Background: Globe retraction occurs when globe is displaced deeper within the orbit from its normal position. It may occur from active co-contraction of the horizontal rectus muscles such as in Duane retraction syndrome. Patients with Duane syndrome have strabismus, upshooting or downshooting eye movements, and narrowing of palpebral fissure and retraction of globe on adduction. Enlargement of orbital cavity after blowout fractures also may cause the globe to be retracted. It may be due to prolapse of orbital contents into adjacent sinus, atrophy of orbital fat, or contracture of necrotic extraocular muscles entrapped within the fracture. Globe retraction also can be seen in metastatic scirrhous breast carcinoma from cicatrization of orbital tissue. Although less common, some cases of sclerosing orbital pseudotumor have been reported to cause globe retraction. A thorough history and examination are required to determine appropriate management for patients with globe retraction. Pathophysiology: Mechanism for globe retraction in Duane retraction syndrome is believed to be anomalous innervation of lateral rectus muscles from branches of oculomotor nerve (cranial nerve III). Both electromyographic and autopsy studies in Duane syndrome patients have demonstrated this anomalous innervation. In attempted adduction, simultaneous contractions of lateral and medial rectus muscles cause the globe to retract. Anomalous innervation between medial rectus and vertical rectus or oblique muscles also may explain upshoots and downshoots seen in adduction.

Our laboratory is studying the genetic basis of Duane syndrome, but we have not yet identified any Duane syndrome gene loci. Duane syndrome (DS) is an eye movement disorder present at birth (congenital) characterized by a limited ability to move an eye inward towards the nose (adduction), outward towards the ear (abduction), or in both directions. In addition, when the affected eye(s) move inward towards the nose, the eyeball retracts (pulls in) and the eye opening (palpebral fissure) narrows. In some cases when the eye attempts to look inward, it moves upwards (upshoot) or downwards (downshoot). Similar to CFEOM, Duane syndrome falls under the larger heading of strabismus (misalignment of the eyes) under the subclassification of incomitant strabimsus (misalignment of the eyes which varies with gaze directions) and subheading of extraocular muscle fibrosis syndromes (conditions associated with restriction of both active and passive movement of the eyeball). Although "muscle fibrosis" suggests that syndromes under this heading are primary disorders of muscle, evidence suggests that DS (and other syndromes under this heading, including CFEOM) may actually be primary disorders of nerve innervation. Bilateral Duane's retraction syndrome. A clinical-pathologic case report. Hotchkiss MG, Miller NR, Clark AW, Green WR. Duane's retraction syndrome (DRS) is a congenital eye movement disorder characterized by abduction deficiency, adduction limitation, globe retraction, and palpebral fissure narrowing on attempted adduction. Although data from surgical and autopsy studies suggest a neurogenic cause for this disorder, only one well-documented case of DRS has been studied pathologically, and that report was incomplete. We describe the intracranial and orbital pathology of a clinically documented case of bilateral DRS. Both abducens nuclei and nerves were absent from the brainstem, and the lateral rectus muscles were partially innervated by branches from the oculomotor nerves. This report presents evidence that a cranial nerve anomaly may cause DRS. The reason for this neurogenic abnormality is briefly discussed.

In the present paper the authors report a case of bilateral Duane retraction syndrome with an extraordinary hand abnormality. Numerous theories concerning the etiology and pathogenesis of Duane retraction syndrome and its relationship to the associated findings have been proposed, but the exact mechanism is still under investigation. Further multi-centered studies and the classification of the associated clinical findings in an organized manner may help to explain this mechanism.

Duane Retraction Syndrome
A syndrome characterized by marked limitation of abduction of the eye, variable limitation of adduction and retraction of the globe, and narrowing of the palpebral fissure on attempted adduction. The condition is caused by aberrant innervation of the lateral rectus by fibers of the oculomotor nerve . There are three subtypes: type 1 (associated with loss of abduction), type 2 (associated with loss of adduction), and type 3 (loss of abduction and adduction). Two loci for Duane retraction syndrome have been located, one at chromosome 8q13 (DURS1) and another at chromosome 2q31(DURS2). It is usually caused by congenital hypoplasia of the abducens nerve or nucleus, but may rarely represent an acquired syndrome .     Hello, I am Caroline Andrews, the Research Coordinator for a laboratory based at Children’s Hospital, Boston, USA www.childrenshospital.org/research/engle , whose focus is to identify and study the genes mutated in eye movement disorders. Duane syndrome is one such disorder that we are studying. Our lab recently identified the first Duane syndrome gene, SALL4. This gene was identified in three families who had Duane syndrome coinherited with radial ray anomalies (Okihiro syndrome or Duane radial ray syndrome) and was published in the American Journal of Human Genetics, 2002 Nov;71(5)1195-9. We believe that this gene may play an important role in development of the abducens nerve (Cranial Nerve VI). The reason why I am posting this message on this website is because there have been several reports in medical literature describing individual's who have a microdeletion on chromosome 22 and also have Duane retraction syndrome. It is therefore postulated that Duane retraction syndrome might be a new feature in 22q11 deletion syndrome. We would love to hear from you if you have 22q11 and Duane syndrome and to enroll you in the genetic study that we are undertaking here at Children's Hospital, Boston. Participation is very easy and we have enrolled study participants from all over the world. Ideally we would like to obtain a blood sample from each participant but if you are reluctant to have your blood drawn we would be very happy to obtain cheek swabs from you instead (although the yield of DNA obtained is usually much lower than from blood). Providing a cheek swab can be easily undertaken by simply scraping the lining of the cheek with a swab (resembling a q tip) which we will provide and can be undertaken without the need to visit your doctor.
