Dyspareunia in women
Clinicians should consider dyspareunia to be primarily a physical, rather than an emotional, problem until proven otherwise. In most instances of dyspareunia, there is an original physical cause. Among many African women, dyspareunia results from the damage caused by female genital mutilation When pain occurs, the woman may be distracted from feeling pleasure and excitement. Both vaginal lubrication and vaginal dilation decrease. When the vagina is dry and undilated, penile thrusting is painful. Even after the original source of pain (a healing episiotomy, for example) has disappeared, a woman may feel pain simply because she expects pain. In brief, dyspareunia can be classified by the time elapsed since the woman first felt it:
  • During the first 2 weeks or so, dyspareunia caused by penile insertion or movement of the penis in the vagina or by deep penetration is often due to disease or injury deep within the pelvis. After the first 2 weeks or so, the original cause of dyspareunia may still exist with the woman still experiencing the resultant pain. Or it may have disappeared, but the woman has anticipatory pain associated with a dry, tight vagina.

Genital pain before, during, and/or after intercourse. It may affect up to 2/3 of all women. The medical literature does not estimate the number of men with this condition. Men with this condition, please read the next section. On the humorous side, it has been said that: "Dyspareunia is better than no pareunia.” Dyspareunia (difficult mating) is defined as genital pain that occurs before, during, or after intercourse. This is not a well-understood condition. It can be life-long or acquired (2). Dyspareunia is a common sexual problem. Some sources estimate dyspareunia occurs in two thirds of all women (2). One study concluded dyspareunia is common and usually appears in young women at the beginning of their sex lives. The fact that medical professionals are often not aware of this syndrome can cause patients much frustration and misery (1). The medical literature does not quantify the number of men faced with this problem. Many people discuss pain with their sexual partner; most people do not seek the advice of a physician.

Dyspareunia and vaginismus are the two most common sexual dysfunctions in women. Dyspareunia is recurrent genital pain caused by sexual activity. Primary dyspareunia is defined as constant pain during sexual activity, while secondary dyspareunia occurs after a period of pain-free lovemaking. Vaginismus is a conditioned pain caused by involuntary spasm of the muscles around the lower one third of the vagina, resulting from the association of sexual activity with pain and fear. Butcher reviews these two conditions and their respective management approaches and briefly discusses the role of orgasm in women. The immediate cause of vaginismus is involuntary spasm of the muscles around the lower one third of the vagina. This initial response may be secondary to sexual abuse, frightening childhood medical procedures, painful first intercourse, relationship problems, sexual inhibition or fear of pregnancy. Primary vaginismus is diagnosed in women who have never experienced vaginal penetration, while secondary vaginismus denotes prior successful vaginal penetration. The symptoms can range from minor to severe, when the woman avoids all forms of sexual touching or intimacy. Patients in the latter category often have been unable to complete gynecologic examinations, have difficulty using tampons and fail to present for Papanicolaou tests.

Dyspareunia is the umbrella term for painful sexual intercourse in women. The causes of dyspareunia vary widely, and include, but are not limited to, infection, an abnormally positioned uterus, muscle spasms in the vagina, preexisting disorders of the reproductive system, and emotional distress from prior sexual trauma or abuse. The pain, depending on its cause, can range from mild to intense, and women who suffer from dyspareunia regularly can find it very distressing, because of both the physical discomfort and the impact on their intimate lives.
Detailed Description
Dyspareunia may arise from a broad range of underlying problems. If dyspareunia involves the vaginal tissue and entrance of the vagina, it could be cause by vaginal dryness due to lack of arousal, side effects of medication (allergy, high blood pressure, anti-depressant medications are most often to blame), or menopause and the friction of intercourse. Other common causes of this type of dyspareunia include allergic reactions to laundry detergents, soaps, or other topical products such as spermicides. Bacterial or fungal infections that irritate vaginal tissue can also be to blame, as can STDs such as herpes and genital warts. Another potential cause of this type of dyspareunia can be a condition called vulvodynia, in which the vulva are tender and hypersensitive to touch.

Definition
Dyspareunia refers to pain in the pelvic area during or after sexual intercourse. This can occur in both men and women although it is more common in women.
Causes
Women It is believed that dyspareunia is caused by organic (physiological) factors at least 75% to 80% of the time. Pain that occurs initially at entry but decreases over time is commonly caused by inadequate lubrication. This is frequently due to a lack of sexual arousal and effective stimulation, and sometimes due to medication that decreases vaginal lubrication (such as antihistamines). Other causes in women include vaginitis (vaginal infections), endometriosis, cystitis or urethritis, orthopedic problems, retroversion of the uterus, and chronic constipation. Psychological factors are infrequently involved and will most commonly be associated with previous sexual trauma (rape or abuse), feelings of guilt, or negative attitudes toward sex. Men In men, the most common causes of pain that occur at the time of ejaculation are prostatitis and urethritis. This is often associated with a recent reduction in the frequency of sexual activity.

Background Dyspareunia is a multi-faceted problem that can involve a number of factors and disease states. Women rarely report it on their own but may discuss the issue upon direct inquiry. Evaluation of a patient responding affirmatively should involve a complete and careful sexual history and a comprehensive physical exam. It is important to identify the severity, character and location of pain during intercourse. This information can be used to determine whether the patient is experiencing superficial or deep dyspareunia or both. In cases of deep dyspareunia, particular attention should be paid to uterine position during the pelvic exam. If the pain experienced during intercourse can be reproduced with a digital exam, the patient is a good candidate for uterine suspension. Some investigators have advocated a pessary trial prior to uterine suspension. However, others have noted that the trial is unnecessary and that pessary use can cause sexual dysfunction and further gynecological concerns. [1] Uterine suspension represents a long-term solution for women with dyspareunia secondary to a retroverted uterus.
