WHAT IS IMPOTENCE (ERECTILE DYSFUNCTION)? Impotence is the inability to achieve or maintain an erection sufficiently rigid for sexual intercourse, ejaculation, or both. Sexual drive and the ability to have an orgasm are not necessarily affected. Impotence is medically defined as the inability to sustain an erection sufficient for intercourse on at least 25% of attempts.
Impotence is not new in medicine or human experience, but it is not easily or openly discussed. Erectile dysfunction (ED), also known as impotence, is the inability to achieve or sustain an erection for satisfactory sexual activity. Erectile dysfunction is different from other conditions that interfere with sexual intercourse, such as lack of sexual desire and problems with ejaculation and orgasm. This article focuses on the evaluation and treatment of erectile dysfunction. How common is erectile dysfunction?
Erectile dysfunction (ED, impotence) varies in severity; some men have a total inability to achieve an erection, others have an inconsistent ability to achieve an erection, and still others can sustain only brief erections. The variations in severity of erectile dysfunction make estimating its frequency difficult. Many men also are reluctant to discuss erectile dysfunction with their doctors, and thus the condition is under-diagnosed. Nevertheless, experts have estimated that erectile dysfunction affects 30 million men in the Untied States. While erectile dysfunction can occur at any age, it is uncommon among young men and more common in the elderly. By age 45, most men have experienced erectile dysfunction at least some of the time. According to the Massachusetts Male Aging Study, complete impotence increases from 5% among men 40 years of age to 15% among men 70 years and older. Population studies conducted in the Netherlands found that some degree of erectile dysfunction occurred in 20% of men between ages 50 to 54, and in 50% of men between ages 70 to 78. In 1999, the National Ambulatory Medical Care Survey counted 1,520,000 doctor-office visits for erectile dysfunction.

DEFINITION Erectile dysfunction is the inability to develop and maintain an erection for satisfactory sexual intercourse or activity (in the absence of an ejaculatory disorder such as premature ejaculation). Erectile dysfunction is the preferred term rather than the more commonly used term of impotence. There is no universally agreed upon criteria for how consistent the problem has to be and for what duration it needs to be present to fulfill the definition. A period of persistence over 3 months has been suggested as a reasonable clinical guideline.

When a man becomes sexually aroused, increased blood flow to the genital area readies the body for intercourse. The penis becomes enlarged and erect. In men with erectile dysfunction (ED), however, this physical response doesn't happen as it should. And this isn't just a one-time or occasional occurrence. In fact, occasional failure to become aroused or desiring sex less often than your partner is perfectly normal. Stress, fatigue and anxiety can affect the body's response to sexual stimulation. The problem occurs when this lack of response happens persistently and on a regular basis for more than 25 percent of the time. With ED, intercourse is difficult or impossible. ED is not only a common problem, particularly among older men, but also it is undertreated. A few years ago, the Massachusetts Male Aging Study of middle-aged and older men showed 35 percent of men ages 40 to 70 years had complete ED, which was strongly related to age, health status and emotional function. According to the American Medical Association (AMA), about 20 million American men, mostly older than 65, are affected.
