- Eosinophilia - 2/10/03 re: Cause of rash, hyper-IgE and eosinophilia 11 year old female from Mexico has had recurrent exfoliating erythroderma since the end of November. Associated with recurrent high fever, marked elevation in eosinophils and IgE near 3,000. IgG has been 2,130, IgA and M are normal. During exacerbations has had severe Coombs negative anemia, thrombocytopenia, elevated BUN and Creatinine, Has progressed to severe respiratory compromise requiring ventilation at one point. Skin Bx: no vasculitis. Blood smear normal. ANA negative, Strep negative, EBV negative, Initial Dx Exfoliative Erythroderma felt to have resulted from medication, but unknown if it was from NSAID, Penicillin or Macrolide. Had been on all three while in Mexico around time of onset.
Responded well to treatment with high dose steroid, now has recurrence of dermatitis as she is being weaned from the steroid, and her creatinine has started rising. Anyone have any good ideas?
The case you describe in an 11-year-old child does sound complex. You did not mention whether or not the prominent respiratory symptoms were accompanied by abnormalities seen on plain chest x-ray or high-resolution chest CT exam, or whether the anemia was severe enough to cause respiratory distress on its own. You also did not mention whether a bone marrow exam was carried out during the anemia/thrombopenia and the results if done.

Epidemiologic Notes and Reports Clinical Spectrum of Eosinophilia-Myalgia Syndrome California
As of January 12, 1990, 210 eosinophilia-myalgia syndrome (EMS) cases (1,2) had been reported to the California Department of Health Services (CDHS)*; three patients have died. This report summarizes clinical features and laboratory findings in 118 EMS patients for whom completed case-report forms are available. One hundred five (89%) of the 118 patients were female; 94% were non-Hispanic white, 2% were Hispanic, less than 1% were black, and less than 1% were Asian (for 3%, race was unknown). Patients ranged in age from 29 to 73 years (median: 48 years). One hundred three (87%) patients became ill during or after July 1989. All patients reported use of L-tryptophan (LT) supplements before onset of illness; the time from onset of LT use to onset of symptoms varied from less than 2 weeks to greater than 15 years (median: 152 days). Hospitalization was reported for 34 (29%) patients. The most commonly reported symptoms were myalgia (100%) and arthralgia (69%), followed by dyspnea or cough (64%) and rash (64%) (Table 1). Edema occurred in 51% of patients, and fever was reported by 47%. Scleroderma-like skin changes (e.g., skin thickening) and increased hair loss were each reported in 18% of patients. Neuropathy was diagnosed by physical examination or electrophysiologic testing in 16 (14%) cases. At least three patients have developed a progressive poly neuropathy; electromyographic testing detected axonal loss in all these patients.

As of December 1, 1990, 151 cases of eosinophilia-myalgia syndrome (EMS)* had been reported to the New York State Department of Health (NYSDOH); 10 of these patients died. Of the 151 case-patients, 149 were known to have used supplemental L-tryptophan (LT) before onset of illness. Because anecdotal reports indicated that some patients in New York had changes in mental status and other symptoms not previously described in association with EMS, the NYSDOH conducted a survey from December 1990 through March 1991 to determine the prevalence of self-reported symptoms in patients with EMS. A detailed questionnaire was sent by the NYSDOH to all living patients, asking them to report on the presence of 47 specific symptoms at any time during their illness and at the time of the survey. For each symptom, severity was reported as minimal, moderate, or extreme. Of the 139 living case-patients who had used LT before onset of EMS, 91 (65%) completed the questionnaire a median of 16 months after onset of illness (range: 1140 months). The median age of respondents was 51 years (range: 3284 years); 78% were women. Thirty-seven (41%) had been hospitalized at least once following onset of symptoms. Patients who completed and who did not complete the questionnaire were similar in age, sex, and peak eosinophil count.

History
The patient is a healthy 43 woman with a 20-year history of pruritic lesions on her ears and periauricular areas. The lesions started insidiously with no tendency for spontaneous resolution. Previously, she had been treated with excisions, liquid nitrogen cryotherapy, electrodesiccation, and intralesional glucocorticoids, all of which provided temporary improvement but no lasting benefits. Moreover, the use of 585-nm, pulsed-dye laser treatment to several lesions was associated with resolution with recurrence after one month. Most recently, she underwent 12 sessions of radiation therapy, which led to a remission that lasted approximately six months. Since her most recent recurrence, she has been using topical medium-potency glucocorticoids, doxepin cream, and topical anesthetics to achieve variable and brief symptomatic improvement.

Eosinophilia-myalgia syndrome (EMS) is characterized by an elevated eosinophil count ( eosinophilia ) combined with muscle pain fatigue , tenderness, cough, rashes and neurologic problems. EMS is caused by contaminated tryptophan. It works like an autoimmune disease similar to lupus. Treatment can be high doses of fish oil, antioxidants, and any anti-autoimmune disease agent whether it be Tagamet to chemotherapy, which of course we don't recommend, to drugs like Fastin, Ritalin, etc. Other treatments might include DHEA, thyroid, etc., and of course steroids which again we usually do not recommend because of the side effects.

Eosinophilia-myalgia syndrome is an incurable and sometimes fatal flu-like neurological condition that was caused by contaminated l-tryptophan supplements. In 1989 an outbreak of eosinophilia-myalgia syndrome was traced to an improperly prepared batch of tryptophan. The bacterial culture used to synthesise tryptophan had recently been genetically engineered to increase tryptophan production: unfortunately, with the higher tryptophan concentration in the culture medium, the purification process had also been modified to reduce costs, and a purification step that used charcoal adsorption to remove impurities had been omitted. This allowed another bacterial metabolite through the purification, and this contaminant of the end-product had been responsible for the toxic effects. Regardless of the origin of the toxicity, tryptophan was banned from sale in the US, and other countries followed suit. Tryptophan remains banned from sale to the public. External links:
