You are in: Lung and Airway Emphysema Overview Emphysema is a long-term, progressive disease of the lung that primarily causes shortness of breath. In people with emphysema, the lung tissues necessary to support the physical shape and function of the lung are destroyed. It is included in a group of diseases called chronic obstructive pulmonary disease or COPD (pulmonary refers to the lungs). Emphysema is called an obstructive lung disease because the destruction of lung tissue around smaller airways, called bronchioles, makes these airways unable to hold their shape properly when you exhale. This group of diseases ranks as the fourth leading cause of death in the United States. Unlike heart disease and other more common causes of death, the death rate for COPD appears to be rising.

In response to the recent developments in the fields of volume reduction surgery and lung transplantation, a Multidisciplinary Clinic has been established for the evaluation and care of patients with surgically treatable emphysema and chronic obstructive pulmonary disease. In addition, an Emphysema Cooperative Study Group has been established consisting of community and academic pulmonologists and thoracic surgeons throughout New England. Activities are centered around a prospective randomized trial seeking to compare the efficacy of pulmonary rehabilitation alone in emphysema patients eligible for volume reduction surgery versus rehabilitation of these patients followed by volume reduction surgery. People with emphysema develop bacterial lung infections more often than do people without the disease, and these infections can be life threatening. Such simple measures as drinking sufficient fluids and occasional use of a mist inhaler can help prevent lung infections. The fluid in the drink or inhaler helps keep lung secretions from becoming mucous; mucus in the lung passages provides an environment favorable to bacterial infection.
Antibiotic Treatment
Doctors prescribe a number of antibiotics to fight bacterial infections. If a person with emphysema gets infections frequently, his or her doctor may recommend antibiotics as a preventive measure even if the patient does not have an infection.

Peer Review Status: Externally Peer Reviewed by Mosby The assessment of cigarette smokers with dyspnea and airway obstruction is difficult due to the number of primary lung diseases (emphysema, bronchitis, asthma, or bronchiectasis) that can present with a similar constellation of symptoms and objective data. [40] In addition, cardiac failure and pulmonary embolus can mimic primary airway disease. The American Thoracic Society [43] has defined emphysema as a condition of the lung characterized by abnormal, permanent enlargement of airspaces distal to the terminal bronchiole, accompanied by the destruction of their walls, and without obvious fibrosis. The association between exposure to cigarette smoke and emphysema is well documented. [44] However, there also are data suggesting a relationship between fibrosis and smoking. [44, 45] This has raised important questions about the pathogenesis and definition of emphysema. Using a scanning electron microscope, Nagai and Thurlbeck [46] have demonstrated the presence of alveolar wall fibrosis in areas of centrilobular emphysema. Emphysema is classified into three subtypes based on the portion of the acinus that is involved.

Thirty month old male with progressive respiratory distress. Clinical History:
This is a two and a half year old male admitted to the hospital with a two day history of cough, rhinorrhea, and increasing respiratory distress. There was no history of foreign body aspiration. He was found to be positive for RSV. A chest film taken at that time showed over inflation of the right upper lobe. The patient showed initial improvement, but then decompensated one day later, suddenly developing severe retractions and hypercarbia. Clinical Physical Exam:

Although there tends to be some scarring with time because of superimposed infections, the emphysematous process is one of loss of lung parenchyma, not fibrosis. There are two major types of emphysema: centrilobular (centriacinar) and panlobular (panacinar). The former involves primarily the upper lobes while the latter involves all lung fields, particularly the bases. Centrilobular emphysema occurs with loss of the respiratory bronchioles in the proximal portion of the acinus, with sparing of distal alveoli. This pattern is most typical for smokers.Panacinar emphysema occurs with loss of all portions of the acinus from the respiratory bronchiole to the alveoli. This pattern is typical for alpha-1-antitrypsin deficiency.

    What Is Emphysema?
    Emphysema is a condition in which there is over-inflation of structures in the lungs known as alveoli or air sacs. This over-inflation results from a breakdown of the walls of the alveoli, which causes a decrease in respiratory function (the way the lungs work) and often, breathlessness. Early symptoms of emphysema include shortness of breath and cough.
    How Serious Is Emphysema?
    Emphysema is a widespread disease of the lungs. In 1993, 1.9 million people in the U.S. had emphysema. It is estimated that 50,000 to 100,000 Americans living today were born with a deficiency of a protein known as alpha 1-antitrypsin (AAT) which can lead to an inherited form of emphysema. Emphysema ranks 15th among chronic conditions that contribute to activity limitations: over 43 percent of individuals with emphysema report that their daily activities have been limited by the disease. Many of the people with emphysema are older men, but the condition is increasing among women. Males with emphysema outnumber females by 22 percent.
