Nuri ARDA Dokuz Eylül University School of Medicine, Department Neurosurgery, Inciralti, Izmir , Turkey ABSTRACT A 67-year-old male was operated for a recurrent L4-5 disc herniation. Intraoperative corticosteroid was used topically on epidural space before closure. 15 days after surgery he was readmitted because of severe pain in low back and lower extremities. A lumbar epidural abscess was diagnosed and he was reoperated, his abscess was drained and he was treated with parenteral antibiotics. In this article effect of intraoperative topical epidural corticosteroid usage on postoperative epidural abscess is discussed and literature is reviewed. It is stated that, postoperative lumbar epidural abscess is a benign disease in case treated properly. Surgical drainage and 3-4 weeks of parenteral antibiotherapy is adequate.     Married, lives with wife in Maryland and previously lived in West Virginia, works in dry cleaning industry, and denies alcohol, tobacco, or illicit drug use. They have two children, both alive and well, and they own no pets.
    Physical Examination He was afebrile with other vital signs within normal parameters. General - obese man who appears to be in no distress. Skin - no rash or lesions. HEENT - no significant abnormalities. Neck - supple without lymphadenopathy. Chest, heart, and abdomen were normal, although rectal tone was diminished and he had mid back spinal tenderness. Extremities - without edema. Neuro - oriented, cranial nerves II-XII intact, diminished sensation below T10 level, 3/5 strength in his right lower extremity and 2/5 strength in his left lower extremity, normal strength in his upper extremities, and normal reflexes in his upper extremities but absent reflexes in lower extremities with concomitant up-going toes.

    Abstract
    Dissemination of primary cutaneous nocardiosis is a rare event. An interesting case of a 20 year old female labourer with progressive weakness in both the lower limbs and large multiple subcutaneous abscesses over the back, since 4 years, is presented. MRI showed an epidural abscess compressing the cord. Histopathology of skin lesions suggested a chronic suppurative lesion.     Staphylococcal osteomyelitis with epidural abscess
    A 56 year old man with falls
    This 56 year old man had a five year history of Parkinson's disease. Six months earlier he had been treated for septicaemia, and had made a full recovery. Over recent weeks he had suffered an increasing number of falls. He was admitted and again found to be septicaemic, for which he was treated. However he also complained of pain in the groin, and was noted to have bilateral weakness and spasticity of the legs. Investigations revealed a lytic bone lesion in the right superior pubic ramus. However X-rays of the spine were reported to show only some degenerative change in the cervical region. An MRI of the spine was obtained to investigate his leg weakness. Further images were obtained following contrast.
    Findings
    The T1 weighted image shows reduced signal from the marrow of the C6 and C7 vertebral bodies, with narrowing of the C6/7 intervertebral disc. There is additional soft tissue posteriorly which impinges on the cord. The post contrast images demonstrate marked enhancement of the marrow of the affected vertebral bodies, and also of the posterior extension.

When our 5 year old daughter was diagnosed with a Brainstem Glioma, the hospital and Child Cancer Foundation (in New Zealand) gave us books to read, if you wanted to wade through all the technical stuff, personally I couldn't cope with it all. Then when I was looking at sites on the internet, I read about this book. I had a look at it at Amazon, and thought about it for a few days, trying to weigh up if it would be relevant to NZ'ers. I finally took the plunge and got it. I am soooo relieved that I did. Suddenly I was able to read something and actually get some benefit from it. The inserts from the parents are wonderful - suddenly I don't feel so alone, and the advice is universal. It relates to anyone in any country going through this nightmare. This book is a "must have" for caregivers As a parent to a child who had a brain tumor, I feel that this book covers all the major topics. When our son was diagnosed back in '98, I searched for such a book but none existed. Our son has since passed away but we are thankful that this book is available - particularly for newly diagnosed families. We purchased copies for our public library and for The Jimmy Fund in memory of our three year-old son, Kevin Kirsch. Thank you for writing this much needed guide. It will be a resource for caregivers and medical professionals alike.
