Menstrual cramps (Dysmenorrhea)
Description Dysmenorrhea is the medical term for menstrual cramps. Dysmenorrhea affects most women at some stage during their reproductive years. There are two types of dysmenorrhea, primary dysmenorrhea and secondary dysmenorrhea. Primary dysmenorrhea is now believed to be caused by an excess of one of the prostaglandin hormones normally found in the body. Excess amounts of this horm one result in excessive contractions of the uterus (cramping). Secondary dysmenorrhea may be caused by an underlying disease, infection, or gynelogical problem. Symptoms Abdominal cramps, lower back pain, and leg pain are the most common symptoms of dysmenorrhea. Nausea and vomiting, constipation or diarrhea may also be present. If the pain is severe, light-headiness may be experienced. Primary dysmenorrhea typically begins with the onset of the menstrual period and usually does not last more than two days. Secondary dysmenorrhea may begin prior to the menstrual period and last throughout the period. If the pain in the pelvic area is accompanied by fever, abnormal vaginal bleeding and/or pain with intercourse, it may indicate a serious infection and medical care should be sought promptly. Treatment The first important step in treatment is to see a Women's Health Care clinician for an examination in order to rule out the possibility of underlying disease or anatomic abnormality as the cause of dysmenorrhea. Secondary dysmenorrhea is often difficult to identify and treat.There are a number of treatment alternatives for primary dysmenorrhea. Antiprostaglandin medications, which inhibit the production of prostaglandin hormones, are available and frequently provide relief. These include ibuprofen and aspirin, which can be purchased over the counter, and related medications, which require prescriptions. They are non-narcotic. Women who are allergic to aspirin, have severe or persistent anemia, or have ulcers or intestinal bleeding

CONDITION: DYSMENORRHEA I. Overview. Primary dysmenorrhea is defined as recurrent cramping pain in the lower abdomen occurring just before or during menstruation, in the absence of other diseases such as endometriosis. Prevalence rates are as high as 90 %. Women with primary dysmenorrhea have increased production of endometrial prostaglandin, resulting in increased uterine tone, and stronger, more frequent contractions. 90% of women will respond to nonsteroidal anti-inflammatory medication or oral contraceptives. II. Aeromedical Concerns. Primary dysmenorrhea causing recurrent pelvic pain can be a distraction in flight and may occasionally cause incapacitation. Aircrew should be grounded during a work-up for significant recurrent pelvic pain until the etiology is known or the condition is controlled. Significant recurrent pelvic pain is defined as that which necessitates recurrent absences of more than a few hours of routine duty.
III. Information Required for Waiver Submission. An aeromedical summary should contain the following: date of diagnosis, history of symptoms, history of work absences, treatments used, last symptoms, side effects on medication, and any current medications or ongoing treatments.

Dysmenorrhea Pilot Study
Spinal Manipulation Found Effective in Decreasing Menstrual Pain
The results of a recent clinical pilot study originally conceived and developed by two National College of Chiropractic students, Katrina Kokjohn, D.C., and Della Schmid, M.S., D.C., "Pain and Prostaglandin Levels in Dysmenorrheic Women following Spinal Manipulation," indicates that menstrual pain decreased significantly for those treated by spinal manipulation. Patricia Brennan, Ph.D., dean of research at National College of Chiropractic supervised the research team responsible for the project. The major funding for the pilot study came from the National College of Chiropractic, with support by a grant from the Foundation for Chiropractic Education and Research (FCER), and tertiary support from the Practice Consultants Clinical Research Center, which was established by FCER through a grant from William Harris, D.C., to help foster scientific research in the profession. While more testing is called for, this trial found that women who received chiropractic treatment, consisting of spinal manipulation, reported significant reductions in back pain and menstrual distress. A measurable reduction in the substance believed to be responsible for menstrual pain was also noted.
