SUMMARY OF EVIDENCE
Note: Separate PDQ summaries on Prevention of Gastric Cancer and Gastric Cancer Treatment are also available. There is insufficient evidence to establish that screening would result in a decrease in mortality from gastric cancer in the U.S. population. (Level of evidence: 5) Endoscopy is sometimes used to screen for gastric cancer. There are serious but uncommon side effects associated with endoscopy, which may include the following: perforation, cardiopulmonary events, aspiration pneumonia, and bleeding requiring hospitalization. Level of Evidence: 5: Opinions of respected authorities based on clinical experience, descriptive studies, or reports of expert committees

In the UK, stomach - or gastric - cancer is the sixth most common cancer, with about 10,000 people diagnosed each year. Around 6500 of these are men. More than 8 out of 10 cases are diagnosed in people over the age of 60. The success of treatment depends on how early the diagnosis is made. ... The building blocks of the body are tiny cells, which normally repair and reproduce in a controlled process. With cancer, this process goes wrong and cells can divide and grow in an uncontrolled way, causing a lump - a tumour - on or within the part of the body affected. If this tumour contains cells that are able to invade neighbouring tissues or organs, it is known as malignant. If the tumour does not do this it is termed benign, and is not cancer.
The upper digestive tract
The stomach
The lining of the stomach produces a B acid to help digest food and also protect from harmful bacteria. The food is also broken down mechanically by the churning action of the stomach.

T he overall incidence of gastric cancer in the United States has rapidly declined over the past 50 years. Gastric cancer is now the 13th most common cause of cancer mortality in the United States, with an estimated 12,100 deaths in 2003. However, in developing countries, the incidence of gastric cancer is much higher and is second only to lung cancer in rates of mortality. The typical patient with gastric cancer is male (male-to-female ratio, 1.7:1) and between 40 and 70 years of age (mean age, 65 years)

Gastric cancer
Definition: Gastric cancers are malignancies of the stomach.
Alternative Names: Cancer - stomach; Stomach cancer; Gastric carcinoma
Causes, incidence, and risk factors: Several different types of cancer can occur in the stomach. The most common type is called adenocarcinoma, which refers to the way the cancer looks under the microscope. There are several types of adenocarcinoma. Other types of gastric cancer occur much less frequently so this article is focused on adenocarcinoma of the stomach. Adenocarcinoma of the stomach is a common cancer of the digestive tract worldwide, although it is relatively uncommon in the US. It occurs most frequently in men over 40 years old. The incidence of this form of gastric cancer is extremely high in Japan, Chile, and Iceland. The incidence of most types of gastric adenocarcinoma in the US has declined over the years. Experts propose that the decrease may be related to reduced intake of salted, cured, and smoked foods and increased vitamin C consumption.

Green tea and risk of gastric cancer
Some, but not all studies have found a reduced risk of gastric cancer in association with green tea consumption. This study examines whether green tea protects against gastric cancer among a large number of the Japanese population.
Message
Green tea consumption is not associated with the risk of gastric cancer in Japanese men and women.

Y Tsubono et al. Green tea and the risk of gastric cancer in Japan. The New England Journal of Medicine 2001 344: 632-636.
Study
Participants were 26,311 Japanese (11,902 men and 14,409 women), aged at least 40 years, residents of three municipalities from a region in northern Japan (chosen for its high incidence of gastric cancer). In 1984 participants completed questionnaires, which were delivered to and collected from their homes, on their health habits (e.g. diet, alcohol consumption, smoking), frequency of green tea consumption and family history of disease. They were followed for nine years. Green tea consumption was divided into four categories: less than one cup a day; one or two cups a day; three or four cups a day; and five or more cups a day. In the region studied, a typical cup contains 100 ml of tea.

Early symptoms can include indigestion, feeling bloated after eating, mild nausea, loss of appetite, or heartburn. In more advanced stages symptoms may include blood in the stool, vomiting, weight loss, or pain in the stomach. Known risk factors include prior stomach infection by Helicobacter pylori, smoking, frequent diet of dry salted foods, Menetrier's disease, and familial polyposis. Most cancers of the stomach are adenocarcinomas of which there are many sub-types.

Gastric Cancer - Linitis Plastica
A gastric carcinoma most often appears as a mass in the gastric mucosa. However, sometimes it only infliltrate the lumen of the stomach, and the only endoscopic finding is nondistensible stomach with a narrowed lumen. This type of gastric malignancy is called "linitis plastica". These pictures are from a patient with linitis plastica in the corpus region of the stomach, here a normal antrum is seen.
The corpus region is not distensible by air inflution, however, the mucosa looks quite normal. This finding - a nondistensible part of the stomach - is a serious finding, indicating a diffuse type of gastric cancer.
A cancer of the "linitis plastica"-type in the gastric body (corpus) The biopsies are superficial, and the cancer is spreading deep under the mucosa, so benign histology from the biopsies do not exclude a malignant disease.
This is an adenocarcinoma of the diffuse type causing a linitis plastica in the corpus. The affected part is seen to the right in these pictures, where the usual mucosal folds have dissappeared and the stomach wall is stiff and easily bleeding. To the left, some normal mucosal folds are seen.
