How the Urinary Bladder Works
If you are like most women, you take your urinary system for granted - until it causes problems. Most of us have enough going on in our busy lives and don't have time to worry constantly about how our bladder is doing.
Actually, your urinary system is really an amazing piece of work. Every time you eat, your body absorbs fluids into your blood stream. Every day, the billions of cells in your body generate waste products that are toxic and must be removed. Your two kidneys constantly filter your bloodstream to remove excess fluid and toxins from your body. This excess fluid, or urine, is transported down each ureter (YOUR-eh-tur) and stored in your urinary bladder (not to be confused with your gall bladder) until it is socially acceptable to empty. The bladder is a balloon-shaped muscular organ that changes shape as it fills with urine.

Is it possible to get "bruised ovaries" during sex?
09-Aug-2002 Dear Cecil: I have a question that only you might be able to answer. My brother claims to be well-endowed enough that a former "girlfriend" says he bruised one of her ovaries. Now, nothing is totally impossible, but how likely is it that this could happen with normal human beings? Are there ANY cases of it on record? What are the most common injuries from rough sex, anyway? Dwayne White, Southside High School Cecil replies: Not to suggest your brother was jerking you around, Dwayne, but we need to consider the physical realities. Do you have any idea where the ovaries are? Allow me to give you a little tour. Entering the female reproductive system by means of the vagina, we travel four to five inches until we reach the cervix, the muscled entrance to the uterus. The cervix normally is tightly closed, although it can dilate, not without difficulty, to allow passage of babies, IUDs (inserted through a straw), etc. Arriving in the uterus, a fist-sized organ located an inch or two below the waistline, we then make a sharp right or left turn to reach the fallopian tubes. The tubes are three to four inches long with an inner diameter the size of a human hair. They connect the uterus to a point near the ovaries, curving around a good deal in the process. In other words, to bruise an ovary directly, your brother would need something on the order of one of those scary prehensile tentacles in

Pelvic organ prolapse refers to descent (or prolapse) of the vaginal walls and/ or uterus below their normal positions. In severe cases, the vaginal walls or cervix protrude beyond the vaginal opening and are visible or palpable outside the body.
Many women with pelvic organ prolapse report problems with bladder and bowel functions. However, these symptoms are not always due to pelvic organ prolapse and therefore accurate diagnosis is very important. Symptoms that are often associated with pelvic organ prolapse include urinary incontinence, difficult urination, discomfort with sexual intercourse, stool incontinence, difficult defecation, low back pain and low abdominal pain.
Diagnostic procedures
In most cases, pelvic organ prolapse is diagnosed during a gynecologic (pelvic) examination. By observing the position of the vaginal walls and cervix, the physician can identify areas of prolapse. Usually, the woman is examined while straining or bearing down to demonstrate all areas of prolapse. The position of various sites of prolapse can be measured to identify areas of mild, moderate, or severe weaknesses in support.

Urge Incontinence : The inability to hold urine long enough to reach a toilet. There is a strong and sudden urge to urinate followed by an involuntary loss of urine. The ability to delay emptying the bladder is reduced or lost with urge incontinence. Sometimes people find environmental clues such as putting their keys in the door or hearing running water can set off the urge to go. For unknown reasons as the bladder ages, urge incontinence may occur and may get worse. A healthy person has a series of warnings or messages that the bladder is full and can postpone voiding until a socially acceptable time is found Stress Incontinence : The involuntary loss of urine during coughing, sneezing, laughing or other physical activities that increase abdominal pressure. The amount of urine lost may vary from dribbling to gushes of urine depending on how long it has been since the bladder was last emptied and how severe the problem. Stress incontinence may be caused by weakness of the pelvic floor muscles or from sphincter incompetence. As the muscles that support the bladder and external sphincter lose strength, they relax and shift downward; therefore bladder control may decrease.

Dear Dr. SerVaas: Recently, a doctor recommended that I have surgery to correct a fallen bladder. I have heard about a device called a pessary, which is inserted in the vagina to hold the bladder in place. Can you tell me about this device and whom I can contact to learn more about it? Available by prescription, pessaries are usually made of silicone and must be fitted by a healthcare professional. Doctors say that finding the most effective and comfortable pessary sometimes requires several fittings. Pessaries offer a safe and economical alternative to surgery for some women. Many can be worn for days at a time before they need to be removed and cleaned with soap and water. They are not contraceptive devices. CYSTOCELE
CYSTOCELE AND/OR UTERINE PROLAPSE A cystocele or uterine prolapse is not life-threatening. It does not cause or become cancerous. Attempted correction of these situations is actually elective. If a woman notices these symptoms, she should see her physician for evaluation. Treatment may involve trying to keep the bowel contents soft so there is less bearing down at the time of a bowel movement. Certain exercises to strengthen the muscles of the area called Kegel exercises may be prescribed. Surgical correction is often available. A pessary is a device that is placed in the vagina to help hold up the bladder and uterus. Your doctor may discuss this with you and fit you with a pessary. When a pessary is in place, the patient should not be able to feel it. If you have further questions concerning cystocele and/or uterine prolapse, please contact our office to make an appointment.
