Painful coitus or attempted coitus.
Etiology
Dyspareunia is usually introital but may also occur before, during, or after intercourse. The cause may be psychologic factors or local trauma (eg, hymenal tears, laceration of the fourchette, bruising of the urethral meatus). After an injury, painful superficial ulcerations may develop. Other causes include inadequate lubrication, usually secondary to improper or insufficient foreplay; forceful pressure against a sensitive urethra during coitus; improper intromission; introital lesions due to inflammatory conditions (eg, vestibulitis); infections (eg, abscesses of Bartholin's glands or ducts); inflammation of labial sweat glands; irritation due to the use of improperly fitted or inadequately lubricated condoms; allergic reactions to the contents of contraceptive foams and jellies and condoms; abnormalities of the female genital tract (eg, congenital septum, a rigid hymen); and dermatologic disorders (eg, lichen sclerosis). The most common psychologic cause is vaginismus (see below). Acquired dyspareunia is unrelated to first coitus and often develops years later. Causes include menopausal involution with dryness and thinning of the mucosa, tight introitus secondary to perineorrhaphy after an episiotomy or plastic repair of the vagina, marked retroflexion of the uterus with ovaries prolapsed into the cul-de-sac, endometriosis, vaginitis, suburethral diverticulum, and pelvic inflammatory disease. Radiation therapy for malignancy may result in dyspareunia. Psychologic and related factors are similar to those involved in female orgasmic disorder (see above). Inadequate stimulation or psychologic inhibition of arousal may result in inadequate vaginal lubrication and cause coital pain.

Dyspareunia
Dyspareunia (dys*pa*reu*nia) means difficult or painful sexual intercourse. Although the condition is present in both sexes, it is far more common in women, where it is being described as pain with initial penetration, during thrusting, or after intercourse. I never understood why tampons were so painful when everyone can use them just fine . . . Intercourse? I expected pain the first time, but it keeps being painful every time, from the moment he begins to penetrate until he is done. Although we have two kids, I am reluctant to engage in intercourse and still cannot use tampons. Gynecologic exams are also painful, and the doctor does not know what is the problem.

What are some of the causes of dyspareunia?
Any of the genital parts can cause pain during sex. Some conditions affect the skin around the vagina. Some women have a viral infection or vaginal yeast infections, but sometimes the cause is unknown. The pain from these conditions is usually felt when a tampon or penis is inserted into the vagina. Sometimes, even sitting or wearing pants can cause discomfort. We know that the mind and the body work together. This is seen with sexual problems. Often the problem that first caused the pain may go away, but you may have learned to expect the pain. This can lead to further problems because you may be tense or you may be unable to become aroused. The problem can then become a cycle and you are caught in the middle. Return to top

PAINFUL INTERCOURSE (DYSPAREUNIA) Many women have had the experience of discomfort or pain with sexual intercourse. This can be felt as external or deep or both. The causes and solutions depend on the exact details.
EXTERNAL (OR SUPERFICIAL) DYSPAREUNIA
This type of discomfort usually feels like a burning sensation or a sensation of dryness, with too much friction. This most commonly occurs as a result of inadequate lubrication. The solution is more foreplay to increase arousal and natural lubrication and/or assisting nature by adding some lubrication. External dyspareunia can also be a sign of infection, most commonly a yeast infection. Persistence of this problem requires an evaluation by a gynecologist. When you go to the doctor, make sure you tell him or her exactly what's wrong. If you just go in for a regular check-up, you may not have the problem solved.
DEEP DYSPAREUNIA
Many women sometimes feel discomfort or pain with deep penetration. This may be a normal response to the pressure on sensitive internal parts. The obvious solution is to avoid deep penetration. Some common solutions include:

