What is epididymitis?
Epididymitis is an inflammation of the epididymis. The epididymis is the tube through which sperm passes. The tube is located at the back of each testicle. Epididymitis can affect both testicles, but it most often occurs on just one side.
How does it occur?
Epididymitis is caused by bacterial infections, such as:

Epididymitis is inflammation or infection of the epididymis. In this long coiled tube attached to the upper part of each testicle, sperm mature and are stored before ejaculation. Description Epididymitis is the most common cause of pain in the scrotum. The acute form is usually associated with the most severe pain and swelling. If symptoms last for more than six weeks after treatment begins, the condition is considered chronic. Epididymitis can occur any time after the onset of puberty but is most common between the ages of 18 and 40. It is especially common among members of the military who exercise for extended periods without emptying their bladders. Factors that increase the risk of developing epididymitis include:
  • infection of the bladder, kidney, prostate, or urinary tract
  • other recent illness
  • narrowing of the urethra (the tube that drains urine from the bladder) use of a urethral catheter
Causes and symptoms Although epididymitis can be caused by the same organisms that cause some sexually transmitted diseases (STDs) or occur after prostate surgery, the condition is generally due to pus-generating bacteria associated with infections in other parts of the body.

Male Reproductive System Acute epididymitis causes swelling of the scrotum, pain in the testicles, and sometimes a fever of six weeks duration or less (usually with a gradual onset over several days). If not treated, or in some other cases, the condition can become chronic. In chronic cases, there is usually no swelling, but simply pain. The incidence is approximately 600,000 cases per year. The highest prevalence is in young men 19 to 35 years of age. The disorder is a major cause of hospital admissions in the military (causing approximately 20% of admissions).

WHAT YOU SHOULD KNOW
Epididymitis (EP-ih-DID-ee-MY-tis) is an infection of the epididymis, a tube located behind the testicle. The infection is common in men 19 to 35 years old.
Causes
The problem may start with an infection of the bladder or prostate gland. The infection is sometimes contracted through sexual contact.
Signs/Symptoms
The usual symptoms are fever, pain, redness, and swelling of the scrotum and painful urination. You also may feel a lump in your scrotum.
Care
Your doctor will prescribe an antibiotic to fight the infection. Rest until you feel better, and follow the directions listed below.
WHAT YOU SHOULD DO
  • Rest in bed until fever, pain, and swelling go down. The testicle may stay swollen and hard for several days or even a few weeks.
  • If your doctor prescribes an antibiotic to fight the infection, take it exactly as directed and finish the entire prescription. If you stop taking the drug too soon, a few germs may survive and re-infect you.

Scrotal disorders may be due to inflammation of the scrotal wall or contents, trauma (see below), neoplasms of the testis or testicular appendages (see Ch. 233 ), or mechanical abnormalities involving the scrotal contents or adjacent structures (see Testicular torsion under Renal and Genitourinary Defects in Ch. 261). Scrotal abscesses may complicate epididymo-orchitis (see under Epididymitis, below), especially when treatment is delayed. Some abscesses drain spontaneously, but most require surgery, usually orchiectomy with drainage. Urethral stricture and diverticulum may be accompanied by abscess formation with swelling of the scrotum, pain, and erythema and extravasation of urine into the scrotum and perineum. Incision and drainage as well as antibiotics are often indicated, but urinary diversion by suprapubic cystostomy is needed only in severe cases with abscess. Persistent strictures can usually be treated successfully by dilation or endoscopy (internal urethrotomy). In difficult or recurrent cases, a permanent stent may be placed endoscopically. Epididymitis caused by sexually transmitted enteric organisms (e.g., Escherichia coli Although most patients can be treated on an outpatient basis, hospitalization should be considered when severe pain suggests other diagnoses (e.g., torsion, testicular infarction, or abscess) or when patients are febrile or might be noncompliant with an antimicrobial regimen.
Diagnostic Considerations
Men who have epididymitis typically have unilateral testicular pain and tenderness; hydrocele and palpable swelling of the epididymis usually are present. Testicular torsion, a surgical emergency, should be considered in all cases, but it occurs more frequently among adolescents and in men without evidence of inflammation or infection. Emergency testing for torsion may be indicated when the onset of pain is sudden, pain is severe, or the test results available during the initial examination do not support a diagnosis of urethritis or urinary-tract infection. If the diagnosis is questionable, a specialist should be consulted immediately, because testicular viability may be compromised. The evaluation of men for epididymitis should include the following procedures.
