Dysmenorrhea (painful menstruation) means that women have periodic pain during or prior to or after menstrual period in the lower abdomen or even faint in severe cases. It has been customary to classify cases of dysmenorrhea into two main groups: primary or functional dysmenorrhea referring to the one which is not caused by organic diseases, and secondary dysmenorrhea referring to the one caused by organic diseases in reproductive system. Main Points of Diagnosis 1. The pain in the lower abdomen appears with menstrual cycle. 2. The pain can spread over the whole abdomen, lumbosacral region or there is dragging pain in the vulva and anus. It varies in degrees and can be relieved when the menstrual blood flows smoothly. 3. Gynecological examination: Those without organic diseases in the pelvic cavity are primary dysmenorrhea; those with organic diseases in the pelvic cavity are secondary dysmenorrhea.

Basic Information

Description
    Severe, painful cramps during menstruation. Primary dysmenorrhea means pain has recurred regularly or within a year or two of the first period (puberty). Secondary dysmenorrhea means pain began years after periods started. Women with dysmenorrhea are generally fertile. Severity of symptoms varies greatly from woman to woman, and from one time to the next in the same woman. Dysmenorrhea usually is less severe after a woman has had a baby.
Frequent Signs and Symptoms
  • Cramping and sometimes sharp pains in the lower abdomen, lower back and thighs. The pain starts at onset of menses and lasts for hours to days.
  • Nausea and vomiting (sometimes).
  • Diarrhea(occasionally).
  • Sweating
  • Lack of energy.
  • Urinary frequency.
  • Irritability, nervousness, depression.
Causes
  • Strong or prolonged contractions of the muscular wall of the uterus. These may be caused by concentration of prostaglandins (hormones found in the cervix and uterus). Research shows that women with dysmenorrhea produce and excrete more prostaglandins than those who don't have as much discomfort.
  • Dilation (stretching) of the cervix to allow passage of blood dots from the uterus to the vagina in cases where the cervix is narrowed or constricted
  • Other causes include:
    • Pelvic infections.

Period pains are cramping abdominal pains experienced during, and sometimes just before, a woman's monthly period. They affect 80% of women at some time in their lives. Usually they are not a sign of a serious underlying problem and can be treated with self-help methods. ... The cause of period pain is not certain. Once an egg has been released from one of the ovaries, natural chemicals produced by the body called prostaglandins are made in the lining of the uterus (womb). Some prostaglandins cause the walls of the uterus to contract. Some women produce higher levels of prostaglandins, which may cause increased contractions of the uterus. These cramps may be more painful because there is reduced blood (and therefore oxygen) supply to the myometrium (muscle wall of the uterus) during the contractions.

Cramping lower abdominal pains are the most common symptom. Pain can also spread to the lower back and the thighs. When severe, the pain can be accompanied by nausea or vomiting, diarrhoea, constipation or feeling faint. Some women may also get headaches.

Sometimes a combination of factors may be responsible. Medical history and a careful physical exam are helpful in diagnosing the problem. The Causes of Painful Periods Include:
  • Tipped uterus or retroverted uterus A tipped uterus may also cause pain during intercourse Endometriosis The tissue lining the uterus – the endometrium – may grow outside the uterus causing pain during periods. Hormonal changes and imbalances – A hormone produced by cells in the uterine lining called prostaglandin causes uterine contractions. Overview Even if you're not familiar with the term, if you're a woman, chances are you've experienced dysmenorrhea at some time in your life. Dysmenorrhea is simply the medical term for menstrual cramps, that dull or throbbing pain in the lower abdomen many women experience just before and during their menstrual periods. For some women, the discomfort is merely annoying. For others, it can be severe enough to interfere with everyday activities for a few days every month. Dysmenorrhea can be primary or secondary. Primary dysmenorrhea involves no physical abnormality. These so-called normal cramps affect 50 percent to 90 percent of all menstruating women. Primary dysmenorrhea usually begins within three years after a girl begins menstruating. Secondary dysmenorrhea involves an underlying physical cause, such as endometriosis or uterine fibroids.

Menstrual cramps start a day or so before actual menses. They are felt in the lower or middle abdomen. They can radiate (spread) to the hips, thighs, and back. They are decribed as a "colicky" type of pain. That means the pain rises to a peak and falls, and starts over again. This reflects the contractions of the uterus that underlie the cramps.
PRIMARY (NORMAL) VS. SECONDARY (UNDERLYING PROBLEMS): WHO GETS WHICH
Secondary dysmenorrhea is when cramps are due to an actual disease, such as endometriosis. The severity is NOT a clue as to whether or not the cramps are due to a disease that needs to be treated.
THE GOOD NEWS
Menstrual cramps are closely tied to ovulatory cycles. So, that means women who have them are showing an important sign of their potential for fertility. MECHANISM: HOW MENSTRUAL CRAMPS HAPPEN
A substance called prostagladins are the cause of most symptoms with dysmenorrhea. These are produced by the body and found in the uterine lining. When the lining starts to shed with menstruation, they are released. They cause the uterus to contract forcefully, which is the reason for much of the cramping itself.
SELF TREATMENT FOR MENSTRUAL CRAMPS
Medications
The main treatment for menstrual cramps are a class of drugs called Non-steroidal anti-inflammatory drugs or NSAIDs for short. These are drugs every woman knows: ibuprofen (Motrin) or Naproxen (Aleve). They work by stopping the body from making prostagladins. They also work by preventing blood clotting.