    Dyspareunia
Dyspareunia (painful sexual intercourse ) often can be resolved, even when long-standing, self-perpetuating pain is a factor. This article is about dyspareunia in women. Clinicians should consider dyspareunia to be primarily a physical, rather than an emotional, problem until proven otherwise. In most instances of dyspareunia, there is an original physical cause. Among many African women, dyspareunia results from the damage caused by female genital mutilation When pain occurs, the woman may be distracted from feeling pleasure and excitement. Both vaginal lubrication and vaginal dilation decrease. When the vagina is dry and undilated, penile thrusting is painful. Even after the original source of pain (a healing episiotomy, for example) has disappeared, a woman may feel pain simply because she expects pain. In brief, dyspareunia can be classified by the time elapsed since the woman first felt it:
    • During the first 2 weeks or so, dyspareunia caused by penile insertion or movement of the penis in the vagina or by deep penetration is often due to disease or injury deep within the pelvis. After the first 2 weeks or so, the original cause of dyspareunia may still exist with the woman still experiencing the resultant pain. Or it may have disappeared, but the woman has anticipatory pain associated with a dry, tight vagina.

and Dyspareunia (pain with sexual intercourse) - pelvic congestion
Q 23. What causes pain associated with sexual intercourse (dyspareunia)? Dyspareunia is a very distressing problem, which may have dire effects on a relationship. Two types of dyspareunia relating to their site are recognised. Superficial dyspareunia is pain around the entrance to the vagina. Superficial dyspareunia may be due to vulval problems ( Q 31.1 ), vestibulitis ( Q 31.13 ) or vaginal problems such as vaginitis ( Q 22.6 ). Another possible cause is vaginismus where there is tension or spasm of the muscles around the vaginal entrance. The spasm may be such that the couple believe that there is an obstruction requiring surgical correction. Sometimes there is deeper pain and vaginismus is a protective mechanism. More frequently there is no disease but a functional problem due to an earlier sexual encounter, fear of pregnancy or inadequate arousal. Deep dyspareunia is pain deep inside the vagina and pelvis. Endometriosis ( Q 23.18

and Dyspareunia (pain associated with sexual intercourse).
Q 23. 32 How can dyspareunia be treated? Dyspareunia may result in apareunia (love-making has stopped – or never commenced) and relationships may be put in jeopardy. Dyspareunia and reduced libido are about the most sensitive of symptoms that a woman may experience and support and understanding are essential. If a psychological or marital problem is suspected an appropriate counsellor or psychiatrist may be required to provide support. The story, examination findings and appropriate investigations will indicate the underlying cause of the problem. When a specific cause is found appropriate treatment can be instituted. Sometimes pelvic congestion may occur if there has been a high degree of arousal but not orgasm. Lubricants, such as KY jelly may help when natural lubrication is inadequate. Local trauma (physical damage) either arising childbirth or from injudicious sexual activity will usually heal with time. Sometimes healing tissue (granulation) may require cauterisation with a silver nitrate stick in the clinic (this is not painful). Infections such as a folliculitis (“a boil”), candida ( Q 22. 6

    dyspareunia Alternative Names
painful sexual intercourse, pain with intercourse
    Definition
Dyspareunia is pain in the vagina when the penis is inserted during sexual intercourse.
What is going on in the body?
Pain during sexual intercourse can be upsetting. The cause of the pain may be hard to find. There might be physical and psychological causes.
Physical causes may be within the female reproductive tract, including the vulva, vagina, cervix, uterus, Fallopian tubes, ovaries and abdominal cavity.
Some women may assume that sex will be painful. Or they might associate menstruation and childbirth with pain. Even when no physical causes are found and the pain is gone, the memory of the pain may persist. This can interfere with pleasure. The range of physical and psychological factors that prevent a woman from enjoying a sexual relationship should be considered.
What are the signs and symptoms of the condition?
Dyspareunia may be divided into two categories, superficial and deep. Superficial means that there is immediate pain at the opening of the vagina when a man attempts to insert the penis. The most likely reasons for this are tears, infections or structural defects of the vulva or vagina.
Deep dyspareunia means that there is pelvic pain or discomfort after penetration of the penis. This is usually associated with deep thrusting. The likely causes are abnormalities of the deeper structures of the vagina. Typical symptoms include:
