There are two kinds of kidney failure:
  • Acute Renal Failure
    Sometimes, kidney failure occurs suddenly after complicated surgery or severe injuries, or when blood vessels leading to the kidneys become clogged. Chronic Renal Failure
    Kidney failure can also occur slowly. Chronic renal failure can develop with few symptoms in the early stages. Many people with chronic renal failure don't have symptoms until their kidney function has decreased to less than 25 percent of normal. High blood pressure and diabetes are the most common causes.

Diabetes Surveillance, 1999 Chapter 1: The Public Health Burden of Diabetes Mellitus in the United States End-Stage Renal Disease Diabetes is the leading cause of end-stage renal disease (ESRD), i.e., kidney failure requiring dialysis or transplantation ( ). In the United States, diabetes accounts for approximately 40% of all new cases. Persons with diabetes are the fastest growing group receiving kidney dialysis or transplantation. This report shows that ESRD remains a significant complication among persons with diabetes and that blacks and the elderly are disproportionately affected. The age-adjusted incidence of ESRD-DM treatment increased by 291% in the general population and by 222% in the diabetic population (Tables 8.11 and 8.20). For each of these respective populations, the increases in the crude incidence of treatment (303% in the general population and 224% in the diabetic population) were similar to the increases in the age-adjusted incidence, suggesting that the increases in incidence were not due to the aging of the population. The increase in the diabetic population was 74% that of the general population, indicating that most of the increase in the general population is due to the increasing incidence of ESRD-DM in the diabetic population and that only 26% is due to the increasing prevalence of diabetes.

Diabetes is the leading cause of end-stage renal disease (ESRD) (i.e., kidney failure requiring dialysis or kidney transplantation), and ESRD treatment has increased among American Indians/Alaska Natives (AI/ANs) ( ). To assess trends in the incidence of ESRD attributable to diabetes mellitus (ESRD-DM) treatment among AI/ANs with diabetes, CDC and the Indian Health Service (IHS) analyzed data from the U.S. Renal Data System (USRDS). This report summarizes the findings of the analysis, which indicate that the incidence of ESRD-DM treatment is increasing among AI/ANs with diabetes, particularly young AI/ANs. ). USRDS collects, analyzes, and distributes information about the incidence, prevalence, treatment, and costs of ESRD ( ), including demographic (e.g., age, sex, and race) and ESRD-related information (e.g., first date of treatment and primary cause of renal failure), ESRD-DM* (i.e., listed in USRDS as the primary cause of renal failure), and first treatment (e.g., kidney dialysis, peritoneal dialysis, or kidney transplantation) in each year during 19901996. The incidence of ESRD-DM treatment was calculated using annual age-specific and sex-specific diabetes prevalence estimates from the IHS outpatient database ( ) and annual estimates of the AI/AN population from the U.S. Bureau of the Census.     Updated April 14, 2003 Mission Statement: To improve supportive care and quality of life for ESRD patients and their families. The End-Stage Renal Disease (ESRD) Workgroup worked to improve supportive care and quality of life for dialysis patients from Spring 2000 - Spring 2002. Chaired by Alvin Moss, MD (Director of the West Virginia University Center for Health Ethics and Law), this workgroup consisted of 23 members including nurses, social workers, nephrologists, and kidney transplant recipients. To accomplish its goals, the workgroup divided into three subgroups Education, Quality of Life, and Quality of Dying. The ESRD Workgroup made considerable progress in bringing palliative care issues to the attention of the nephrology community, including an invitation from the Chair of the American Board of Internal Medicine Subspecialty Board on Nephrology to submit questions on end-of-life care for the board certification examination. In addition, the Renal Physicians Association (RPA) and the American Society of Nephrology's (ASN) joint Quality Patient Care Committee meeting revised their position statement on Quality Care at the End-of-Life to incorporate workgroup findings in the Fall of 2001. This revised position statement has been widely circulated. The ESRD Workgroup completed its work with the support of The Robert Wood Johnson Promoting Excellence program in the winter of 2002, with finalization of a

The authors of this article remind us at the outset that "compared with white men, African-American men have a 4-fold higher age-adjusted risk of treated end stage renal disease (ESRD)." Furthermore, "ESRD attributed to hypertension (high blood pressure) at initiation of renal replacement therapy is over 5-fold greater in African Americans than in whites." In addition to hypertension as an explanation for the higher rates of ESRD in African-American men, low socio-economic status (SES) is also independently related to ESRD. This study sought to determine the reasons for the 4-fold increased incidence of treated ESRD in African-American men compared with white men. For that purpose, the authors conducted a prospective study of 332,544 men (300,645 white and 20,222 African American) over a 16 year period. The results of the study included the following:
    • African-American men had higher mean levels of systolic and diastolic blood pressure and a lower mean level of total serum cholesterol.

    End stage renal disease (ESRD) Each year more than 50,000 people in the United States are diagnosed with end-stage renal disease (ESRD). End-stage renal disease occurs when approximately 90% of normal kidney function has been lost. Symptoms include nausea, vomiting, weakness, fatigue and sluggishness, confusion and difficulty concentrating, decreased urine output, anemia, and loss of appetite. It can cause complications such as high blood pressure and congestive heart failure (a buildup of fluid in the heart).
    There are two accepted treatments for ESRD:
  • Dialysis , which substitutes for the kidneys in filtering body waste, and Transplantation , in which a failed kidney is replaced with a healthy donor kidney. For eligible patients, kidney transplantation offers freedom from dependence on dialyis machines, and breakthroughs in surgical techniques and immunosuppresion therapy afford this opportunity to more people than ever before. Kidney transplant recipients today can expect to lead normal, productive lives for decades.

  • Diseases of the Kidneys: The most common causes of kidney failure that necessitate kidney transplantation include: Diabetes , whether Type I (juvenile) or Type II (adult-onset), is the leading cause of kidney failure in the U.S. Diabetes may cause many other diseases and complications, and should be monitored carefully.
