VOL 114 / NO 4 / OCTOBER 2003 / POSTGRADUATE MEDICINE Download this Patient Note in PDF format (66K file) Your breasts feel thick, lumpy, and tender, particularly right before and during your menstrual period. You wonder if these symptoms are something to worry about. Your doctor tells you that you have fibrocystic breast disease, a scary name for a common, noncancerous condition that is believed to affect more than 60% of women. What is fibrocystic breast disease?
This condition, which is also called mammary dysplasia, benign breast disease, and diffuse cystic mastopathy, is really not a disease at all. Rather, these terms refer to changes in a woman's breasts that become more noticeable just before and during her menstrual period. These changes, which may include lumps, tenderness, and heaviness, often occur even in women who do not have fibrocystic breast disease. It appears that most women with this condition are not at increased risk for breast cancer. However, only a doctor can tell whether the changes are due to fibrocystic breast disease or another problem. This may be determined with just a manual examination, but sometimes an ultrasound, mammogram, or biopsy is needed. What causes fibrocystic breast disease?

Age-related changes resembling fibrocystic disease in iodine-blocked rat breasts. Krouse TB, Eskin BA, Mobini J It has been reported that dietary restriction and chemical blockade of iodine causes histopathologic changes in peripubertal female rat breasts. This study extended the age range to include midreproductive life and perimenopausal rats; there is a wider spectrum of structural alterations that are associated with the older breast, with sodium perchlorate as the blocking agent. In 16-week-old rats, breasts showed general increased parenchymal activity and growth, regressing after removal of the block. In 42-week-old rats, breasts showed noticeable calcospherite deposition, intralobular fibrosis, and cystic changes resembling human fibrocystic disease. In 52-week-old rats, breasts exhibited atypical lobules cytologically, papillomatosis, sclerosing adenosis, calcifications, and a lobular transformation of a histologically dysplastic type. It is the older rat that experiments will more closely parallel the human condition.

Fibrocystic Breast Disease Fibrocystic disease is a diagnosis based on finding breast pain, cysts, and noncancerous lumpy areas of the breast. But a study has discovered that about 80% of these cases are normal changes in breast anatomy and NOT associated with breast cancer. Women's Health Services Breast Self-Exam Almost three-quarters of women have been told by a healthcare provider that they have "fibrocystic disease," a diagnosis based on finding breast pain, cysts, and noncancerous lumpy areas in their breasts. This diagnosis seemed to be rampant about two decades ago. But a study by the National Cancer Association Consensus Committee discovered that about 80% of what is called "fibrocystic disease" is actually normal changes in breast anatomy and is NOT associated with breast cancer, says Christian Northrup, MD, author of "Women’s Bodies, Women’s Wisdom." The term has been used to describe just about any kind of breast thickening, tenderness, lumpiness or other symptoms. But these are variations of normal breasts, in fact, fibrocystic "disease" is clearly not a disease, says Northrup. True "fibrocystic changes" are actually limited to a very small number of these conditions and can only be identified by a pathologist examining breast tissue. Most women with breast cysts do not have an increased risk of developing breast cancer and need only to have the cysts treated.

Fibrocystic Breast Disease
If you have ever discovered one or more lumps (a change in the density of an area of the breast), you were probably concerned. Such lumps often are caused by changes that can occur with the natural menstrual cycle of a woman's body. Generalized breast lumpiness is one common type of benign (not cancerous) breast change. Benign breast conditions also include several types of distinct, solitary (single) lumps. Such lumps, which can appear at any time, may be large or small, soft or rubbery, fluid-filled or solid. Generalized breast lumpiness is known by several names, including fibrocystic changes, fibrocystic disease, and benign breast disease. Such lumpiness, which is sometimes described as "ropy" or "granular," can often be felt in the area around the nipple and areola and in the upper outer part of the breast. During pregnancy, the milk-producing glands become swollen and the breasts may feel lumpier than usual. It can be difficult for a woman to examine her breasts when she is pregnant, but she should continue to do so; although very uncommon, breast cancer has been diagnosed during pregnancy. Lumpiness may become more obvious as a woman approaches middle age and the milk-producing glandular tissue of her breasts increasingly gives way to soft, fatty tissue. Most often, women who find lumps in their breasts are between the ages of 30 and 50. The same hormones that control the menstrual cycle - estrogen and progesterone-also can cause the breasts to become lumpy or "fibrocystic." The incidence tends to decrease with menopause.

  • A common condition of the female breast is characterized usually by nonmalignant lumps and pain. It is estimated that more than 50 percent of adult females have fibrocystic disease of the breast. It can affect females from puberty to around age 50 (about 20% of premenopausal women have the disorder). It is most common in women of childbearing age. It often disappears after menopause (unless estrogen-replacement therapy [ERT] is used). Since it is so common, it is no longer considered a disease, and is usually not a cause for concern.
  • Understanding Fibrocystic Breast Disease FREQUENT SIGNS AND SYMPTOMS It is characterized by the presence of round lumps in the breast that move freely and are either firm or soft.
  • Lumps are usually in both breasts. Solitary lumps may occur, but multiple lumps are common.

