Vaccine protects monkeys from Ebola infection
From Christy Feig CNN Medical Correspondent (CNN) For the first time researchers have found a potential vaccine for the Ebola virus. The vaccine was tested only in monkeys, but it could be a significant development because doctors currently have nothing to treat or prevent infections. The current Ebola outbreak in Uganda has killed 145 people. Since it first appeared in 1976, the virus has killed more than 800, infecting nearly 1,100. The disease causes its victims to bleed to death within days of infection. ALSO Meet one of the CDC experts who went to Uganda to help control the Ebola outbreak there The vaccine was created by Dr. Gary Nabel and colleagues at the Vaccine Research Center at the National Institutes of Health. Research appears in the journal Nature. "We found that it's possible to protect against the lethal effects of the Ebola virus in a primate model. By vaccinating animals ahead of time against Ebola virus we were able to prevent them from dying from the infection when they were challenged with virus," said Nabel.

Twenty years ago Ebola virus first emerged in simultaneous outbreaks in Sudan (ref 1) and Zaire.(ref 2) Two subsequent outbreaks have occurred, (ref 3,4) but transmission among human populations has not been sustained. Despite substantial progress in our understanding of Ebola we have not identified its natural reservoir or the trigger for its re-emergence in new outbreaks in humans. As the journal went to press, the World Health Organisation had reported 114 cases of Ebola infection and 79 deaths in a new outbreak centred in Kikwit, a rural town of 400000 situated in Bandundu Province, Zaire, 1000 km from the location of the 1976 outbreak in Zaire.(ref 5) A cordon sanitaire has been placed around the town, but some travellers have circumvented it. The few cases reported in nearby towns have so far been among already ill patients transferred from Kikwit to other hospitals. The index case, seen in early April, was a hospital laboratory worker presumed at first to have typhoid; subsequent cases were initially found among a surgical team and others who cared for the laboratory worker, with secondary spread to other health workers and to family members acting as carers. Two thirds of the deaths have been among health workers. Until the outbreak provoked a response, Kikwit General Hospital was short of barrier nursing supplies and disinfectants. The current outbreak resembles earlier African out breaks, (ref 1,2,3) in which the first cases were found in hospitals where infection control mechanisms were not in place because of economic constraints. Ebola virus was spread to health workers in contact with body fluids, and also from patient to patient by the reuse of unsterilised needles. Secondary transmission occurred also among family members who administered care, among those who prepared corpses for burial, and in other towns where travellers from the epicentre subsequently became ill and infected new carers. Airborne spread is not considered important; one study noted that those exposed to ill patients even in small, crowded village huts were not at increased risk without direct contact.(ref 3)

Ebola outbreak controlled, say WHO
Story Tools GENEVA, Switzerland (Reuters) The toll from the Ebola outbreak in Congo has risen to 75 deaths among 93 cases, but is believed to be under control, the World Health Organization (WHO) said Tuesday. The outbreak in the Kelle and Mbomo districts near the central African country's border with Gabon is thought by scientists to have been caused by the consumption of infected monkey meat. The WHO last Wednesday put the death toll at 64. Ebola killed at least 73 people in Congo and Gabon in an epidemic from October 2001 to February 2002. It is passed on by infected body fluids and certain strains can kill between 50 and 90 percent of its victims through massive internal bleeding, The number of victims exceed those reported from Congo and Gabon a year ago and came from an investigating team of WHO and government experts deployed in the remote northwest area. "It would be unrealistic to say that we've seen all the cases, but it is realistic to say that we have seen most," WHO spokesman Ian Simpson told Reuters.

Story Tools BRAZZAVILLE, Congo Republic (Reuters) The deadly Ebola virus has killed 100 people in the remote forests of Congo Republic and wiped out nearly two-thirds of the gorillas in a reserve. "We have reached the fateful figure of 100 dead," Congo's Health Minister Alain Moka said on Tuesday at a ceremony to accept donations to help fight the outbreak. The latest Ebola epidemic to hit the central African country struck in January in the dense forest region of Cuvette-Ouest about 440 miles north of the capital Brazzaville. Ebola is passed on by infected body fluids and kills between 50 and 90 percent of its victims. It starts with a high fever and headache and can lead to massive internal bleeding. "The government has already spent more than $500,000 to put in place the logistics needed to help the stricken people but the state alone cannot help," Moka said. "We must have the support of everybody and the international community."

Level 4: Virus Hunters of the CDC , by Joseph B. McCormick and Sue Fisher-Hoch, both physicians, tells many stories of treating high-morbidity, high-mortality diseases in developing countries. It is an interesting book that continues several stories that have been emerging over the past five years - emerging viruses, of course, but also bacterial resistance, safe handling of infectious material, the dangers of used needles, and, of course, the environmental conditions that predispose humans to contract disease.
