What is ehrlichiosis? Ehrlichiosis is a tick-borne disease which can be caused by either of two different Ehrlichia bacteria. Human Monocytic Ehrlichiosis (HME) is caused by Ehrlichia chaffeensis, which is transmitted by the lone-star tick (Amblyomma americanum). Human Granulocytic Ehrlichiosis (HGE) is caused by an Ehrlichia equi-like bacteria that is transmitted by the deer tick (Ixodes scapularis). In New York State, most cases of ehrlichiosis have been reported on Long Island and in the lower Hudson Valley. Who gets ehrlichiosis? Anyone can get ehrlichiosis, although the majority of known cases have been in adults. People who spend time outdoors in tick-infested areas from April until October are at greatest risk for exposure. How is ehrlichiosis transmitted? Ehrlichiosis is transmitted by the bite of infected ticks, including the deer tick and the lone star tick. Ehrlichiosis cannot be spread from person-to-person. What are the symptoms of ehrlichiosis? The symptoms of HME and HGE are the same and usually include fever, muscle aches, weakness and headache. Patients may also experience confusion, nausea, vomiting and joint pain. Unlike Lyme disease or Rocky Mountain spotted fever, a rash is not common. Infection usually produces mild to moderately severe illness, with high fever and headache, but may occasionally be life-threatening or even fatal.

EHRLICHIOSIS What is ehrlichiosis? Ehrlichiosis is a disease of humans and animals caused by bacteria named Ehrlichia . The bacteria, which are transmitted by ticks, can infect certain types of white blood cells. Cases of ehrlichiosis have been confirmed in many states in this country. In Illinois, less than 10 cases of ehrlichiosis are reportedly yearly. How do you get ehrlichiosis? The bacteria are transmitted to humans by the bite of an infected tick. The lone star tick, the American dog tick (or wood tick) and the deer tick (or black-legged tick) have been associated with ehrlichiosis. Who gets ehrlichiosis? Anyone is susceptible to the disease, but persons who spend time outdoors in tick-infested environments are at increased risk of exposure. In the Midwest, risk of exposure is greatest from spring through late autumn, but some ticks can become active any time of year if the temperature is warm enough (about 40 degrees Fahrenheit or more at ground level). What are the symptoms of ehrlichiosis? Illness due to ehrlichiosis can be so mild that no medical care is sought or the illness can be severe and sometimes fatal. Symptoms are generally non-specific and other diagnoses may be considered. The more common complaints are fever, headache and muscle aches. Persons with ehrlichiosis also may experience loss of appetite, nausea and vomiting. A rash can occur but is usually not present.

Ehrlichia are intracellular bacteria also transmitted by the Ixodes tick resulting in a disease called Ehrlichiosis. Several different Ehrlichia bacteria exist but the one associated with the deer tick is called Human Granulocytic Ehrlichiosis, HGE, or HE, named after the cell which it infects. After incubating about 1-3 weeks, HE can most commonly cause fever, headache and muscle aches. Fatigue and gastrointestinal complaints may include nausea, vomiting, and diarrhea. The illness may vary from mild to severe and life-threatening. Effective antibiotics eradicate the infection and fewer problems develop with earlier treatment. Other pre-existing conditions worsen the course of this infection.

September 1998 Ehrlichiosis Introduction There has been increasing awareness of disease states caused by Ehrlichia species, both in veterinary and human medicine. This newsletter reviews canine and feline ehrlichiosis. Ehrlichia species are gram negative pleomorphic bacteria of the family Rickettseaceae. Many genera of ticks are vectors for Ehrlichia species, which are transmitted within salivary secretions of infected ticks. Organisms are phagocytized by circulating leukocytes, or enter platelets by endocytosis. In the acute stages of disease, organisms may be visualized within membrane-lined intracytoplasmic vacuoles, called morulae. Canine ehrlichiosis is classified by the cellular trophism of the infecting organisms. Monocytic morulae represent infection with E. canis, and produce the disease called canine monocytic ehrlichiosis. Neutrophilic morulae are seen with E. equi and E. ewingii infections, and the resulting disease is called canine granulocytic ehrlichiosis. Morulae found within platelets represent infection with E. platys

The American dog, brown dog, lone star, black-legged, and probably the Western black-legged ticks transmit the pathogens. There are two types of the human ehrlichiosis; human granulocytic ehrlichiosis (HGE) and human monocytic ehrlichiosis (HME). Signs/Symptoms:
Fever, malaise, headache, chills, severe muscle aches or pain, vomiting, anemia, lung infection, decrease in white blood cells, elevated liver enzymes. A rash occurs in a small percentage of people. Neurologic manifestations include seizures, meningitis, confusion, ataxia, cranial nerve palsy, and change in mental status. Delayed treatment can result in death.

