    Erectile Dysfunction Treatments
There is now a major set of stories on erectile dysfunction and erectile dysfunction treatments in Bandolier 's Sexual Health pages.
Conditions that are common often become expensive if an effective treatment comes along. For erectile dysfunction what most of us know (or think) is that treatments are reserved for funny boxes in the pages of our more popular newspapers. We might be dimly aware that a number of physical treatments have been used, including semi-rigid or inflatable implants, vacuum constriction devices, and vascular surgery. Some of these can be effective in some men, but they can be expensive and may not be appropriate in every affected man.
    New treatments
Bandolier has noticed that new pharmacological treatments are being reported in large and well-conducted trials in our top medical journals. Alprostidil (prostaglandin E1) relaxes corpora cavernosal smooth muscle in vitro, and is effective in the treatment of impotence. Another treatment is sildenafil, which is a phosphodiesterase inhibitor, which also relaxes cavernosal smooth muscle.
The new treatments are effective but delivery systems have been complicated. The idea of intracavernosal injections brings tears to the eyes of many. Intraurethral application seems better, but a pill? Now you're talking.

erectile dysfunction. Introduction. Using the term erectile dysfunction makes it clear that those other problems are not involved.

    ERECTILE DYSFUNCTION
    Introduction
Erectile dysfunction, sometimes called "impotence," is the repeated inability to get or keep an erection firm enough for sexual intercourse. The word "impotence" may also be used to describe other problems that interfere with sexual intercourse and reproduction, such as lack of sexual desire and problems with ejaculation or orgasm. Using the term erectile dysfunction makes it clear that those other problems are not involved. In older men, ED usually has a physical cause, such as disease, injury, or side effects of drugs. Any disorder that causes injury to the nerves or impairs blood flow in the penis has the potential to cause ED. Incidence increases with age: About 5 percent of 40-year-old men and between 15 and 25 percent of 65-year-old men experience ED. But it is not an inevitable part of aging. ED is treatable at any age, and awareness of this fact has been growing. More men have been seeking help and returning to normal sexual activity because of improved, successful treatments for ED. Urologists, who specialize in problems of the urinary tract, have traditionally treated ED; however, urologists accounted for only 25 percent of Viagra mentions in 1999.

    Yohimbine for erectile dysfunction update
    Clinical bottom line
Yohimbine has been tested against placebo in 10 randomised trials with 659 men. Yohimbine provided satisfactory erections in 30% of men compared with 14% with placebo. The NNT compared with placebo for one man to achieve satisfactory erections was 6.4 (4.6 to 11). Adverse events were generally mild, though some men withdrew from treatment because of them.
    Study
It was decided to update the review by Ernst and colleagues , because there were more trials that could be added and in order to calculate NNTs. Additional searching was done using PubMed, Medline and the Cochrane Library, up to end 2000. Randomised trials in which yohimbine was compared with placebo were sought. Details of the trials were abstracted and quality scoring done with a 5 point scale. For crossover or partial crossover designs, details of the first phase were used (as a parallel group trial) where possible, and where this was not possible the full crossover data was used. The outcome sought was patient/partner judgement of satisfactory erections suitable for intercourse, or actual intercourse, at home. Ideally this was on a patient basis, rather than on event basis, which was a secondary outcome. Relative risk and NNT were calculated using standard methods.

(pa- pav- er-een) Primary Usage in MS Generic Available Erectile dysfunction Yes This medication is taken by injection. Description
Papaverine belongs to a group of medicines called vasodilators, which cause blood vessels to expand, thereby increasing blood flow. Papaverine is used in MS to treat erectile dysfunction. When papaverine is injected into the penis, it produces an erection by increasing blood flow to the penis. Proper Usage
Papaverine should never be used as a sexual aid by men who are not impotent. If improperly used, this medication can cause permanent damage to the penis. Papaverine is available by prescription and should be used only as directed by your physician, who will instruct you in the proper way to give yourself an injection so that it is simple and essentially pain-free. Precautions
Do not use more of this medication or use it more often than it has been prescribed for you. Using too much of this medicine will result in a condition called priapism, in which the erection lasts too long and does not resolve when it should. Permanent damage to the penis can occur if blood flow to the penis is cut off for too long a period of time. Examine your penis regularly for possible lumps near the injection sites or for curvature of the penis. These may be signs that unwanted tissue is growing (called fibrosis), which should be examined by your physician.
