Pituitary dwarfism is a condition of growth retardation characterized by patients who are very short in stature but have normal body proportions. It is caused by a dysfunction of the pituitary gland, the pea-sized mass of tissue located at the base of the brain. (Illustration by Electronic Illustrators Group.) Definition
Pituitary dwarfism is a condition of growth retardation in which patients are very short, but have normal body proportions. Some children who have this condition go through delayed, but normal puberty and have normal reproductive capabilities. Others never become sexually mature. Description
Pituitary dwarfism is caused by a dysfunction of the pituitary gland. The pituitary gland is a pea-sized mass of tissue at the base of the brain. Functionally, it is divided into two parts, the anterior pituitary and the posterior pituitary. The anterior pituitary secretes growth hormone (GH), thyroid-stimulating hormone (TSH), follicle-stimulating hormone (FSH), luteinizing hormone (LH), adrenocorticotropic hormone (ACTH), and prolactin. The posterior pituitary releases antidiuretic hormone (ADH) and oxytocin. There are two types of pituitary dwarfism:
  • Panhypopituitarism due to a deficit of all of the anterior pituitary hormones. There is proportionate, generalized slow growth and patients do not go through puberty. This accounts for about 2/3 of pituitary dwarfism cases.

Review of the Annotated Bibliographies section in the article "Growth and Failure to Grow" by Patti Rieser (PENS Reporter; Fall 1997) cited an informative article by Stanhope (1994) regarding the diagnosis of psychosocial dwarfism. The Endocrine Service at Cardinal Glennon Children's Hospital (CGCH) has followed a very interesting case of psychosocial dwarfism. CM, a 7-year-old white female presented to CGCH in June 1995 for evaluation of short stature. She was brought to the endocrinologist by both parents at the suggestion of the primary care physician. The parents describe CM as a picky eater and allergic to cow's milk. They were concerned about other children "babying" her. The parents stated they had been working with the school to help CM with her "special needs." Physical exam revealed a very cooperative, quiet, poised child with a high-pitched squeaky voice. CM had the appearance of a 3 year old, rather than a 7 year old. Height measured at 95.8 cm (-4.5 standard deviations below the mean). Weight was 14 kg (-3 standard deviations below the mean). Review of past measurements revealed a cessation of growth between the ages of 5 to 7 years. Possible differential diagnoses included hypopituitarism, psychosocial dwarfism, and GI disorder. Screening labs were negative with the exception of IGF-1=18ng/ml (low) and T4=4.9 (low normal). Bone age was 3 years 6 months with chronological age being 7.5 years. Subsequent growth hormone stimulation testing X 2 revealed low peak growth hormone levels of 6.8 and 4.0 respectively. MRI was interpreted as normal.

Genetic mechanisms in equine dwarfism
Dwarfism is considered rare in large horses and considered fare more common in USA miniature horses, especially among those who breed horses with known dwarf ancestors. Some suggest that equine incest may contribute to horse dwarfism. This author published that her dwarf horse was "the result of an accidental breeding between a full brother & sister". Regardless of the mechanism, it appears that some USA miniature breeding programs suffer from very high reproductive mortality. Fox Holler Farm published their 2004 breeder-reported statistics. These publication says that, as of March 1, 2004, 69 pregnancies resulted in the deaths of 24 foals, a astronomical infant mortality rate of more than one-third. This suggested link between genetics and miniature horse dwarfism is not surprising, given that the hallmark of dwarfism is a small size. One author has published photographs of dwarf miniature horses and claims that "bug" (their dwarf horse) had a champion AMHA pedigree: "

A Little Person On 'Little People' Recently, I gave myself the title of being the first person in my county's entire library system to get their hands on a copy of Dan Kennedy's book, Little People. While to most this was no
extraordinary feat, but my anticipation of getting to read this was not unlike a child waiting for Christmas morning; this book was considered to me, and possibly the rest of the dwarf community, to might as well have been on the New York Times bestseller list.
As someone who has recently re-introduced herself to LPA, after a 13 year period of 'denial' (as some put it), I have since amassed a collection of every kind of informational resource there is on dwarfism, and LPA in general.
I can say, without a doubt, Dan Kennedy's book is by far the best, if not most brilliant work on the subject there is today, something cannot say about the typical 'medical textbook'-style writing on dwarfism I have become so used to.