What is canine ehrlichiosis?
Ehrlichiosis is an infectious disease of dogs. It first gained attention as a significant disease when military dogs returning from Vietnam during the 1970's were found to be infected. The disease seems to be particularly severe in German shepherds and Doberman pinchers.
The organism responsible for this disease is a rickettsial organism; rickettsiae are similar to bacteria. Ehrlichia canis is the most common species involved in ehrlichiosis, but occasionally, other strains of the organism will be found, i.e., Ehrlichia platys. Because of its origin in military dogs in Vietnam, it has also been called "tracker dog disease" and "tropical canine pancytopenia.
How does a dog get infected with ehrlichia?
Ehrlichiosis is transmitted to dogs through the bite of infected ticks; the brown dog tick, Rhipicephalus sanguineus, is the main reservoir of the organism in nature.
What are the signs of ehrlichiosis?
Signs of ehrlichiosis can be divided into three stages: acute (early disease), subclinical (no outward signs of disease), and chronic (long-standing infection).
In areas where ehrlichiosis is common, many dogs are seen during the acute phase. Infected dogs may have fever, swollen lymph nodes, respiratory distress, weight loss, bleeding disorders, and, occasionally, neurologic disturbances. This stage may last 2-4 weeks.

Ehrlichiosis is a tick-borne illness transmitted to humans. It is primarily found in the southwestern areas of the United States and somewhat in the upper Midwest. It has been seen in Oklahoma, Missouri, Virginia, Georgia and Illinois. The Lonestar tick is the most frequent culprit. You can see an example of the Lonestar tick in our tick section on Survive Outdoors. Ehrlichiosis does resemble Rocky Mountain spotted fever. The incubation period is anywhere from 7-21 days. An incubation period is defined as the time between exposure (being bitten by tick) and appearance of your first symptoms. Signs and Symptoms
Symptoms associated with Ehrlichiosis may be broad ranging, from a very mild flu like presentation to a life-threatening situation. Symptoms may also be similar to those associated with Lyme disease. In ehrlichiosis, approximately 35% of patients do develop a red dot type rash. It is unlike a Lyme disease rash in appearance. The rash occurs more commonly in children and young adults and targets the palms and soles of the feet in less than 10% of the cases, according to Auerbach. Treatment should begin as soon as Ehrlichiosis is suspected, even before a diagnosis is made.

Feline Ehrlichiosis and Hemobartonellosis Michael Lappin United States FELINE EHRLICHIOSIS Cats experimentally infected with E. risticii develop morulae in mononuclear cells and occasionally develop clinical signs of disease including fever, depression, lymphadenopathy, anorexia, and diarrhea. Cats experimentally infected with E. equi develop morulae in neutrophils and eosinophils, not mononuclear cells. Ehrlichia canis DNA and DNA genetically identical to the granulocytic Ehrlichia of horses, dogs, and humans in Sweden have been amplified from naturally exposed cats. Ehrlichia -like morula have been detected in mononuclear cells or neutrophils of naturally-exposed cats in the United States, Kenya, Brazil, France, Sweden, and Thailand. Clinical diagnosis has been also been based on the combination of either positive E. canis or E. risticii serology, clinical or laboratory findings consistent with ehrlichial infection, exclusion of other causes, and response to an anti-rickettsial drug. There are currently over 50 cases of suspected or proven cases of feline ehrlichiosis in the world’s literature. It is unknown how the clinically ill, naturally exposed cats described in the literature were infected. Exposure to arthropods has been reported in about 30% of the cases. Pathogenesis of disease associated with ehrlichiosis in cats is unknown. Of the cats in which age was reported, most were older than two years. Most cats were domestic shorthaired and both males and females have been affected. Anorexia, fever, inappetence, lethargy, weight loss, hyperesthesia or joint pain, pale mucous membranes, splenomegaly, dyspnea, and lymphadenomegaly were the most common historical and physical examination abnormalities. The first I heard of this disease was when I took a Babe into my vet for a health check just a couple days after I got her. I had been waiting for her for a long time and she had been ill prior to my getting her. Even though I was told she had been tested heart worm negative I had my vet recheck her just for my piece of mind. She had come from an area in the country with very little heart worms and had not even been tested for it until I asked about it. This time my paranoia paid off because in addition to testing for heart worms my vet also tested for C.E. And she was absolutely, no doubt about it, positive! We had spent quite a while talking and working with Babe before this test was done. There had been so many things which had not been thought out well before I got her and she had been treated incorrectly for months with a variety of drugs. Treating the symptoms of the problem instead of considering that they were symptoms of a bigger problem. I understand that this is the single most common problem with healing with C.E. There is a tendency to not put the problems together in one package, all caused by one disease.
