What is fifth disease?
Fifth disease is a viral infection which often affects red blood cells. It is caused by a human parvovirus (B19). For many years, fifth disease was viewed as an unimportant rash illness of children. Recently, studies have shown that the virus may be responsible for serious complications in certain individuals. Who gets fifth disease? Anyone can be infected, but the disease seems to occur more often in elementary school-age children. How is the virus spread? The virus is spread by exposure to airborne droplets from the nose and throat of infected people.

    Fifth disease, or erythema infectiosum, is a reasonably harmless red rash of childhood - the fifth rash on the old list of common red rashes. Hence the name, which is certainly easier to say than "erythema infectiosum." It is caused by human parvovirus B19 (no relation to dog parvovirus... parvo just means "small"). It is also known as "slapped cheek disease," because for the first day or two of the illness the cheeks become bright red as if slapped. Then a characteristic lacy rash appears on the extensor surfaces (those opposite the elbow and knee creases) of the arms and legs as well as the trunk. Heat - such as a warm bath - will make the rash appear worse and may cause mild itching. It occurs often in epidemics; stopping them is made more difficult by the fact that the disease is not transmitted once the rash appears . So your child can go to school with this rash. It chiefly affects kids ages 5 through 15; older patients with the disease may (as often the case with childhood infections) have more symptoms, especially joint aches. Most teachers test immune to the virus by the time they have been teaching a few years.

    What is Fifth disease?
    Fifth disease was so named because it was the fifth pink-red infectious rash to be described by doctors. With Fifth disease:
    • Your child has a bright red or rosy rash on both cheeks for 1 to 3 days ("slapped cheek" appearance). The rash on the cheeks is followed by a pink lacelike or netlike rash on the arms and legs (mainly on the thighs and upper arms). The "lacy" rash comes and goes several times for 1 to 3 weeks. Your child has a low-grade fever (less than 101°F, or 38.4°C) or no fever at all.

    Last Revision Date: Unknown Fifth Disease, Slapped Cheek Disease, Academy Rash General information Erythema infectiosum is a common disease of school children characterized by a red rash on the cheeks followed by a lacy rash on the arms and legs. It usually causes very little illness although adolescents sometimes have mild joint pains or swelling. Erythema infectiosum was given the name "fifth disease" because it was the fifth of the childhood rashes (measles, German measles, etc.) to be described. The disease is caused by parvovirus B19. It is spread by direct contact with infected mucus from the nose or mouth or by inhaling droplets coughed or sneezed into the air. Illness starts from 4 to 18 days after exposure. Because of the close and prolonged contact between students, epidemics of fifth disease are common in primary schools and academies. The illness Fifth disease rash sometimes begins with no illness at all but usually starts with 3 or 4 days of low grade fever and symptoms of a mild cold. This is followed in several days by the appearance of a bright red, raised rash which covers the cheeks, making them look as if they had been slapped. The area around the mouth is usually not involved. After another 1 to 3 days a pink, blotchy and often itchy rash appears on the upper part of the arms and legs, spreading to the trunk and buttocks. As it fades, the rash develops a lace-like or "net-like" appearance which can last for 5 to 10 days. Once cleared, it may reappear for several weeks or months if the skin becomes flushed from exercise, sunlight or a warm bath.

    Fifth Disease is an old name for a common childhood illness. It is more technically called Erythema Infectiosum. In the old days, different types of rashes signified different illnesses and each was assigned a number. It was difficult to distinguish or test for viruses and bacteria, and many people did not even believe that they existed. Measles was called first disease. Second disease was that dreaded Scarlet Fever that the child in my boyhood western movies always succumbs to. It has been easily treatable for years. Third disease was Rubella. The fourth was Duke's Disease which might have been a staphylococcal or an enteroviral infection. But Fifth Disease seems to have stuck with us. Fifth Disease can be thought of as a bad cold with a red rash on the cheeks. It is caused by parvovirus B19. First symptoms are: runny nose, malaise, and fever. This may be followed by a mild rash most intense on the face. This illness can be completely asymptomatic as well. The virus is spread, just like other cold viruses, through airborne droplets and body fluids. Symptoms begin between 5 and 15 days after exposure. It is a very common entity which you get only once, and tend to carry life-long immunity afterwards. It is no more severe or deadly than the common cold, unless your health is compromised to begin with. Panic seems to arise when a “visible illness” is around. I think it generally alarms people. Personally, I would be happy roaming among patients with hordes of Chicken Pox, Measles and Fifth Disease than I would among persons afflicted with resistant Tuberculosis or staphylococcal infections. In reality, once the rash appears, it is really a signal that your immune system is well on the way to complete destruction of this invader.

    Fifth Disease is typically a mild illness that is caused by a virus, parvovirus. It causes a blotchy red rash that begins on the cheeks (i.e., a slapped cheek appearance) and spreads to the trunk and limbs. The rash usually fades within a week, but it can come and go for several weeks, often appearing after exercise or sun exposure. The rash can be itchy. The rash may be preceded or accompanied by a mild fever and "cold" symptoms. Rarely, Fifth Disease can cause more severe illness in people with sickle cell anemia and immune problems (e.g., HIV/AIDS, cancer chemotherapy, organ transplant, taking steroid medication). If a pregnant woman catches Fifth Disease for the first time, there is a small risk (less than 10%) that it can infect the fetus and cause miscarriage or stillbirth. How is it Spread?
    Fifth Disease is spread by contact with nasal mucus and saliva. It can be spread by coughing and sneezing, kissing on the lips and sharing food, eating utensils and mouthed toys. It is also spread by touching your hands to your nose and mouth, reusing tissues, and forgetting to wash your hands after blowing noses. It spreads most easily in crowded and poorly ventilated rooms.     Staff Writers COSHOCTON A mild illness accompanied by a rash called fifth disease has cropped up in Muskingum County, affecting several children and at least one adult. However, health department authorities in Coshocton County and the city said they have not received any reports of the disease. Fifth disease, most commonly found in children, is caused by the parvovirus B19 and is characterized by a rash appearing on the face and limbs. The tell-tale rash, which appears to be a lacy or marble-like rash, often is called a "slapped cheek" rash because of the way it appears on the face. The disease is transmitted by direct or indirect contact with "respiratory secretions," according to Becky Beiter, director of nursing with the Coshocton County Health Department. Children can be infectious for five days before the rash appears through two days after.

    Chickenpox and fifth disease are childhood illnesses that can pose risks to the fetus if a mother contracts them during pregnancy. Fortunately, most fetuses are not harmed by these illnesses. However, a pregnant woman should consult her health care provider if exposed to either of these infections, so that steps to help prevent complications for mother and baby can be taken when necessary.
    Between 85 and 95 percent of pregnant women are immune to chickenpox, and about 60 percent are immune to fifth disease. If a woman is certain she already has had these illnesses, there is no need to be concerned about them during pregnancy, even if exposed to someone who has one of them. Only women who have never had these infections before are at risk of contracting them.
