Overview Erectile dysfunction (ED) affects the lives of 15 million to 30 million American men and their partners. The term erectile dysfunction covers a range of disorders, including curvature of the penis during erection (Peyronie's disease), prolonged painful erection not associated with sexual desire (priapism) and premature ejaculation. But usually it refers to the inability to obtain an adequate erection for satisfactory sexual activity. Although erectile dysfunction is more common in men older than 65, it can occur at any age. An occasional episode of erectile dysfunction happens to most men and is perfectly normal. In fact, in most cases it's nothing to worry about. As men age, it's also normal to experience changes in erectile function. Erections may take longer to develop, may not be as rigid or may require more direct stimulation to be achieved. Men may also notice that orgasms are less intense, the volume of ejaculate is reduced and recovery time increases between erections.

On this page: Erectile dysfunction, sometimes called "impotence," is the repeated inability to get or keep an erection firm enough for sexual intercourse. The word "impotence" may also be used to describe other problems that interfere with sexual intercourse and reproduction, such as lack of sexual desire and problems with ejaculation or orgasm. Using the term erectile dysfunction makes it clear that those other problems are not involved. In older men, ED usually has a physical cause, such as disease, injury, or side effects of drugs. Any disorder that causes injury to the nerves or impairs blood flow in the penis has the potential to cause ED. Incidence increases with age: About 5 percent of 40-year-old men and between 15 and 25 percent of 65-year-old men experience ED. But it is not an inevitable part of aging. ED is treatable at any age, and awareness of this fact has been growing. More men have been seeking help and returning to normal sexual activity because of improved, successful treatments for ED. Urologists, who specialize in problems of the urinary tract, have traditionally treated ED; however, urologists accounted for only 25 percent of Viagra mentions in 1999.

Erectile dysfunction doesn't have to be a part of getting older. It's true that as you get older, you may need more stimulation (such as stroking and touching) to get an erection. What is Topiglan? Topiglan is MacroChem's Phase 2 investigational topical drug for treating erectile dysfunction. Topiglan, first formulated as a hydro-alcoholic gel, and now as a cream, contains MacroChem's proprietary absorption-enhancer, SEPA®, and alprostadil (synthetic prostaglandin E1 - "PGE1"). In August 1999, MacroChem received U.S. Patent 5,942,545 on this SEPA/PGE1 agent. In 2001, a European patent application for Topiglan was allowed. Additional international Topiglan patents are pending. PGE1 is a natural compound first isolated from human ejaculate, and discovered to have a relaxing effect on the smooth muscle lining of arterial walls. However, as an erectile dysfunction agent, it acts only when injected locally, into the cavernosa of the penis, or inserted as a suppository into the urethra. Ordinarily, alprostadil does not appear to readily penetrate intact skin.

  • Erectile dysfunction (ED) is a common condition and can usually be managed pharmacologically, with drugs delivered by intracavernosal injection (ICI), transurethrally or orally.
  • The cardiovascular status of the patient and his overall fitness for renewed sexual activity must be assessed before treatment for ED is initiated.
  • The efficacy of sildenafil is related to the extent and severity of ED, and is significantly reduced in patients with severe vasculogenic ED, ED associated with diabetes and after radical prostatectomy.
  • Alprostadil (prostaglandin E ) is the drug of first choice in patients treated with ICI; it is effective in 72.6% of men with ED and is associated with a low risk of priapism and cavernosal fibrosis.
  • Transurethral alprostadil is significantly less effective than alprostadil ICI, producing improved erections in 30%-40%, but rigid erections in only 10%, of men with ED.
  • There is Level II evidence that:
    • alprostadil ICI is an effective treatment for ED
    • papaverine ICI is associated with a high risk of cavernosal fibrosis and priapism
    • papaverine ICI should be restricted to informed patients refractory to treatment with alprostadil ICI
    • transurethral alprostadil is less effective than alprostadil ICI
    • sildenafil is an effective treatment for ED.

What Is Erectile Dysfunction? Erectile dysfunction (ED) is the inability to obtain and maintain an erection that is firm enough for a man to have intercourse, or the inability to keep an erection long enough to ejaculate inside the vagina. It is a newer and better name for what was previously called impotence. It can happen in one of two ways:
  • A man may be unable to get an erection at all. A man may lose his erection during intercourse, before he ejaculates.
A man is considered to have erectile dysfunction if he is unable to get or maintain an erection in at least 50 percent of his attempts at intercourse. Erectile dysfunction is a problem for both the man and his partner. In most cases, it is a temporary condition that will disappear with little or no treatment. In others, it can be an ongoing difficulty that requires treatment. In most men, erectile dysfunction does not affect their sex drive. ED also does not affect a man's ability to have an orgasm once he achieves an erection.

The most common health disorder to inflict older men is not hypertension or arthritis, but instead erectile dysfunction (ED, for short). In the United States alone between 10-20 million males are effected with some form of severe erectile dysfunction. If we include less severe degrees of erectile dysfunction, this estimate might reach 30 million! Erectile dysfunction typically appears in males in their fifth decade of life. It is estimated that approximately 50% of all males between the ages of 50 and 70 exhibit some degree of erectile dysfunction. By stark contrast, at 40 the incidence of erectile dysfunction is on average considerably less, or about 5%. Nevertheless, certain environmental factors such as drug abuse, stress, anabolic steroids, poor diet and bad health, can accelerate the expression of erectile dysfunction causing it to manifest at an earlier age than is normal. Sadly, despite the high incidence of erectile dysfunction in older males, less than 5% ever seek treatment. Erectile dysfunction IS NOT an inevitable consequence of aging!
