lithium, cobalt and iodides. In rare cases, thyroid cancer also causes goiters.