Dysmenorrhea is the medical name for the pain of severe and disabling menstrual cramps. Approximately 52% of women are affected by dysmenorrhea and about 10% of these are incapacitated each month for up to three days. The pain usually starts as menstruation starts, but mild cramping or lower abdominal pain may begin 24 to 48 hours before the flow begins. It may last only a few hours until the flow is well-established, but some women are troubled for several days. What causes Dysmenorrhea?
The exact cause of dysmenorrhea is still unknown, but a group of substances called prostaglandins have been the focus of recent attention. Prostaglandins are produced by the lining of the uterus and cause the muscle wall of the uterus to contract, producing the type of pain most women with dysmenorrhea have. Women with severe cramps appear to produce more of these chemicals than women with mild or no cramps, especially just before and on the first day of their periods. Are there other symptoms?

Dysmenorrhea is the occurrance of painful cramps during menstruation. Description More than half of all girls and women suffer from dysmenorrhea (cramps), a dull or throbbing pain that usually centers in the lower mid-abdomen, radiating toward the lower back or thighs. Menstruating women of any age can experience cramps. While the pain may be only mild for some women, others experience severe discomfort that can significantly interfere with everyday activities for several days each month. Causes and symptoms Dysmenorrhea is called "primary" when there is no specific abnormality, and "secondary" when the pain is caused by an underlying gynecological problem. It is believed that primary dysmenorrhea occurs when hormone-like substances called "prostaglandins" produced by uterine tissue trigger strong muscle contractions in the uterus during menstruation. However, the level of prostaglandins doesn't seem to have anything to do with how strong a woman's cramps are. Some women have high levels of prostaglandins and no cramps, whereas other women with low levels have severe cramps. This is why experts assume that cramps must also be related to other things (such as genetics, stress , and different body types) in addition to prostaglandins. The first year or two of a girl's periods are not usually very painful. However, once ovulation begins, the blood levels of the prostaglandins rise, leading to stronger contractions.

Estrogen is released to stimulate the growth of the uterine lining. Then the pituitary gland releases a hormone to prepare the egg. Progesterone is released which further increases the blood vessels in the uterus. The egg is released during ovulation. If the egg is not fertilized the levels of progesterone and estrogen drop and menstruation occurs. The blood vessels are shed from the uterus during menstruation, the first day usually being the heaviest. Menstrual cramps usually occur on the first day of the menstruation cycle when the flow begins. Cramps can last for a couple of hours to a few days. The cramps feel like

DYSMENORRHEA Painful menstruation is difficult to treat, not because the disorder will not yield to therapy, but because women have about the worst record of noncompliance to medical counsel with this disorder that with any other. The reason for noncompliance will become obvious as the causes of dysmenorrhea are discussed. One important cause of dysmenorrhea is that of wearing improper clothing. Tight bands around the waist, hips, or thighs impede the circulation to the pelvic organs and increase the likelihood of painful menstruation. Also, the clothing of the extremities must be proper. There should not be one inch of chilled skin on the extremities any time during the month, not only during the menstrual period itself. The extremities should be covered with as many layers of clothing as is the trunk. To fail to do so is to cause the circulation to be imbalanced, and increases the likelihood of pelvic congestion and painful menstruation. Most American women do not get sufficient exercise; this fact is second only to improper clothing in the causation of dysmenorrhea. A study showed that 75% of a group of 5,000 junior high school girls were either cured or had definite improvement of menstrual discomfort with simple bending and stretching exercises. The first exercise was stretching, done by touching the fingers to the ankles with the knees held straight. The second exercise was thrusting the leg backward as the arms were swung high over the head. These two exercises performed 4-10 times daily, four times weekly for several weeks resulted in a 75% improvement rate. In our experience, one hour of useful vigorous labor out-of-doors daily has been more effective in curing dysmenorrhea than have calisthenics.

Dysmenorrhea, or painful menstruation, is classified as either primary or secondary. Primary dysmenorrhea generally occurs within a couple of years of the first menstrual period. The pain tends to decrease with age and very often resolves after childbirth. Secondary dysmenorrhea is menstrual pain caused by another condition, commonly endometriosis . It starts later in life and tends to increase in intensity over time. As many as half of menstruating women are affected by dysmenorrhea, and of these, about 10% have severe dysmenorrhea, which greatly limits activities for one to three days each month.
