End-Stage Renal Disease is a stage of renal impairment that appears irreversible and permanent, and requires a regular course of dialysis or a kidney transplant to maintain life.
Certification Services . There are 5 types of End-Stage Renal Disease facilities that are subject to certification:
  • Renal Transplantation Center - a hospital unit which provides transplantation and other medical/surgical specialty services associated with transplantation, including inpatient dialysis. A transplantation center may also be a renal dialysis center. Renal Dialysis Center - a hospital unit which provides the full spectrum of diagnostic, therapeutic, and rehabilitative services to care for dialysis patients, including inpatient and outpatient dialysis. Renal Dialysis Facility - a unit which provides dialysis services directly (i.e., not through contracted providers).

    Overview
    Children's Hospital Medical Center of Cincinnati has cared for children with end-stage renal disease for more than 30 years, and our nephrologists and surgeons are in the forefront of clinical research for children with end-stage renal disease. Areas of Emphasis
    • Managing children whose renal failure is accompanied by severe congenital anomalies and/or bladder dysfunction. Using a team approach to nutrition, growth promotion, and staged surgical repair, with attention to maintaining access to dialysis, the end-stage renal disease team has successfully rehabilitated a number of children with devastating anomalies. Staged surgical procedures, including bladder augmentation or replacement; creation of a Mitrofanoff neourethra; urinary and bowel incontinence repairs; and nephrectomy. Developing and monitoring clinical guidelines for childhood renal transplantation.

    Various rare forms of kidney disease which lead to end-stage renal disease (ESRD) are known to be caused by gene defects. However, there is increasing evidence from human family studies, as well as genetic linkage analysis in animals, demonstrating that there are also inherited factors influencing the risk for developing diabetic or hypertensive ESRD. To determine the epidemiology of various causes of familial ESRD, we screened dialysis patients for familial clustering of ESRD and its risk factors.

    What is renal failure?
    Renal failure refers to temporary or permanent damage to the kidneys that results in loss of normal kidney function. There are two different types of renal failure - acute and chronic. Acute renal failure has an abrupt onset and is potentially reversible. Chronic failure progresses slowly over at least three months and can lead to permanent renal failure. The causes, symptoms, treatments, and outcomes of acute and chronic are different. Conditions that may lead to acute or chronic renal failure may include, but are not limited to, the following:

    When someone has end stage renal disease, the kidneys are damaged.
    The kidneys no longer work well enough to remove wastes and excess fluids from the body. Complete kidney failure occurs when less than 10% of your kidneys are working. Toxins , waste products from foods and body cells, plus extra fluids build up in the blood. If the excess wastes and fluids are not removed, the whole body is affected. You will become ill from the build up of waste products and fluid. When the kidneys cannot work normally any longer, this is called kidney failure. When kidney failure progresses to the point where the kidneys do not function well enough to keep a person healthy, he or she will need treatment. A process called "dialysis" is used to replace some of the functions (waste and fluid removal) of the kidneys. Or, a patient may be a candidate for a kidney transplant, which is when a new kidney from a living or deceased donor replaces the patient's nonfunctioning kidney (patients usually receive dialysis while they are waiting for a donor kidney). Not all people with kidney failure are candidates for a transplant. Kidney failure can happen suddenly or slowly.

    CKD: An Insidious Disease Chronic kidney disease (CKD) is an insidious disease that gradually impairs kidney function. In its earliest stages, patients may be unaware they have the disease, but over a period ranging from several years to several decades, CKD will often progress to end-stage renal disease (ESRD), requiring renal replacement therapy (dialysis or kidney transplantation) to sustain life. CKD arises as a consequence of diabetes mellitus, hypertensive nephrosclerosis, chronic glomerulonephritis, polycystic kidney disease, and a host of other disorders.1 Diabetes and hypertension are the two leading causes of end-stage renal disease ESRD, accounting for approximately 43% and 27% of all new end-stage renal disease ESRD cases, respectively.

    Staging and Prevalence of CKD While the terminology of the formal literature has been inconsistent and confusing, this monograph has adopted the CKD staging terminology recently proposed by the National Kidney Foundation’s Kidney Disease Outcomes Quality Initiative (NKF-K/DOQI) and uses the term chronic kidney disease to encompass the entire spectrum of kidney disease, from its earliest stages through end-stage renal disease ESRD.

    The following patient profile tables provides national data on ESRD patients who are enrolled in the Medicare program as of December 31, 1997. Patient data on deaths and transplants are for those patients included as of December 31, 1997, but who died or were transplanted in calendar year 1998. The data from these profile tables will be used by HCFA to evaluate patient data of individual ESRD facilities that file ESRD exception requests. Providers may use this patient data to compare national data with their patient data. This data will assist providers to determine if the characteristics of their patient mix is different than national data. We will also use the following inpatient data for dialysis patients that was obtained from Table 46 of the Health Care Financing - Research Report - End Stage Renal Disease, 1993 - 95 - HCFA Pub.
