Definition: Epididymitis is an inflammation of the epididymis (the tubular structure that connects the testicle with the vas deferens).
Causes, incidence, and risk factors: Acute epididymitis causes swelling of the scrotum, pain in the testicles, and sometimes a fever of six weeks duration or less (usually with a gradual onset over several days). If not treated, or in some other cases, the condition can become chronic. In chronic cases, there is usually no swelling, but simply pain. The incidence is approximately 600,000 cases per year. The highest prevalence is in young men 19 to 35 years of age. The disorder is a major cause of hospital admissions in the military (causing approximately 20% of admissions). Diagnostic Considerations Men who have epididymitis typically have unilateral testicular pain and tenderness; hydrocele and palpable swelling of the epididymis usually are present. Testicular torsion, a surgical emergency, should be considered in all cases but is more frequent among adolescents. Torsion occurs more frequently in patients who do not have evidence of inflammation or infection. Emergency testing for torsion may be indicated when the onset of pain is sudden, pain is severe, or the test results available during the initial examination do not enable a diagnosis of urethritis or urinary tract infection to be made. If the diagnosis is questionable, an expert should be consulted immediately, because testicular viability may be compromised.

The epididymis is a crescent shaped structure. It runs down the middle of the back of the testis from top to bottom. It functions to carry the sperm from the testes (where they are produced) to the vas deferens, which is a muscular tube that carries the sperm. The epididymis is more susceptible to infection than the other structures in the scrotum and this infection is called epididymitis. Infections can subsequently lead to scar tissue, which can block the sperm from moving through it and trap it in the testes (or the part of the epididymis before the blockage.) The sperm are still being produced; they are just not able to come through. If this happens on both sides a man may have no sperm in the ejaculate. Definition
Acute epididymitis is an inflammation of the epididymis. This is a tubular structure that surrounds and attaches to each testicle. The epididymis helps transport and store sperm cells produced in the testicles. Chronic epididymitis causes pain and inflammation in the epididymis. There is often no swelling of the scrotum. Symptoms last for six weeks or more. This type of epididymitis is less common.
Causes
Epididymitis is most often caused by a bacterial infection. For example: Other causes of epididymitis include:
  • Injury Viruses, such as the mumps Genital abnormalities Treatment with the heart rhythm drug Amiodarone (Cordarone)
Risk Factors
A risk factor is something that increases your chances of getting a disease or condition. Only men can develop epididymitis. Other risk factors include:

GENERAL INFORMATION Epididymitis is an inflammation of the epididymis, an oblong structure attached to the upper part of each testis. The inflammation may be so great that the testicle itself feels greatly enlarged and hardened. However, this does not mean that the testicle itself is infected.
Some rarer types of bacteria create a different picture altogether. Tuberculosis, for instance, is a bacteria that can spread to the epididymis in unusual circumstances. Tuberculosis epididymitis develops very slowly over days and weeks. Non bacterial epididymitis simply means an inflammation of the epididymis not caused by a bacteria. The causes are many and include viruses, trauma, and unknown causes. The unknown causes are commonly referred to in medicine as 'idiopathic' - an unknown cause. Non-bacterial epididymitis can be just as painful and with a very quick onset. The viral types may be associated with fevers and generalized weakness and ill feeling. Epididymitis caused by trauma or the 'idiopathic' types are not usually associated with the fevers and chills and generalized symptoms. Many believe that these types of epididymitis are caused by a rupture or blow-out of the sperm ducts causing a leakage of sperm fluid into the tissue. The body tissues can be quite irritated by this leakage and hence the redness and swelling. The treatment for these types of epididymitis may include antibiotics just to prevent bacteria from infecting the area whose resistance has been weakened.

Presentation
An adult male complains of dull to severe scrotal pain developing over a period of hours to a day, and radiating to the ipsilateral lower abdomen or flank. There may be a history of recent urethritis, prostatitis or prostatectomy (allowing ingress to bacteria), straining with lifting a heavy obiect, or sexual activity with a full bladder (allowing reflux of urine). There may be fever, nausea, or urinary urgency or frequency. The epididymis, is tender, swollen, warm, and difficult to separate from the firm, nontender testicle. Increasing inflammation can extend up the spermatic cord and fill the entire scrotum, making examinations more difficult, as well as produce frank prostatitis or cystitis. The rectal exam therefore may reveal a very tender, boggy prostate.
What to do:
  • Ascertain that the testicle is normal in position and perfusion. Doppler ultrasound may help pick up a drop-off in arterial flow from spermatic cord to testicle in testicular torsion.
  • Palpate and ausculate, the scrotum to rule out a hernia. Gently palpate the prostate once. Culture urine and/or any urethral discharge to identify a bacterial organism.

It presents as sudden painful scrotal swelling. There usually is pus in the urine. The germ that causes it is usually bacterial; we are not always successful in figuring out exactly which germ it is. Surgical exploration is often the only way to be sure, and must be undertaken very quickly to save the testicle if torsion is present. (25-Mar-04) I was recently diagnosed with Epidiymitis. The doctor took a urine sample and ran lab tests. I have no discharge and no pain urinating.
The tests came back negative for bacteria. He told me it was safe to have sex and I could not transfer this to my partner.
However, I did not get a good explanation for what caused this. Everything I see on the web says it is caused by chlamydia bacteria which is sexually transmitted.
Can I be sure that I do not have chlamydia since it did not show up in the urine tests and I have no other symptons that are described with chlamydia? A Forum-M.D.-KYP
(28-Mar-04) dcnelsonmn Chlamydia is the most common cause for epididymitis, but other bacteria can be responsible as well. Bacteria from UTIs as well as prostatitis can also cause epididymitis. Less commonly, gonorrhea can cause epididymitis.
A digital rectal exam for prostatitis as well as culturing and analyzing the prostatic secretions can be considered to look for the responsible bacteria.
If the urine tests as well as urethral swabs are negative for chlamydia, it is unlikely that it is present.

(25-Apr-04) My Problem all began about 6 to 7 months ago. My symptoms started as jock itch, then became severe dysuria, I had horrible shocking pains in my testicles(mainly the right one),severe constipation, flu like symptoms, and severe fatigue. I was tested for Chlamydia/GC,HSV,AIDS, and lymes disease. All came back negative.
I was treated with more than one type of antibiotic.Primarily doxycyline. The antibiotics did not help at all.My doctors then believed I had a viral problem because I was not responding to antibiotics.
My general doctors were baffled.I was diagnosed with Epididymitis rather quickly when I visited a second urologist. I had a ultrasound done on my testicles, the ultrasound report said that it was a negative study other than a tiny cyst in the head of my right epididymis.I have had multiple urinalysis done, no problem ever found. The urologist heard my symptoms and read my report and quickly decided I had epididymitis. He told me to take hot baths and wear tight underwear.
At that point my symptoms were painfull dysuria and severe pain in my testicle that would get worse as the day went on.
