What is emphysema?
Emphysema is a chronic (long-lasting) disease that gradually destroys the lungs. This destruction means that you become unable to breathe in enough oxygen. You also have trouble breathing out carbon dioxide. How does it occur? Emphysema is almost always caused by cigarette smoking. However, rare genetic diseases can cause similar changes in the lungs and, when combined with cigarette smoking, more rapid destruction of the lungs. Cigarette smoke can damage the cells in your lungs. This damage changes the structure of the tiny air sacs in your lungs called alveoli. The surface area of the air sacs for the exchange of oxygen and carbon dioxide becomes smaller, and the air sacs become less elastic. These changes affect your ability to breathe out carbon dioxide after breathing in. As the carbon dioxide accumulates in your lungs, there is less and less room available for oxygen to be breathed in. Once this damage has occurred, it is not reversible. Researchers are looking for ways to predict who is more likely to develop emphysema if they smoke. At this time, however, they have found no tests for this risk.

Tobacco use and smoking are very dangerous addictions which commonly cause a wide variety of diseases, cancer and death. The vast majority of tobacco users and smokers are hooked when they are children. During this time period they are easily influenced by peer pressure and advertising. Once hooked, the majority of tobacco users become hopelessly addicted.
More than 5 million children living today will die prematurely because of a decision they will make as adolescents-the decision to use tobacco and smoke cigarettes. View our new FLASH Enhanced Tobacco Website !!!

Studies indicate that nonsmokers are also adversely affected by environmental tobacco smoke. There are 4000 chemicals in tobacco with 100 identified poisons and 63 known drugs which cause cancer. The most talked about drug in tobacco is nicotine. Nicotine is vasoconstrictor and a nerve toxin, it has been classified as a class I insecticide. Other poisons include: Arsenic, Cyanide, Formaldehyde (used to embalm bodies), Ammonia Bromide (a toilet cleaner )- Ref. 1.

Question: I am a 43 year old female who has been treated for hiatil hernia/gerd since 1991 nothing completely helped. I would really like to know your opinion of the meds the dr. has me on. At night it's worse of all i can't sleep because I have alot of trouble breathing. I have tried everything they told me to and I feel like I've swallowed a golf ball all the time. Answer: You didn't name the drugs. Usual drugs are drugs to decrease acid(Zantac, etc at first and then Prilosec or Prevacid) and then drugs to decrease reflux. This is usually propulsid since reglan(the other) can have long term neurologic side effects. Lastly is surgical repairs. Usually done through the laparoscope by putting a ring type structure around the esophagus and keeping the stomach in the abdomen. Emphysema [posted 8/6/98]
Question: I have suddenly developed a form of emphysema, with difficulty breathing, sometimes with very little exertion. At other times I can perform strenuous exercise with no apparent adverse effects. I was taking doxycycline about eight months ago and was trying to be careful in avoiding the sun. One day I went for a walk and forgot a hat on a sunny day and was stricken with exhaustion, high fever (105 degrees) and had to be taken to the emergency room. Is doxycycline a possible culprit for my emphysema?

How Is Emphysema Treated? There is no cure for emphysema. The goal of treatment is to slow the development of disabling symptoms. The most important step to take is to stop smoking. Treatments for emphysema caused by smoking include medication, breathing retraining, and surgery. People with inherited emphysema due to alpha-1 antitrypsin deficiency can receive alpha 1-proteinase inhibitor (A1PI), which slows lung tissue destruction. This medication is administered weekly through an intravenous (IV) infusion. In an IV, the medication will drip from a plastic bag into a narrow tube and through a needle inserted in a vein in your arm so that it can reach your bloodstream. It usually takes about 30 minutes for the medication to be administered. An IV may be uncomfortable when the needle is inserted, but the administration of the medication will not be painful. Side effects of this treatment are rare and, if they do occur, are mild. They include fever, light-headedness, dizziness, and flu-like symptoms. Need To Know: The long-term effects of this therapy are not known. A1PI is not recommended for those who develop emphysema as a result of cigarette smoking.

Surgical Treatment
for Emphysema
(Please Note: Lung Volume Reduction Surgery is not currently available at Cheshire Medical Center or Dartmouth-Hitchcock Medical Center. If interested, you should discuss with your physician whether this procedure is appropriate for you and where the nearest center is located.) Lung Reduction Surgery is a procedure designed to improve respiratory function in a select group of patients with severe bullous emphysema. In patient with emphysema normal lung tissue is often replaced by large "bullae". Bullae are thin walled balloon-like structures that can expand to compress relatively normal lung causing further reduction in lung capacity. Bullae more often occur in the upper portions of the lung and when they are localized or are very large their surgical removal (bullectomy) may allow better function in the remaining lung. For other emphysema patients bullae may be distributed more homogeneously throughout the lung. In these patients the lungs may also become over inflated for the size of the chest cavity leaving inadequate space for expansion. By surgically reducing the size of the lungs some patients have noted significant improvement in symptoms due to this over expansion and loss of elasticity.

Most of us started smoking back in the 40's and 50's. The money spent, even back then, by the tobacco companies, were in the billions of dollars, just in advertisements and cigarette handouts. Even now, their advertising dollars are directed to the younger generation. I have a disease know as emphysema. I would like to give all teenagers a brief description of my life as a person with emphysema. The facts that I provide to you will be a true-life experience of my trials and tribulation of my disease. Young people sometimes do not realize that smoking can destroy their future. They do not understand the problems that can develop from smoking. They do not understand the affect it can have on their health. When I was a young person, I never realized that smoking could diminish the most important factor in my life my breathing. When you can't breath you begin to realize how insignificant a cigarette can be in meeting your daily habit. I wish now I had never had a habit of smoking. I realize now breathing is a life function you can not live without, but cigarettes you can. If you had endured the pain I have suffered throughout the last ten years, you would never smoke a cigarette. I have had several collapsed lungs. I have had my back cut open, and had my lung partially removed. I have also spent months at a time in a hospital away from my family. The suffering is still not over; I now have to have my other lung partially removed. This procedure will prolong my life, for a later lung transplant.

Emphysema
Definition: Emphysema is a lung disease that involves damage to the air sacs (alveoli). The air sacs are unable to completely deflate (hyperinflation) and are therefore unable to fill with fresh air to ensure adequate oxygen supply to the body.
Causes, incidence, and risk factors: Cigarette smoking is the most common cause of emphysema. Tobacco smoke and other pollutants are thought to cause the release of chemicals from within the lungs that damage the walls of the air sacs. This damage becomes worse over time, affecting the exchange of oxygen and carbon dioxide in the lungs. A naturally occurring substance in the lungs called alpha-1 antitrypsin may protect against this damage. People with alpha-1 antitrypsin deficiency are at an increased risk for this disease.
