New Laser Treatment for Farsightedness Offered at New York Weill Cornell Medical Center
New York, NY (August 3, 2000) A new procedure has been approved by the U.S. Food and Drug Administration (FDA) to correct farsightedness. Laser Thermal Keratoplasty (LTK) takes less than three seconds per eye and involves no cutting or removal of corneal eye tissue. Currently, the only site in the northeastern U.S. that offers LTK is the Weill Cornell Medical Center of NewYork-Presbyterian Hospital at its Laser Vision Correction Center. According to Sandra Belmont, MD, FACS, the Director of the Weill Cornell Laser Vision Correction Center, and one of the principal investigators for the FDA study, farsightedness, or hyperopia, is the most common refractive error among Americans. There are 60 million farsighted Americans over the age of 40 in the U.S. During the LTK procedure, two concentric rings of eight simultaneous spots of laser energy are applied to the mid-periphery of the cornea (outside of the optical zone). A new laser technique for farsightedness Tuesday, December 05, 2000 By Anita Srikameswaran, Post-Gazette Staff Writer Local eye surgeons are now offering an incision-free laser technique for farsightedness. Dr. Deepinder Deepinder Dhaliwal shows patient Claire Keyes of Point Breeze how a laser system would correct her farsightedness at the UPMC LaserVision Center. ( In laser thermokeratoplasty, or LTK, the laser beam heats a ring of tissue around the center of the eye to tighten collagen fibers there. That squeeze steepens the central cornea and corrects hyperopia, a condition in which objects are blurry whether close up or far away. Each eye is treated in a few seconds. Unlike Lasik, the new technique doesn't cut a flap into the cornea or remove any tissue, so the risk of complications is quite low. "You just put your chin in a chin rest and the laser does its thing," said Dr. Deepinder Dhaliwal, chief of refractive surgery for the Laser/Vision Center at the University of Pittsburgh Medical Center. "It's very, very safe." About 77 million people in the United States are farsighted, making the condition slightly more common than myopia or nearsightedness. In June, the Food and Drug Administration approved the laser equipment, made by Sunrise Technologies, for the "temporary" reduction of hyperopia from +0.75 up to + 2.5 diopters in patients aged 40 and older.

The key to understanding refractive surgery is to understand how the eye works. Normal Vision Sharp, clear vision is the result of light rays passing through the cornea, pupil and lens of the eye and focusing directly on the retina. If the cornea is not round, or is too steep or too flat in relation to the length of the eye, light rays focus either in front of or behind the retina. This results in "refractive errors" such as nearsightedness, farsightedness, and astigmatism. Laser vision correction improves the way the eye focuses light by modifying the curvature of the cornea and re-focusing the light directly on the retina. Nearsightedness Nearsightedness, or myopia, occurs when the curvature of the cornea is too steep or the eyeball is too long. With nearsightedness, the eye's strong refractive power forces the image to be focused in front of the retina, rather than on it. When light focuses in front of the retina, near objects can be seen clearly, but far objects appear blurry. Myopia can be treated by flattening the cornea with LASIK, PRK or LASEK

Some Johns Hopkins scientists have just moved us closer to that day: Surgeons at Johns Hopkins' Wilmer Eye Institute are now offering conductive keratoplasty, or CK, to correct low-level farsightedness in selected patients over age 40. The procedure, approved in April by the U.S. Food and Drug Administration, is the first non-laser treatment for hyperopia, a condition in which people can see objects far away but have trouble focusing on those nearby. It is an outpatient surgery performed under local anesthesia in just a few minutes. Unlike laser treatments, which use light waves as an energy source, CK uses radiofrequency waves, a form of electromagnetic energy, to re-shape the peripheral cornea. The energy is similar in some respects to the microwaves that power CB radios and cell phones. CK employs a pen-shaped instrument with a tip as thin as a human hair that releases the radiofrequency energy. The tip is applied in a circular pattern on the outer layer of the front of the eyeball to shrink small areas of tissue. The result is a constrictive band of tissue, similar to a tightened belt, that increases the overall curvature of the cornea.

Laser used to treat nearsightedness can now help the farsighted Portland, Ore. - Patricia Aieollo has worn two pairs of glasses for more than 18 years, one for driving and golf, the other to read and watch television. Aiello is farsighted, or hyperopic, meaning she has a problem seeing things close-up. Now, with the help of an excimer laser, she hopes to eliminate the need for her glasses all together. Aiello had her right eye treated with the laser in May, now she's getting her other eye treated on Fri., June 18 at 10:45 a.m. at the Casey Eye Institute at Oregon Health Sciences University. So far she's pleased with the results of the first procedure. "It's absolutely wonderful. I'm so excited to be getting the other one done, so I don't have to wear my glasses anymore. On the golf course the glasses were especially annoying because they kept sliding down," says Aiello. Last November, the Food and Drug Administration approved the use of the first laser surgery system to treat farsightedness, the Visx laser. Two lasers have been used to treat nearsightedness, or myopia, and myopia with astigmatism for many years. Photorefractive keratotomy (PRK) and laser in-situ keratomileusis (Lasik) both reshape cornea so light focuses more precisely on the retina. PRK shapes the cornea's surface; Lasik, a midcorneal method, reshapes the middle of the cornea rather than the front to improve focus.
