    Introduction Methods Results ... More articles on Aboriginal health Abstract Objectives: To compare the incidence of end-stage renal disease (ESRD) among Aboriginals in New South Wales with the incidence among Aboriginals in the Northern Territory, and to compare the patterns of ESRD among Aboriginals and non-Aboriginals in NSW.
    Design: Secondary data analysis of information from unpublished and published Australia and New Zealand Dialysis and Transplant Registry reports.
    Main outcome measures: Average annual incidence of ESRD (persons per million); form of renal replacement therapy; mortality at 31 March 1998; patient and graft survival one and five years after transplant.
    Results: Each year in NSW, 5-17 new Aboriginal patients are treated for ESRD. There was no increase in the average annual incidence of ESRD among NSW Aboriginals (118 per million in 1988-1989 and 111 per million in 1996-1997), whereas incidence in the NT increased from 255 per million to 800 per million. In NSW, ESRD was attributed to diabetes in 32% of Aboriginal patients, compared with 13% of non-Aboriginal patients ( P
    Conclusion: There is a different pattern of incidence of ESRD and of outcomes with treatment among Aboriginals in NSW compared with those in the NT. A possible explanation is that the lower incidence in NSW reflects less profound socioeconomic disadvantage and better access to primary and specialist care.

    Overview
    • Deborah Carpenter from Westat reviewed the results of the literature search. Cara James from Harvard relayed the results of the interviews with administrators of dialysis facilities and ESRD network organizations. Roger Levine from the American Institutes for Research (AIR) presented the findings from the initial set of focus groups.
    Finally, Brady Augustine, senior advisor to the administrator at CMS, discussed the agency's interest in improving its oversight of dialysis facilities and using payment incentives to reward high quality. He noted that the agency feels a special responsibility to protect ESRD patients because of their vulnerability and the constraints they face. Through the use of this survey tool, CMS hopes to fill in gaps in its knowledge of patients' experiences and promote improvements in the quality of care they receive. Because the agency wants the survey results to be useful for quality improvement (QI) at the dialysis facility and the ESRD network organizational level, it is particularly interested in learning what would meet the needs of these stakeholders. The most common causes of ESRD are, in order, diabetes uncontrolled hypertension glomerulonephritis and polycystic kidney disease Hemodialysis requires that the patient have regular treatments with an artificial kidney attached to a dialysis machine . Most are performed in an outpatient dialysis facility, but patients with supportive partners may opt to perform the treatments themselves in their home. Treatments typically are done three times a week and last for four hours. Advantages of this modality are freedom from dialysis when not on the machine.     Renal failure refers to temporary or permanent damage to the kidneys that results in loss of normal kidney function. There are two different types of renal failure - acute and chronic. Acute renal failure has an abrupt onset and is potentially reversible. Chronic failure progresses slowly over at least three months and can lead to permanent renal failure. The causes, symptoms, treatments, and outcomes of acute and chronic are different. Conditions that may lead to acute or chronic renal failure may include, but are not limited to, the following: Acute Renal Failure Chronic Renal Failure Myocardial infarction - a heart attack may lead to temporary kidney failure. Diabetic nephropathy - diabetes can cause permanent changes, leading to kidney damage. Rhabdomyolysis - kidney damage that can occur from muscle breakdown. This condition can occur from severe dehydration, infection, or other causes. Hypertension - chronic high blood pressure (hypertension) can lead to permanent kidney damage.

    Reimbursement for Treatment of End-Stage Renal Disease
    by Maureen McKinley, MSW, LSCW When beginning dialysis, many people wonder how they are going to pay for this costly treatment. Every dialysis clinic has a social worker to help answer this and other questions related to adjusting to life with kidney disease. The social worker will have information on insurance plans and other sources of financial assistance. Medicare
    Medicare has two types of coverage: Part A and Part B. Part A covers 100 percent of most inpatient expenses and is free to those who have paid into the Social Security system. Part B covers 80 percent of outpatient expenses and is covered by a monthly premium. For those receiving Social Security Disability or Retirement benefits, the Part B premium is automatically deducted from the monthly benefit payment. Those not receiving Social Security payments are billed every three months for their Medicare Part B coverage. It is possible to enroll in just Part A of Medicare. This is not advisable for people with renal disease, however, as it is Part B which covers maintenance dialysis treatment, laboratory work, and many other services needed by people who have ESRD.. Supplemental insurance
    Part B of Medicare pays only 80 percent of outpatient treatment after satisfaction of an annual deductible, so additional insurance is necessary to cover the remaining 20 percent of dialysis. The cost of maintenance dialysis varies somewhat from state to state, but on the average, the monthly Medicare allowable charge is $2,000. Medicare covers 80 percent of this, or approximately $1,600. The $400 that is left over is usually covered by supplemental insurance. Most people have some type of private insurance coverage when they develop renal failure. After they become eligible for Medicare, they use their private insurance as supplemental coverage. Many insurance companies allow subscribers to convert to a Medicare supplement plan, which has less expensive premiums, once enrolling in Medicare. Supplemental insurance can also cover the annual Medicare Part B deductible.

    by Debra Wood, RN Renal failure (or kidney failure) occurs when the kidneys are not able to perform their normal functions. It usually occurs in middle-aged and older people. The kidneys are two bean-shaped organs on either side of the spine in the lower back. Their main functions are to remove waste from the body and to balance the water and mineral content of the blood by removing water. The waste and water combine to form urine. Anatomy of the Kidney www.nucleusinc.com End-stage renal disease (ESRD) refers to a permanent condition in which the kidneys are no longer able to filter waste from the blood. As the wastes build up, the tiny filters in the kidneys (nephrons) continue to lose their filtering ability. Although damage to the nephrons may occur suddenly after an injury or poisoning, many kidney diseases take years or even to decades to cause noticeable damage. The two most common causes of ESRD are:
    • Diabetes – the nephrons are damaged by high blood sugar levels that occur in poorly controlled diabetes High blood pressure – causes damage to the capillaries in the kidneys
    End-stage renal disease can lead to anemia, high blood pressure, bone disorders, heart failure, and mental ‘fuzziness.’

The regulations also require such facilities to participate in network activities and pursue network goals.     Kidney anatomy Definition: End-stage kidney disease is a complete or near complete failure of the kidneys to function to excrete wastes, concentrate urine, and regulate electrolytes . Also called End-stage renal disease (ESRD). Alternative Names: Renal failure - end stage; Kidney failure - end stage; ESRD Causes, incidence, and risk factors: End-stage kidney disease occurs when the kidneys are no longer able to function at a level that is necessary for day to day life. It usually occurs as chronic renal failure progresses to the point where kidney function is less than 10% of baseline. At this point, the kidney function is so low that without dialysis or kidney transplantation, complications are multiple and severe, and death will occur from accumulation of fluids and waste products in the body.
    In the United States, nearly 300,000 people are on long-term dialysis and more than 20,000 have a functioning transplanted kidney. The most common cause of ESRD in the US is diabetes . ESRD almost always follows chronic kidney failure , which may exist for 10 to 20 years or more before progression to ESRD.
