Introduction
Goiter, disease of the thyroid gland, characterized by an enlargement of the gland, visible externally as a swelling on the front of the neck. In simple goiter the basal metabolic rate is somewhat lowered, and in toxic goiter it is elevated.
Simple Goiter Toxic Goiter
This disease, also called exophthalmic goiter, hyperthyroidism, thyrotoxicosis, or Graves' disease, for the Irish physician Robert James Graves, is caused by an excess of thyroxine secretion. The cause of the excessive secretion is obscure. In some cases it may result from excessive stimulation by the pituitary gland. The symptoms of toxic goiter may include a rapid heartbeat, tremor, increased sweating, increased appetite, weight loss, weakness, and fatigue. Some patients have eye problems, such as staring or protrusion. Thiouracil and iodine are sometimes used in the treatment of toxic goiter, as is irradiation of the gland by radioactive iodine.

It makes thyroxin, a hormone that regulates the metabolic activity of the body, rather like the gas pedal on a car. Too much thyroxin increases the metabolism, causing weight loss, temperature elevation, nervousness, and irritability. Too little thyroxin slows the metabolism down, deepens the voice, causes weight gain and water retention, and retards growth and mental development in children. Both conditions also alter hair and skin growth, bowel function, and menstrual flow. Curiously, the thyroid gland is often enlarged whether it is making too much hormone, too little, or sometimes even when it is functioning normally. The thyroid is controlled by the pituitary gland, which secretes thyroid stimulating hormone (TSH) in response to the amount of thyroxin it finds in the blood. Sometimes blood tests, a radioiodine scan, or ultrasound image may be helpful, too. Treatment depends on what's causing the problems.

Endemic goiter Epidemiology The term endemic goiter is a descriptive diagnosis and reserved for a disorder characterised by enlargement of the thyroid gland in a significantly large fraction of a population group, and is generally considered to be due to insufficient iodine in the daily diet. Since nontoxic goiter also exists when there is abundant iodine in the diet, the distinction between endemic and nonendemic goiter is necessarily arbitrary. Endemic goiter may be said to exist in a population when more than 5% of the preadolescent (at 6-12) school?age children have enlarged thyroid glands (2), as assessed by the clinical criterion of the thyroid lobes being each larger than the distal phalanx of the subject's thumb. Detailed criteria are discussed further below. Goiter maps of various countries have been repeatedly drawn, requiring modification as successful prophylactic measures have been introduced. Although goiter was an important problem in many regions of the United States in the past (55), more recent US surveys have shown it in no more than 4?11% of schoolchildren, and with almost no evidence of iodine deficiency (56). This finding is a testimony to the effectiveness of iodine prophylaxis in preventing endemic goiter. The great arc of the Himalayas from West Pakistan across India and Nepal, into Northern Thailand and Vietnam and into Indonesia, is one of the most highly endemic regions of the world. The disease used to be a major problem throughout the Andes. It has been reported from New Zealand and from different places in Europe. The world or regional distribution of goiter was exhaustively reviewed by Kelly and Snedden in 1960 (57) and subsequently by others in 1980 (5), 1993 (58), 1996 (59), 1998 (60) and 1999 (61).

The swollen thyroid shows up conspicuously in front of the throat. Although most goitrous glands are usually confined to the neck, occasionally they may extend into the upper thorax, or more rarely may be intrathoracic. There are several types of goiters: (i) benign, (ii) nodular, or (iii) adenomatous growth, or (iv) diffusely hyperplastic enlargement. In acute cases, the goiter grows rapidly, while in the adenomatous type, a goiter is caused by the growth of an encapsulated adenoma. The nodular form contains nodules, while in a diffuse goiter the thyroid tissue is diffuse, in contrast to the nodular form. A goiter may be cystic when a cyst or cysts are formed, possibly from the degenerated tissues. A normal human thyriod gland weighs only 20 to 30 g, but a large-sized goiter may weigh as much as 1 kg. Endemic goiters occur in a relatively large proportion of population in certain geographic localities of the world, where iodine content in food and water is deficient or exceedingly low. Nodular goiters also occur sporadically in areas where goiters are nonendemic. Besides iodine deficiency in food, a number of different factors may collectively initiate abnormal growth of the thyroid gland. In some individuals thyroid enlargement is the result of a genetically determined defect in the biosynthesis of thyroid hormone.
