Venezuelan Equine Encephalomyelitis
Veterinary Services September 2002 Venezuelan equine encephalomyelitis (VEE) is a mosquito-borne viral disease of all equine species, such as horses, asses, and zebras. After infection, equines may suddenly die or show progressive central nervous system disorders. Humans also can contract this disease. Healthy adults who become infected by the virus may experience flulike symptoms, such as high fevers and headaches. People with weakened immune systems and the young and the elderly can become severely ill or die from this disease.
The highly pathogenic form of VEE has not occurred in the United States since 1971. However, in 1993 an outbreak of VEE in the State of Chiapas, Mexico, prompted the U.S. Department of Agriculture's (USDA) Animal and Plant Health Inspection Service (APHIS) to temporarily increase its surveillance activities and tighten its quarantine requirements for equine species entering the United States from Mexico.

INTRODUCTION The general acceptance of the concept of universal vaccination against hepatitis B, regardless of risk factors, makes it incumbent on physicians to be aware of the complications that have been reported to occur following the hepatitis B vaccination. These complications have included a wide variety of reactions, most of which fit into an autoimmune category (3-19, Table 1). It is in this frame of reference that the two cases of postvaccinal encephalomyelitis seen by the author are being reported. A theoretic framework that might explain the pathogenesis of this complication and the studies suggested by this framework are discussed. CASE REPORTS Case 1
The patient is a 43-year-old female nurse who was in good health until August 1988. She had received two injections of hepatitis B vaccine in June 1988. Four weeks after the first vaccination, her husband noted that she began to have difficulty in concentrating and to have frequent severe headaches. She was taking a postgraduate course in nursing and contrary to her scholastic efforts in the past, she had difficulty in understanding and placing in context the relatively simple concepts that were being presented. In spite of headaches and cognition difficulties, she continued to work as a nurse. In October 1988 she developed a rash in her axilla which was nondescript in nature and which subsided in a month. She remained chronically ill with headaches, arthralgia and cognition defects, but she continued to try to work.

The popular image of chronic fatigue syndrome (CFS) or myalgic encephalomyeltis (ME) over the past several decades has generally tended to view the condition as being that of a new illness - one that has arisen in the 20th century. This image has been reinforced by the media and some of the popular (and professional) literature on the condition, with headlines and titles such as " ME - The New Plague ." So is the illness that new? Despite this popular belief, it would seem that conditions similar to ME/CFS have been around for considerably longer than is generally assumed. The mixture of a broad array of neurological and muscular symptoms that can occur after an infective illness can be traced back into history. For instance, as far back as 1750, Sir Richard Manningham reported a syndrome referred to as ' febricula' (or 'little fever') that had a large number of symptoms but was hard to objectively diagnose.

ADEM is more frequently encountered in children than in adults. We had the opportunity to examine and follow-up a rare case of elderly patient with fulminant ADEM treated successfully with a regimen of intravenous immunglobulin (IVIG) followed by intravenous methylprednisolone (IVMP). Follow-up magnetic resonance imagines (MRI) showed striking resolution of abnormalities over 4 months. He had free of symptoms for 3 years. Distinctive clinical and radiological features of ADEM and multiple sclerosis (MS) were also discussed on the basis of our case and recent literature. Key-words: acute disseminated encephalomyelitis, intravenous immunglobulin, intravenous methylprednisolone, multiple sclerosis

This disease not only affects horses, but is also deadly for humans. The virus was isolated, characterized, and vaccines were produced in the 1930s. The two forms currently active in the United States are Eastern equine encephalomyelitis (EEE) and Western equine encephalomyelitis (WEE). The viruses responsible for causing these diseases are members of a family of viruses called the alphavirus. WEE is most active in the western United States and parts of the Midwest. WEE was responsible for the death of 180,000 horses between 1931 and 1938 in the San Joaquin Valley, Calif. There have been isolated cases in every state west of the Appalachians, parts of western Canada, and in Central and South America. EEE has an eastern United States distribution. There have been isolated cases in Michigan and Wisconsin as well as outbreaks in the Caribbean, Dominican Republic, Haiti, Panama, and Central and South America. The mosquito transmits the virus from small infected animals such as birds and rodents to horses. The warm, humid weather of the summer is good for mosquito breeding and this is when outbreaks are more common. Transmission of EEE and WEE is not horse to human, but bird or rodent to human via the mosquito.

Viral encephalomyelitis of horses is a group of closely related diseases of Equidae which are transmitted by arthropods (insects), and which cause moderate to high mortality after presenting clinical signs of dysfunction of the nervous system. They are caused by Alphavirus (family Togaviridae) , of which three strains are commonly recognized: Eastern Equine Encephalomyelitis (EEE), Western Equine Encephalomyelitis (WEE) and Venezuelan Equine Encephalomyelitis (VEE) , Human infection can occur, and the disease is of significant public health importance. The viruses are restricted to the Americas, with cases reported from the USA, Canada, Venezuela, Brazil and Argentina. Outbreaks have occurred in Central America. In North America the disease tends to be seasonal due to the changes in mosquito and other insect populations. Spread of the disease between areas by migratory birds may be possible, and encephalitis has been detected in infected birds. In general, mortality is lower with WEE infection (20-30 %) than with EEE or VEE (40-80 %). Wild birds act as a reservoir for the virus, and spread is by insects. Mosquitoes appear the main vector, but mites, ticks and lice may also be implicated. The movement of infected mosquioes between distant areas by high winds has been recorded. Rodents and other small mammals may serve as viral reservoirs for VEE.
