Persistent Fever in a 7 Year Old A seven year old girl was hospitalized at a Tertiary Medical Center because of a seven day history of fever in the 40 C in June. It was associated with fatigue and poor appetite. She had also mild acheness all over and swelling of the hands. One week before hospitalization, she was seen by their family physician and was diagnosed to have a viral illness. Two days before she was hospitalized, she had some rashes which were not itchy. A strep test of the throat was positive and amoxicillin was prescribed. On the day she was admitted in the hospital, she was having fever, rashes at the face, and redness of the conjunctiva (the white part of the eye). She said she did not have headache, sore throat, or upper respiratory tract infection. Her immunizations were up to date and she did not have any contact with anyone with an illness. They live in wooded a community and they have a cat. She was ill appearing and tired on physical examination with a temperature of 39.5 C, pulse of 125/minute, breathing rate of 42/minute. Her blood pressure was normal. Both of her eyes were red. Both cheeks were red too. Her throat was red. She no limited motion of the neck which has enlarged lymph nodes. Her lungs were clear and heart did no show any murmur. The abdomen was normal. Lower extremities were swollen. Skin showed raised rashes over her neck and trunk.

Ehrlichiosis is a bacterial infection that is spread by ticks. Symptoms include fever , chills, headache , muscle aches, and tiredness. Description Ehrlichiosis is a tick-borne disease caused by infection with Ehrlichia bacteria. Ticks are small, blood-sucking arachnids. Although some ticks carry disease-causing organisms, most do not. When an animal or person is bitten by a tick that carries bacteria, the bacteria are passed to that person or animal during the tick's feeding process. It is believed that the tick must remain attached to the person or animal for at least 24 hours to spread the infection. There are two forms of ehrlichiosis in the United States; human monocytic ehrlichiosis and human granulocytic ehrlichiosis. Monocytic ehrlichiosis is caused by Ehrlichia chaffeensis , which is spread by the Lone Star tick, Amblyomma americanum . As of early 1998, about 400 cases of monocytic ehrlichiosis had been reported in 30 states, primarily in the southeastern and south central United States. The bacteria that causes granulocytic ehrlichiosis is not known, but suspected to be either Ehrlichia equi or Ehrlichia phagocytophila . Granulocytic ehrlichiosis is probably spread by the blacklegged tick Ixodes scapularis (which also spreads Lyme disease ). About 100 cases of granulocytic ehrlichiosis have been reported in Connecticut, Massachusetts, Rhode Island, Minnesota, New York, and Wisconsin.

What is Ehrlichiosis?
    E hrlichiosis is a tick-associated disease caused by bacteria in the genus Ehrlichia . To date, two types of ehrlichiosis have been identified in humans in the United States; HME, or Human Monocytic Ehrlichiosis, caused by Ehrlichia chaffeensis , and HGE, or Human Granulocytic Ehrlichiosis, caused by a yet unnamed species. It is possible that additional forms of ehrlichiosis in humans may be identified in the future. Unlike Lyme disease, ehrlichiosis is considered an acute infection without chronic long-term consequences. Its severity varies from person to person. Many people exposed to the disease agent remain asymptomatic, while others suffer mild symptoms that resolve without treatment. In a minority of cases, however, ehrlichiosis produces severe symptoms requiring immediate antibiotic treatment. These cases can be life-threatening and even fatal for elderly patients and others with compromised immune systems. Like Lyme disease, transmission of the ehrlichiosis bacterium is delayed an average of 36 hours or more after an infected tick begins to feed. Although Lyme disease and HGE infection rates in deer ticks are thought to be comparable in endemic areas of the northeast, evidence indicates that the transmission of

What is ehrlichiosis?
Ehrlichiosis is an acute disease of humans and animals caused by bacteria named Ehrlichia . These organisms, which are transmitted by ticks, can infect two different types of white blood cells. Where does ehrlichiosis occur? One type of ehrlichiosis, caused by the bacterium Ehrlichia chaffeensis , occurs primarily in the south central and southeastern United States. This particular organism, which infects the white blood cells called monocytes, has been known to exist for several years and has caused hundreds of cases of human illness. A new type of Ehrlichia is now known to occur in Wisconsin, Minnesota, and certain northeastern states. This recently discovered Ehrlichia has not yet been named, but because it infects the white blood cells known as granulocytes, the illness it causes is referred to as human granulocytic ehrlichiosis. The etiologic agent of canine monocytic ehrlichiosis (CME), the rickettsia Ehrlichia canis E. canis ), is a small pleomorphic gram-negative coccoid bacteria that parasitizes circulating monocytes intracytoplasmically in clusters of organisms called morulae. The disease is also known as canine rickettsiosis, canine hemorrhagic fever, tracker dog disease, canine tick typhus, Nairobi bleeding disorder and tropical canine pancytopenia, names representing different aspects of the same disease. What is ehrlichiosis?
Ehrlichiosis (say "err-lick-ee-o-sis") is an infection you can get from a tick bite. The ticks that spread ehrlichiosis are the deer tick, the dog tick and the Lone Star tick. It has been known for many years that dogs, cattle and other animals can get ehrlichiosis. Now it's known that ehrlichiosis can occur in humans as well. What are the symptoms of ehrlichiosis?
The symptoms of ehrlichiosis are like the symptoms of the flu. Symptoms usually start 5 to10 days after you've been bitten by a tick, but they may take up to 1 month to appear. You might have the following symptoms:
  • Fever
  • Chills
  • Malaise (a general feeling of not being well, or feeling tired)

Infected tick bites of the Lone Star tick, the blacklegged or deer tick, and the American dog tick cause disease in humans. Not every tick bite causes disease.
Anyone can get ehrlichiosis People may be exposed when they spend time outdoors in tick-infested areas (such as wooded, brushy, or grassy places). Pets may also bring ticks into the house as another possible means of exposure.
Symptoms appear 5 to 10 days after a tick bite People with ehrlichiosis usually have flu-like symptoms such as fever, headache, tiredness, and muscle aches. Other symptoms may include nausea, vomiting, lack of appetite, diarrhea, cough, joint pains, confusion, and sometimes a rash. It is likely that many people who become infected with the bacteria do not become sick or only get very mild symptoms. However, some people may have severe disease that can result in death.

This finding provides more insight into ehrlichiosis, first identified in humans in the United States in 1986. A bacterium called Ehrlichia ewingii causes ehrlichiosis in dogs, cattle and other animals. But in the past few years, four people exposed to ticks in Missouri have contracted the disease from this bacterium. "We want people to know that ehrlichiosis is in Missouri, Arkansas and southern Illinois," said Gregory Storch, M.D., professor of pediatrics at Washington University School of Medicine in St. Louis. "If someone is bitten by a tick and feels sick afterward, he or she should contact a physician as soon as possible and make sure the physician knows about the exposure." The researchers report their findings in today's issue of The New England Journal of Medicine. The lead authors are Storch and Richard Buller, Ph.D., research instructor of pediatrics.

    Ehrlichiosis Introduction The Ehrlichia are obligate intracellular parasites of leukocytes in various species of animals and humans. Each species of Ehrlichia characteristically parasitizes either neutrophils OR monocytes . They reside within phagosomes, and in virtually all cases are transmitted by ticks. The taxonomy of the Ehrlichia organisms has recently undergone substantial revision. Thus, some Ehrlichia organisms are now within the genera Anaplasma and Neorickettsia As with Lyme disease and Rocky Mountain spotted fever, animals do not serve as direct sources for zoonotic transmission of the organism, but simply increase the chances for human contact with infected ticks.