... Fibrosystic Breast Condition Fibrocystic breast condition is a term used to describe a group of benign breast findings which affect one-third of women between the ages of 30 and 50. Your physician may tell you your breasts are fibrous or cystic feeling. Your physician may also say you have chronic mastitis (inflammation) or mammary dysplasia.
Fibrocystic condition may affect one or both breasts. Not all women with fibrocystic condition will exhibit symptoms. For some, the first symptom will be finding one or more cysts in their breasts.
Years ago, it was reported women with "fibrocystic disease" carried a higher risk for developing breast cancer. Present evidence shows women with fibrocystic disease and other chronic benign breast conditions are more likely to develop breast cancer later in life only if a breast biopsy shows "atypia," or abnormal breast cells. Only a very small percentage of fibrocystic patients will show atypia. Most women with chronic benign breast disease and fibrocystic breast changes are not at higher risk for developing breast cancer.

FIBROCYSTIC BREAST DISEASE Common Name: Benign breast disease Description: Fibrocystic breast disease is a generalized term for non- malignant lumps in breasts, causing pain. Although it is called a disease, it is not really a disease. Therefore the term benign breast disease or fibrocystic condition would be more appropriate. The benign lumps are usually smooth, regular and mobile. At least 50% of women have benign breast disease during their lifetime. Affects women in childbearing years, more common age 30-50. Rare in menopausal women not receiving hormone replacement therapy. Fibrocystic breast disease is the most frequent cause of lumps in the breast. Causes: Causes of benign breast disease are unknown, but lumps are probably sustained by estrogen and possibly also related to dietary fat and caffeine intake. In some cases may be associated with increased levels of prolactin ( a hormone secreted by pituitary gland). Very little is known about genetic aspects of fibrocystic breast disease, but family history of cysts is common.

Fibrocystic Breast Syndrome Although October is Breast Cancer Awareness Month, women need to be aware of their breast self-exams every month. No one, not even your doctor, knows your body like you do. Many women, as many as fifty percent, suffer from fibrocystic breast syndrome . These are benign (non-cancerous) lumps filled with fluid that change during your menstrual cycle. They can make your breasts swell and become tender, sometimes painful, just before and during your period. They are hormonally related. For this reason, the best time to examine your breasts is one week after your period. Everyone should do a breast self-examination once a month. Pamphlets and instruction is available in your doctor's office or by request from the American Cancer Society, Avon Products, or pharmaceutical companies, such as Wyeth-Ayerst. This condition DOES NOT MAKE YOU MORE SUSCEPTIBLE TO BREAST CANCER.

Fibrocystic breast disease provoked by Xenoestrogens are found in everyday synthetic materials. These materials were previously thought to be inert.
    by Elizabeth Smith, M.D.
Xeno literally means foreign. So xenoestrogens means foreign estrogens. Some of the 100,000 registered chemicals for use in the world have hormonal effects in addition to toxic and carcinogenic effects. Also the synergistic effects are known to occur but also are largely exactly unknown. The field of xenohormones is new, and only been in existence since about 1991. Critics have proclaimed that these chemicals are for the most part "weak". The body's hormones are at levels of parts per trillion. However, many of the chemicals that affect the hormone systems are routinely found in the serum after sauna at parts per billion. In other words, these chemicals that affect the hormone systems of the human body occur at 100 to 1000 times greater concentration than that of the normal human hormones. One researcher demonstrated that two "weak" estrogens may act synergistically to give a strong estrogen response. Some of these Xenoestrogens like DDE (a metabolite of DDT) may persist in the body fat for decades. Many of these mimicking hormones were previously thought to occur in pesticides. However, many of the newly discovered xenoestrogens are found in every day previously thought to be inert materials.

Cysts are fluid-filled sacs that often enlarge and become tender and painful just before the menstrual period.
Fibroadenomas are solid, round, rubbery, and freely movable breast lumps. Usually they are painless.
Lipomas are single, painless lumps that are sometimes found in older women. They are made up of fatty tissue and are slow-growing, soft, and movable.
Intraductal Papillomas are small wartlike growths in the lining of a duct near the nipple. They can produce bleeding from the nipple.
Mammary Duct Ectasia is an inflammation of the ducts that causes a thick, sticky, gray-to-green discharge from the nipple. Without treatment, the condition can become painful.

Only a physician can tell if this is due to fibrocystic tendencies or a specific problem. Most women with fibrocystic tendencies do not appear to be at increased risk for breast cancer. How should a woman with this condition do a search for breast disease? Is it impossible? Women with a fibrocystic disease should continue to do breast self-examination. By getting to know the texture of your own breast, whether they are lumpy, or nodular, and the location of the areas of tenderness, are all important points. When you do breast self-examination, you are feeling for a change. The analogy that another a physician has used for this is like looking for a rock or a boulder in a gravel road. Will this eventually or inevitably lead to breast cancer?
