Spinal epidural abscess has an estimated incidence rate of 0.2 to 2.8 cases per 10,000 per year, with the peak incidence occurring in people who are in their 60s and 70s. The most common causative agent is Staphylococcus aureus.(1,2) Epidural abscess caused by actinomycosis is rare; fewer than 80 cases have been reported since the organism was identified in 1878.(3,4) The incidence of spinal epidural abscess appears to have increased in the United States since the 1980s, possibly because of an increase in the age of the population, the number of spinal procedures performed,(5,6) intravenous (IV) drug abuse, and the number of patients with acquired immunodeficiency syndrome (AIDS).(7) Associated predisposing conditions include a compromised immune system such as occurs in patients with diabetes mellitus, AIDS, chronic renal failure, alcoholism, or cancer,(8-10) or following epidural anesthesia, spinal surgery, or trauma(11) (Table 1). No predisposing condition can be found in 20 percent of patients with spinal epidural abscess, and the condition has been reported in patients with no predisposing risk factors.(12)

Spinal Epidural Abscess
Etiology
Although virtually any organism can cause SEA, Staphylococcus aureus causes 60-90% of all cases. Less common pathogens include M. tuberculosis, fungi, and even parasites.
Diagnosis
The combination of back pain, fever, and local vertebral tenderness upon palpation or percussion of the spine strongly suggests a SEA. This triad constitutes adequate reason to obtain a Magnetic Resonance Imaging (MRI) scan and to request neurosurgical help in evaluating the patient. Approximately 66% of patients with SEA present within two weeks of the development of symptoms. Historically, SEA has been characterized by four clinical stages: Stage 1 - Spinal ache; Stage 2 - Root Pain; Stage 3 - Weakness; and Stage 4 - Paralysis. As damage to the spinal cord progresses, the patient may exhibit bowel and bladder dysfunction, weakness or paralysis of the extremities, and sensory deficits. However, neurologic deterioration is highly variable and may not progress from one stage to the next in an orderly manner. Because it is not uncommon for acute SEA to progress from Stages 1 or 2 to Stage 4 (paralysis) in a matter of hours, the utility of staging SEA is questionable.
Therapy
Traditionally, SEA has been considered a neurosurgical emergency. Standard management of patients with non-tuberculous SEA (NT-SEA) is immediate surgical drainage and decompression followed by six or more weeks of antimicrobial therapy directed against the organism isolated at surgery. Because

Spinal Epidural Abscess
Kenneth H. Butler, DO, FACEP Back pain is one of the most common complaints among individuals who come to emergency departments (EDs). As for most ED presentations, the physician will initially consider life-threatening conditions (e.g., ruptured abdominal aortic aneurysm) as well as nonurgent conditions (e.g., musculoskeletal strain); unfortunately, spinal epidural abscess (SEA) is rarely included in the differential. Many patients in whom SEA should be considered have a history of narcotic abuse and are written off as coming to the ED to refill their narcotic prescriptions. Others are “bounce-back” patients, who were seen recently in an ED or an outpatient clinic for assessment of back pain and discharged with analgesics but who continued to have pain and thus have returned for medical help. The cornerstone of the diagnosis of SEA lies in contemporary neuroimaging studies. Magnetic resonance imaging (MRI) with gadolinium is currently the diagnostic procedure of choice. Prompt neurosurgical consultation should be initiated as early as possible. Traditionally, the management of SEA has consisted of immediate surgery followed by antibiotic therapy. Surgical management has advantages, most importantly the ability to decompress compromised neural tissues, thereby preventing further neurologic deterioration. In certain patients who are neurologically intact and for whom the surgical morbidity rate is unacceptably high, conservative medical management may be appropriate. An acute epidural abscess should be suspected in patients with back pain and fever, particularly in those who have abused drugs intravenously.

  • This Danish study comments on previous data reporting an incidence of 0/9,232 cases in Sweden, 2/13,000 in Germany, and 3% in a small US study using epidurals for long term management of chronic pain. The present multicenter prospective study collected data on epidural abscesses as they occurred. A denominator was calculated from the number of epidurals placed at each participating hospital.
Results:
  • 17,372 epidurals were placed during the study period. 12 possible abscesses were found. 9 were real (2 were subcutaneous infections, 1 a complication from catheter placement):

Radiological Findings: Sagittal T2-weighted MR images show an epidural fluid collection centered around the C4/5 disk space ( Fig.1 ). Anterior to the vertebral column there is also slightly increased T2 signal indicating a phlegmon and less likely a fluid collection. These each measure approximately 4 mm in AP dimension. The cervical cord is displaced posteriorly and there is compression of the cord but there is no appreciable signal alterations. The fluid collection is in a left paracentral region ( Fig. 2
