How the LM causes GE reflux
Introduction
Gastro-esophageal reflux (GER) is the preeminent esophageal disorder. It causes or accompanies most other esophageal problems, making a major contribution to the sum of human misery. In the United States alone, the provision of over-the-counter antiacid remedies was a $700 million to $800 million a year industry in 1985 with sales exceeding even the $556,000,000 annual expenditures for cold remedies. Cimetidine has passed Valium as number one in the marketplace. About eight billion times a year, someone in the US is sufficiently distressed to take a Tum, Rolaid or Alka-Seltzer - a staggering statistic that does not even include the (forbidden) usage of bicarbonate of soda.
Although current opinion leans toward the view that reflux is primarily due to hypotension of the LES, I will show that reflux is an effect of LMC. Because an open sphincter will result in reflux, the next proposition to be proved is this:
LM contraction (LMC) is the efficient cause of reflux.

Gastro Esophageal Reflux Disease GERD Almost everyone has experienced heartburn. About 25 million American adults suffer daily from heartburn. It is that burning sensation felt behind the breastbone and sometimes in the neck and throat. Heartburn is caused by stomach acid refluxing or splashing up into the esophagus the muscular tube that connects the throat to the stomach. Occasional heartburn is nothing to be concerned about. However, anyone who has heartburn on a regular basis should consult a physician. Constant exposure to stomach acid can irritate the lining of the esophagus and cause other medical problems. Serious heartburn is known as gastroesophageal reflux disease or GERD. What is GERD?
The esophagus carries food and liquid into the stomach. At the lower end of the esophagus where it enters the stomach, there is a strong muscular ring called the lower esophageal sphincter (LES). The LES should remain tightly closed, except to allow food and liquid to pass into the stomach. Reflux occurs when the LES is not working properly. It may relax for periods of time throughout the day and night, or it may be constantly too weak to function effectively. This allows the stomach's acid juices to flow into the esophagus. How severe the disease becomes depends on how weakened the LES is, and the amount and duration of acid refluxed into the esophagus. It is also common to find a hiatal hernia complicating GERD. With a hiatal hernia, the upper part of the stomach actually pushes up into the chest through a weakness in the diaphragm. The diaphragm is the thin, flat muscle that separates the lungs from the abdomen. When part of the upper stomach is stuck above the diaphragm, stomach acid is retained there for a longer period and is more likely to reflux into the esophagus.

Breast feeding has generally been promoted as an infant health issue. In fact, there are benefits to the mother’s health as well. Some women, with and without implants, have problems nursing. No studies have implicated implants per se as the cause of such difficulties in mothers with breast implants. Women with previous breast surgery (biopsy, reduction, augmentation, other chest surgery) may have a greater risk for lactation insufficiency compared with women who have not had breast surgery. The location of the incision into the breast may have the most significant impact on subsequent lactation ability. While location of the incision is an important consideration, the surgeon’s careful management of the glandular breast tissue also has significant impact on a woman’s ability to breast feed after breast surgery. Researchers in Denmark have assessed the occurrence of esophageal malfunctions and disorders, connective tissue disease, other rheumatic conditions, and congenital malformations in children born to mothers with cosmetic breast implants.

Esophageal Disease Profiled AIDS Clinical Care (09/95) Vol. 7, No. 9, P. 77 A prospective study in Atlanta cataloged the etiology and course of esophageal ulceration in HIV-positive individuals who were diagnosed between 1990 and 1994. All 100 subjects had symptomatic esophageal ulcer identified on endoscopy, and the majority had longstanding HIV infection, with an average of 15 CD4 cells. The researchers tabulated syndromes including cytomegalovirus esophagitis in 50 subjects, herpes simplex esophagitis in nine, idiopathic esophageal ulceration in 41, and gastroesophageal reflux-associated ulceration in four. For the most part, clinical and endoscopic response to therapy was good, although 27 patients relapsed. These findings demonstrate the shifting microbial etiologies of HIV- related disorders and the need for specific tissue diagnoses.

It typically occurs transiently after eating. Gastro esophageal reflux disease(GERD) is the condition where degree of exposure of esophageal mucosa to gastric contents is greater than normal. The most common symptom is heartburn but patients may have other symptoms. The modern concept of reflux esophagitis appears to emerge in a publication by Winkelstein in 1935 when he said: Ò One canÕt escape the suspicion that the disease in these cases is possibly a peptic esophagitis i.e. an esophagitis occurring as a result of acid and pepsinÓ. You may ask: So what is the big deal. Well folks, It really is a big deal. Did U know that according 1988 survey, more than 61 million or44% of adult Americans suffer at least once monthly. Did you know that as many as 13% of adult Americans reported taking antacids two or more times per week. As many as 7% of adult Americans have daily occurrence of heart burn. As such, you would agree with me that this disorder has potential for considerable morbidity and socio-economic consequences.

The esophagus is the muscular tube which propels food from the mouth into the stomach. In the dog it is made up primarily of skeletal muscle. The esophagus normally has tone like other muscles in the body and is closed when it is not being used. Megaesophagus however denotes an esophageal disease where there is no tone to the esophagus and it becomes flaccid and enlarged. What are the symptoms?
Typically patients with megaesophagus will regurgitate their food. This is different from vomiting in that the food passively comes up from the esophagus versus the active abdominal compression that is involved with vomiting. The constant regurgitation of food and liquids predisposes these patients to aspiration of stomach contents into the airways and lungs. They then develop pneumonia so coughing, lethargy, labored breathing or loss of appetite may also be among the first signs noted in these patients. How is megaesophagus diagnosed?
