We are lucky however, because we have more kidney power than we need. Most people are born with two kidneys, but can function easily with just one. However, if you lose 85 to 90 percent of your kidney function, you enter what is called end stage renal disease (ESRD). At that point you need help. There are only two choices according to the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK), dialysis and transplantation. Causes of chronic kidney failure When the kidneys fail, it can be temporary (acute) or permanent (chronic). About End Stage Renal Disease Approximately 225,000 people in Europe, 300,000 people in the United States and one million worldwide suffer from end-stage renal disease (ESRD) and depend on haemodialysis to replace their kidney function. ESRD occurs when a significant portion of normal kidney function is lost. Healthy kidneys concentrate waste materials in the urine, maintain fluid (water) balance, manufacture vitamin D, stimulate red blood cell production, and maintain good bone and cardiac health by controlling serum phosphorus levels. Physicians use haemodialysis to clear waste materials and excess water from the blood of ESRD patients. Vitamin D supplements and injections of erythropoietin (a red blood cell production agent) can be given when needed. However, serum phosphorus is not easily controlled and is largely a function of diet. Therefore, several therapeutic approaches (haemodialysis, dietary restrictions, and phosphate binders) are usually necessary to adequately control serum phosphorus levels. Haemodialysis alone is not able to effectively clear total body phosphorus since it is mainly found in our cells. Dietary phosphorus restriction reduces vital protein intake and can lead to malnutrition. Phosphate-binding agents may be effective in decreasing intestinal phosphorus absorption but in the longer term some phosphate binders are associated with such problems as aluminium toxicity, hypercalcaemia, extraskeletal calcification with increased risk of cardiac and coronary artery disease.

The incidence rate for those 65 years of age and older exceeds 800 cases per million compared with 300 cases per million in those younger then age 65. Hemodialysis is the most common therapy for individuals with ESRD. In order to perform hemodialysis, a device that provides access to the arterial and venous circulation is required. There are different types of vascular access devices that provide for access to the circulation. Your vascular doctor can discuss these different options with you. For example, a primary or native arteriovenous (AV) fistula directly connects an artery and vein; an AV shunt or bridge fistula connects an artery and vein by using a synthetic tube. These procedures take about one hour and may be performed as an outpatient.Your vascular doctor can discuss all of your options with you.

The loss of kidney function in ESRD is usually irreversible and permanent. The ESRD Program of Medicare was established by the Social Security Amendments of 1972 (Public Law 92-603). Pete Stark introduced the End Stage Renal Disease Quality Improvement Act (ESRD). This legislation addresses deficiencies in the delivery of dialysis treatment for those suffering from End Stage Renal Disease, a chronic kidney disorder suffered by a growing number of Americans. Those suffering with End Stage Renal Disease have lost full kidney function and must undergo a transplant or weekly dialysis to survive. 340,000 Americans suffering from ESRD receive dialysis. For the past 30 years, Medicare has funded dialysis therapy for a majority of ESRD patients due to the high cost of treatment. There is alarming evidence that substandard care is being delivered at some Medicare funded sites.
    • In 2000, the Inspector General noted numerous instances of substandard care and a fragmented oversight system that lacks adequate accountability.
    • The GAO reported that only 1 in 9 dialysis facilities underwent an unannounced inspection in 1999.
    • In 1998, the GAO found that almost 1 in 2 dialysis facilities had not been inspected within three years.

    Concerns Regarding Inclusion of End Stage Renal Disease Patients in Managed Care Plans Individuals who develop End Stage Renal Disease (ESRD) are currently unable to enroll in a Medicare+Choice managed care plan. This exception occurred because members of Congress were concerned about the intensive health care needs of ESRD patients. Historically ANNA and other members of the renal community have supported the continuance of this exception. ANNA recognizes that Medicare has implemented strategies to shift some of the financial burden of ESRD care to private insurers such as the extension of the secondary payer status from 18 to 30 months and the HCFA ESRD Demonstration Project. Recent initiatives in Congress and by private corporations demonstrate that the trend toward shifting financial responsibility to the commercial sector may well continue. The lifting of the prohibition of ESRD patients entering managed care plans could occur.     I am a 25 year old woman who has had Kidney Disease for the last seven years. Last year I developed high blood pressure and everything has been going down hill since then. I am presently taking 120 mg of Prednisone every other day along with other drugs for blood pressure, calcuim and cholesterol. The doctors have told me that the Prednisone may not work at this point and that I may have to start dialysis soon. I am very nervous about going on Dialysis and I am doing my best to stay positive but is sometimes hard. I have being doing some research on Dialysis and have decided that PD might be best for me.     HPSD Home Public Health Disease Prevention ESRD End Stage Renal Disease Program PROGRAM MISSION To provide assistance to those individuals suffering from chronic renal disease who are at or below the 300% poverty level.
    END STAGE RENAL DISEASE That stage of renal impairment that appears irreversible and permanent, and requires a regular course of dialysis or kidney transplantation to maintain life.
    Main causes of ESRD are diabetes, high blood pressure, other, glomerulonephritis, polycystic kidney disease and interstitial nephritis. LEGISLATIVE INTENT: It is the intent of the legislature to insure the establishment of a program for the care and treatment of persons suffering from chronic renal diseases who require lifesaving care and treatment for such renal diseases but who are unable to pay for the services on a continuing basis. PRESENT PROGRAM The ESRD program is a $100,000 per biennium funding to the citizens of Montana suffering from failed kidneys. The enabling legislation and the present law require the program to assist individuals financially, who suffer chronic renal disease, who are at or below the 300% poverty level.
