Chronic subcutaneous emphysema following air sac trauma is frequently encountered in avian species. For the companion avian practitioner, larger psittacine species represent patients that are suitable for corrective measures. Miniaturized techniques might be used in smaller patients.
THE STENT: DESCRIPTION AND TECHNIQUE
The stent used is machined from solid virgin Teflon by McAllister Technical Services at West 280 Prairie Avenue, Coeur d'Alene, ID 83814, tel: 1-208-772-9527. The unit is 12.7 or 16 mm in diameter with eight pre-drilled holes for suture placement in the subcutaneous flange. The central stem is 1.5 mm deep and 4.5 mm in diameter with a 1.5 mm orifice. The outer rim is 8 mm in diameter, allowing the skin to tuck under the edge and thereby preventing the healing dermis from closing over the implant. The stents are inert and non-reactive. One post-surgical problem has been occasional plugging of the stent with mucous or congealed tissue fluid which is easily removed with a needle. This has occurred a few times post-operatively in some cases, but this problem eventually clears. Post-operative antibiotics have not been used and infected surgical sites have not occurred. To prevent post-surgical plugging in larger psittacines, it might be useful to use a larger stent with a 3 mm orifice and a 16 mm diameter external flange.

EMPHYSEMA, A - CENTRILOBULAR (UPPER ZONES); AND B - PANACINAR (LOWER ZONES) - In centrilobular emphysema only the central or proximal portions of the respiratory lobule (respiratory bronchial) is involved. Emphysema - an Introduction
E mphysema is a devastating disease caused by destruction of lung tissue. The usual cause is cigarette smoking. As tissue is destroyed in the lung, large airspaces or holes appear in the lung. The elastic properties of the lung are decreased and the airways narrow. This results in overinflation of the lung (lungs too large), large decreases in the patient's ability to move air in and out of the lungs, and decreases in the ability of oxygen to pass into the blood from the lungs. Emphysema, when severe, is as devastating and debilitating as cancer, severe heart failure or any other severe chronic disease. The most troublesome symptom is severe shortness of breath of even the most trivial effort. Patients may develop a whole host of secondary symptoms such as anxiety and depression, severe pulmonary infections, heart failure, and respiratory failure. Mortality is considerably increased in emphysema patients.
Traditional Treatment T raditional medical treatment consists of smoking cessation (the first step), medications to open the airways of the lungs as much as possible (bronchodilators), medications to decrease inflammation in the airways (steroids), oxygen, and antibiotics for pulmonary infections. Inhalers for the administration of bronchodilators and steroids are frequently used.

Certain individuals are also genetically disposed towards emphysema; this form is called alpha 1-antitrypsin (AAT) emphysema. Extensive smoking can lead to irreparable lung damage, even if you quit smoking. In these cases, the condition is not likely to improve and death by respiratory or cardiac (heart) failure is the inevitable outcome (months to years).     LRRI RESEARCH TO FIND A CURE FOR EMPHYSEMA
    Scientists at LRRI have developed a mouse model of cigarette smoke-induced emphysema. Mice that inhale cigarette smoke and develop emphysema not only show the enlarged alveoli and destruction similar to human emphysema, but they also have deposits of tar and chronic inflammation, as do patients with emphysema. Therefore, we feel that this model is the most appropriate for testing potential therapeutic drugs. We're enthusiastic about our current research to devise a treatment based on the inhalation of retinoids. LRRI scientists already have the needed expertise in aerosol drug delivery. Because inhaled retinoids directly target lung tissues, lower doses of retinoids may be effective when administered in this manner. We hope that inhalation of retinoids will avoid many of the problems of systemic toxicity.

    DURHAM, N.C. A new procedure is helping people with advanced emphysema breathe easier. Patients usually carry around oxygen tanks and cannot walk more than a few feet at a time. A new procedure, called lung reduction surgery, could help patients get back on their feet again. One of those patients, Jo Ann Desilets, never thought she would enjoy walking down the halls of a Duke Medical Center. Her emphysema was so bad, she used to need a helping hand and an oxygen tank to get around. "I used it playing golf, which, of course, was a sight to see," she said. "I smoked and I knew better and I'm paying the price." Emphysema is a progressive disease. It causes inflammation in the air sacs of the lungs. Over time, the inflammation destroys airways, making it hard to breathe. Most treatments only help the symptoms.

