Dyspareunia Dyspareunia is the clinical name for painful intercourse. This condition can occur at any age, in both sexes, and the pain can appear at the start of intercourse, midway through, at the time of orgasm, or after intercourse is completed. The pain can be felt as burning, sharp, searing or cramping. It can be external, within the vagina, or deep in the pelvic region or abdomen. The causes of dyspareunia, as with most sexual dysfunction, can be classified as either organic (physical or medical factors such as illness, injury or drug effects) or psychosocial (including psychological, interpersonal, environmental and cultural factors). The cause of a sexual dysfunction in a given individual may be a combination of several factors, and in some cases, the precise cause may not be identifiable at all. Other feelings and psychological conflicts may also affect sexual responsiveness, inhibiting or reducing vaginal lubrication, which can result in painful intercourse. Guilt, depression and poor self-esteem are commonly encountered in association with sexual dysfunctions. However, it is not always clear which came first, the feelings or the dysfunction. Because it is not unusual for people who have sexual problems to become depressed about them or to experience lowered self-esteem, identifying a problematic feeling does not always mean it caused the dysfunction. Dyspareunia is generally thought of as a female sexual dysfunction but it also affects males. Typically, the pain is felt in the penis but it can also be felt in the testes or internally, where it is often associated with a problem of the prostate or seminal vesicles.

As it is well known, during sexual intercourse, two sex organs are involved - the male's penis and the woman's vagina. The vagina, which is an external genital organ, is a tubular muscular structure about three inches long. It is capable of contraction and relaxation at will of the woman. At its entrance are two lips or labia, which close the entrance. In a young girl a delicate skin fold known as hymen is found in the vagina at its entrance. It may have a small hole. In women having normal sexual relation, the hymen is absent.
Causes of Dyspareunia
The pain of dyspareunia as suffered by a woman may have various causative factors. Most of them can be detected early and treated successfully resulting into a normal, enjoyable sexual intercourse. Certain congenital defects (defects that a person is born with) in vagina may obstruct the penetration of penis causing pain. The defects are easily detected and if needed surgically corrected.
The labia may be fused because of congenital anomaly or scarring following infection of labia. Surgical separation is possible in such a case. Also, an intact hymen causing dyspareunia can be excised.

Florida Hospital Family Practice Residency, Orlando, Florida Painful sexual intercourse, called dyspareunia in medical terminology (pronounced "dis-pair-une-ee-ah"), is a common complaint among women seeking gynecologic care. Many women report occasional pain with intercourse, but some women have pain with every episode of sexual relations. Although it may take a few visits to a gynecologist or other health care provider experienced in women's health, the cause of dyspareunia can usually be identified and treated. There are three main types of painful intercourse. Perhaps the least common is pain that occurs only after intercourse or orgasm. This can be due to uterine contractions from orgasm. Women with this problem may get relief by taking medications like ibuprofen before intercourse, which can block pain from uterine contractions. Another source of this type of pain is an allergy to semen, which is actually quite uncommon. After her partner ejaculates there can be an intense burning sensation and redness around the vagina and vulva (the outer "lips" of the vagina). There have actually been a few reported cases of women going into shock after sexual intercourse due to an allergy to semen. A third cause of pain after intercourse is a vaginal infection, such as a yeast infection, which can cause a burning sensation within the vagina due to irritation.

    The role of vaginal apex excision in the management of persistent posthysterectomy dyspareunia
    Am J Obstet Gynecol, Dec. 2000; 183: 1385-9
    Objective: The purpose of this study was to evaluate the effectiveness of vaginal apex excision in the treatment of patients with posthysterectomy dyspareunia.
    Study Design : This was a case series with an independent third-party survey of patients with posthysterectomy dyspareunia managed at the University of Utah Pelvic Pain Clinic. Thirteen patients were first treated with local injections of anaesthetics into localized vaginal pain foci. Further evaluation included formal psychometric testing and a diagnostic spinal block. Nine patients underwent surgical excision of the vaginal apex. An independent interviewer who did not know the patients assessed the effects of this procedure on dyspareunia and coital frequency at a mean of 36.4 ± 3.7 months after the operation.
    Conclusion: Excision of the vaginal apex is an effective treatment for carefully selected patients with posthysterectomy dyspareunia.

DYSPAREUNIA (PAINFUL INTERCOURSE) Gabe Mirkin, M.D. The medical term for painful intercourse is Dyspareunia. Pain causes a woman to fear lovemaking, which can cause her vaginal muscles to contract when they should be relaxing to cause an even more painful condition called vaginismus. Dyspareunia is classified into two types: superficial and deep. Superficial dyspareunia means that the pain come from the opening of or inside the vagina and is usually caused by vaginal infection; a dry vagina; a thin old vagina that needs estrogen to thicken it; skin conditions, such as lichen sclerosis, that can only be diagnosed by a biopsy; or old scars from childbirth or previous surgery. Vaginal infections are associated with itching, burning, a foul-smelling discharge, discomfort much of the time and urinary symptoms such as burning on urination, having to urinate frequently, urgency to urinate and having to get up in the middle of the night to urinate. Women who have an infection need to be treated as soon as possible because allowing a mycoplasma infection to go untreated can result in permanent pain when the bladder is full and arthritis; and untreated vaginal warts can lead to cancer. The most common cause of superficial dyspareunia is an ignorant partner. Most women need time to lubricate. Inserting a penis into a dry vagina can tear the vaginal linings as well as the skin on the penis. No caring individual would ever insert his penis into a vagina until it is lubricated. Most women prefer to use oral sex as foreplay. She often will lubricate while she is performing oral sex and he can lubricate her by depositing saliva, the most effective lubricant known to man. If taking your time doesn't lubricate the vagina, you may need to treat an infection or lack of estrogen. If oral sex turns you off, you may need to use a non-greasy lubricant such as KY jelly that you can buy at any drug store. Greasy lubricants can damage condoms and can be aesthetically unpleasant.

Physical Causes

  • Infections, such as thrush, warts, herpes, sexually transmitted diseases.
  • Medical conditions, such as endometriosis, where the womb lining grows in the wrong place.
  • Pain syndromes, such as Vulvodynia, when there is a burning or severe aching feeling, which can occur without any sexual activity.
  • Birth conditions, such as imperforate or unopened hymen, or splits or fissures in the wall of the vagina.
  • Surgical problems, such as adhesions after an operation on your tummy.
  • After childbirth, while the vagina recovers, it can pe painful for up to a year. Once you have checked with a gynaecologist that it is healed and there is no physical cause for your pain, it is OK to have sex.