    On July 13, 1994, an obituary in the Washington Post reported the death, at age 60, of Richard Joshua Reynolds, III, an heir to the founder of the R.J. Reynolds tobacco company. The headline and an accompanying photograph showed the deceased smoking a cigarette and implied that Reynolds died from emphysema, caused by smoking. Reading the obituary in detail, however, it turned out that he had quit smoking eight years prior to his death; and that there was a family history of emphysema, the deceased's own father having died from the disease at the age of 58. Furthermore, the obituary disclosed that the deceased's own doctor was unable to state the "immediate cause" of his death. Medical opinion concerning emphysema has had an interesting history. My 1973 edition of "Diagnosis and Treatment" (a standard medical textbook), states that emphysema is a disease which involves destruction of the alveolar (lung) tissue but that the cause is unknown, although "many doctors" think it is caused by "cigarette smoking". In 1973, Chronic Pulmonary Obstructive Disease (COPD) had not yet been invented. COPD, while now discussed at length in modern medical textbooks, did not exist in 1973. Some time subsequent to 1973, a genetic cause of emphysema was discovered. In an article in the latest on-line edition of Grolier's Encyclopedia, Howard Buechner, M.D., explains that a significant number of the people with the disease lack a gene that controls the liver's production of a protein called alpha-1 antitrypsin (AAT). This protein controls or degrades an enzyme called neutrophil elastase, produced by the white blood cells. When the enzyme is left unchecked, it destroys alveolar tissue.

    The term 'chronic obstructive pulmonary disease' (COPD) is often used to refer to chronic bronchitis and emphysema. 'Chronic' means that the problem won't go away but can usually be controlled. COPD is mostly caused by smoking. In COPD the airways have become narrow and damaged and it is harder to breathe. Chronic bronchitis means the air passages are inflamed, which causes cough and excess phlegm. If you stop smoking, the phlegm usually dries up! People with COPD tend to get worse with chest infections and during cold weather. Good treatment will help you get better quicker.

    Clinical Presentation
    Presents as tachypnea or other mild respiratory distress without cyanosis. CXR shows unilateral hyperinflated lobe, bronchovascular markings to periphery of involved lobe, possibly mediastinal shift and/or atelectasis or collapse of adjacent lobe(s).
    Differential Diagnosis
    • Intrinsic
      • Obstruction from mucous plug
      • Narrowed bronchial segment
      • Diaphragmatic hernia
      • Cystic adenomatoid malformation
      • Congenital lung cyst
      • Congenital lobar emphysema
    • Extrinsic
      • Vascular ring or sling
      • Malplaced/dilated pulmonary artery causing compression
      • Tumor or other mass lesion
      Key Clinical Points
      • Time of first presentation: typically within first 6 months of life
      • Lobe affected: LUL 50%, RML 24%, RUL 18%
      • Associated with congenital heart disease in 30% (TOF, VSD, TAPVR)
      • Males:females 1.8:1
      Pathophysiology
      Lung contains alveoli that are distended to 3-10 time the normal size. There is no destuction of lung, but normal tissue with distal overinflation caused by partial airway obstruction.
      Management
      For infants with acute/serious respiratory symptoms, segmentectomy or lobectomy is the treatment of choice. The incidence of complications is low. Asymptomatic patients may not require any definitive treatment. Most respiratory symptoms do not progress and resolve within a year of age.

    Chapter 13: Smoking and Emphysema
    On July 13, 1994, an obituary in the Washington Post reported the death, at age 60, of Richard Joshua Reynolds, III, an heir to the founder of the R.J. Reynolds tobacco company. The headline and an accompanying photograph showed the deceased smoking a cigarette and implied that Reynolds died from emphysema, caused by smoking. Reading the obituary in detail, however, it turned out that he had quit smoking eight years prior to his death; and that there was a family history of emphysema, the deceased's own father having died from the disease at the age of 58. Furthermore, the obituary disclosed that the deceased's own doctor was unable to state the "immediate cause" of his death. Medical opinion concerning emphysema has had an interesting history. My 1973 edition of "Diagnosis and Treatment" (a standard medical textbook), states that emphysema is a disease which involves destruction of the alveolar (lung) tissue but that the cause is unknown, although "many doctors" think it is caused by "cigarette smoking". In 1973, Chromic Pulmonary Obstructive Disease (COPD) had not yet been invented. COPD, while now discussed at length in modern medical textbooks, did not exist in 1973. Some time subsequent to 1973, a genetic cause of emphysema was discovered. In an article in the latest on-line edition of Grolier's Encyclopedia, Howard Buechner, M.D., explains that a significant number of the people with the disease lack a gene that controls the liver's production of a protein called alpha-1 antitrypsin (AAT). This protein controls or degrades an enzyme called neutrophilelastase, produced by the white blood cells. When the enzyme is left unchecked, it destroys alveolar tissue.
